The neck of the bladder, or the posterior, division of the urethra, em braces that portion of the latter, which is situated between the external sphincter or the musculus compressor urethrw and the inner sphincter. The strength and the power of resistance of these two muscles are quite different. While the outer sphincter ofl'ers resistance, as well to the fluid secretion pressing from without onwards, as also to the fluid which endeavors to pass from within outwards, and while this resistance is overcome by the influence of the will, it 18 found, on the other hand, that the internal sphincter gives way before the slightest pressure, and therefore offers only a slight obstacle to either the passage of the urine from the bladder or back into the bladder from the posterior part of the urethra. The desire to urinate is due mostly to the fact that a large quantity of urine having collected in the bladder, the action of the inner sphincter is overcome and the urine then pressed into the neck. At the moment when the desire to micturate is greatest, the bladder and the neck become a common cavity, and the further progress of the urine is only hindered by the action of the will in keeping the external sphincter closed. The dissimilar action of these two sphincters is perceived when injections are thrown into the bladder. If we wish to force a fluid through the urethra into the bladder, without the aid of a catheter, and by means only of a hand syringe or an irrigator, we will find the greatest opposition at the musculus compressor urethroe, and in some cases it will be impossible to force the substance into the bladder, notwithstanding great force may be used, and, as a consequence of which, the fore-part of the urethra is painfully distended. Book excerpt: Excerpt from North Carolina Medical Journal, Vol. 15: Wilmington, January, 1885 That, in spite of this, however, the Pars prostatica belongs to the bladder, or vice versa, the Trigonum Lieutodii to the Prostata, shown alone by the fact that both the ridges of the Trigonum can be followed, anatomically, far into the Prostata, and moreover, that the muscular fibres lying next to the urethral means membrane in the Pars prostatica represent a continuation of the muscular structure of the bladder. Book excerpt: Excerpt from North Carolina Medical Journal, Vol. 19: January, 1887 Next, the point for operation being agreed upon, we must choose a special method of operating. For the leg we have a choice of many methods. Each surgeon can suit his fancy. Usually the method is determined by the kind of covering it will give the bones of the stump, or by the facility or celerity with which it can be executed, or by the evil consequences it enables us to avoid, or because of the like lihood of its yielding quick union. A purely circular incision, with tegumentary ap, used to be greatly in vogue, and is yet practiced by some. It is tedious to turn up the ap, like I do the cuff of my sleeve, and this cannot be done where the leg is muscular and conical. Such a ap comes together like the mouth of a purse, and this is pre ventive of union by the first intention. I never practice or recom mend such a method. I also am disposed to reject antero-posterior aps, whether tegumentary or in part muscular, for the edge of the cut tibia is apt to induce ulceration of the anterior ap, or the weight of the posterior ap prevents union. There is greater danger of necrosis of the bone and of a conical stump after this method. I rather prefer the lateral skin aps of equal size, or the hood-flap of Dr. Stephen Smith of New York. This latter, however, is not as easily turned up as are the lateral aps, and moreover, when the sutures are adjusted, it is found that there is a super uity of tissue, above and over the tibia, which rather opposes primary union, and does not readily permit of the covering adapting itself to the bones. Of the total mortal ity. Volkmann has called attention to the profuse parenchymatous oozing which frequently attends this operation, and which has done so much towards adding to its great mortality. Enough has been said to show the necessity of taking special precautions in performing this operation to prevent by appropriate measures the loss of blood. The femoral artery is not the only source of dangerous hemorrhage. It may come from the profunda femoris, which may have a high origin, from the obturator, ischiatic or circumflex, the femoral vein, occasionally quite severe, and Volkmann has reported one case in which two pints of blood were lost by the parenchymatous bleeding, which is often quite severe.
