Book excerpt: Many noninvasive examination methods of the heart have not held out against the invasive methods, which modern cardiac therapy, surgically or with catheterization, requires. They have disappeared completely or are only used by isolated groups of researchers. However, there is an obvious tendency to apply the invasive procedures as the last diagnostic possibility. In the attempt to select clinically relevant methods, the expert authors of this book demonstrate that echocardiography, expanded with contrast and Doppler, has been developed into one of the most important noninvasive methods. The results with tissue characterization show that the possibilities of this method have not yet been fully explored. Nuclear procedures are widely used, although they should only be applied in direct connection with clinical cardiology. The new lead methods of the ecg, such as ecg-mapping, show that standard electrocardiography of electrical functions is not yet fully exploited. The rapidly developing method of computer tomography is also being applied to cardiology. Since nuclear magnetic resonance requires extensive equipment con struction, its future is as yet unsure. Of course, a book like this does not intend to treat the subject of noninvasive cardiology in extensive detail. Established methods like standard electrocardio graphy, phonocardiography and sphygmography are not discussed. The aim of this book is rather to demonstrate the trend of present developments in the field. The pediatric cardiac sonographer has used 2D and TM imaging for several years to study the structural abnormalities associated with various lesions. Pulsed Doppler, continuous wave Doppler, and more recently, color flow mapping techniques have been used to study the pathophysiology, and to make quantitative measurements thereof. To those already working in pediatric echocardiography, this book is intended to serve as a guide to the applications of the various Doppler modalities in specific abnormalities. The other group for which this text is intended are the adult cardiac sonographers who see the occasional patient with congenital heart disease in their echocardiography laboratories. The incidence ofcongenital heart disease is not decreasing. As more patients survive to child bearing age, the adult cardiologist sees more and more referrals from the pediatric cardiologist to follow up the parent with corrected congenital heart disease, and in many smaller institutions, to examine their offspring as well. Since cardiovascular nuclear imaging emerged as a new subspecialty in the mid-1970s, the field has gone through an explosive growth. Radionuclide techniques became readily recognized as important new diagnostic aids in the armamentarium of the clinical cardiologist. Initially, cardiovascular nuclear imaging focused on static myocardial imaging using either thallium-201 or technetium-99m-pyrophosphate for diagnosing acute myocardial infarction. Shortly thereafter, multigated equilibrium radionuclide angiocardiography became the most widely used noninvasive method for assessing cardiac function. Furthermore, attention and clinical application shifted towards the use of radionuclide techniques in conjunction with exercise testing, either with thallium-20 1 myocardial perfusion imaging or technetium-99m left ventricular function studies. The future of cardiovascular nuclear imaging appeared exciting and promising. However, around 1980 pessimists predicted the premature demise of cardiovascular nuclear imaging with the introduction of digital subtraction angiography and nuclear magnetic resonance imaging. These doomsayers have been proven wrong: in 1985 cardiovascular nuclear imaging is thriving and, in many centers, even expanding. Although digital substraction angiography and magnetic resonance imaging provided exquisite anatomic detail, for practical evaluation of patients with ischemic heart disease - in the Coronary Care Unit or exercise laboratory - nuclear techniques appeared to be more practical.
