Book excerpt: Noninvasive brain stimulation (NIBS) technologies have been applied to study brain physiology and, more recently, have been recognized for their therapeutic potential as an adjunctive treatment for various neurologic and psychiatric disorders. Transcranial magnetic stimulation (TMS) and transcranial electric stimulation (tES) are two of the most studied NIBS modalities in Parkinson's disease. They are non-systemic and relatively safe. Most therapeutic trials have been conducted to ameliorate motor symptoms of Parkinson's disease (PD) with overall positive results using various stimulation modalities and methods. Notwithstanding significant results, evidence has not yet been compelling mainly due to small-size studies, lack of standardization of methodologies and other study design limitations. NIBS hold promise for treatment of PD symptoms and PD related complications. Large, well designed clinical trials are needed to corroborate these positive findings and inform its durability and the overall clinical relevance for the treatment of PD.

Clinical applications include, but are not limited to, common neurological disorders such as Parkinson’s disease and other movement disorders. The bulk of evidence supporting the clinical efficacy of various invasive and non-invasive approaches for neuromodulation has progressively led scientific societies, patients’ associations, and regulatory entities to acknowledge the critical role played by neuromodulation in the therapeutic algorithms of a wide range of neurological disorders. As a result, new technologies have been recently introduced into the market or are currently under validation. Their potential implementation into innovative protocols for neuromodulation demands a critical revision of what are the unmet needs for neuromodulation in movement disorders.

Similar to Parkinson's disease, essential tremor, dystonia, Huntington's disease, and other aging-related movement disorders are widespread in the elderly with relatively unideal medicine-treatment effects. In recent years, deep brain stimulation (DBS) has been established as an effective treatment for PD and other aging-related movement disorders, especially with motor symptoms. However, there are still many problems to be solved in research work and clinical practice in this field. For example, it is unclear regarding the most effective stimulation targets or the underlying mechanism of DBS. And it is still unknown how to choose the optimal targets among STN, GPi, PSA, VIM, etc, or if we could choose the nerve fiber for stimulation besides the nuclei, And despite wearable sensors could be a powerful tool for the digital evaluation of movement disorders or provide the feedback biomarkers for DBS, the closed-loop biomarkers for DBS are still lacking. Furthermore, apart from PD, the DBS treatment in gene-related dystonia, Huntington's disease, and other movement disorders also needs to be further explored.

Book excerpt: The aim of this study was to clarify and compare the efficacies of rehabilitation using transcranial direct current stimulation (tDCS) and continuous theta burst stimulation (cTBS), a form of repetitive transcranial magnetic stimulation (rTMS), in convalescing stroke and Parkinson's disease patients. For both types of stimuli, kinetic analysis and performance analysis of upper limb motor paralysis and gait analysis showed an increase in speed of movement, and an improvement in performance was observed. Both stimuli resulted in significant improvement compared with a sham stimulus. Change in speed of movement and performance was observed with both tDCS and cTBS, but there was not a significantly large difference between the stimuli. Improved movement due to reduction of excessive tension caused by spasticity was observed. In patients with Parkinson's disease, gait speed and step length were increased. It is suggested that performance was improved because movement became smoother. The efficacy of tDCS and cTBS in patients with motor disorders caused by stroke or Parkinson's disease will probably be further improved when combined with physical therapy.

Given the growing interest in non-invasive neuromodulation technologies, the Institute of Medicine's Forum on Neuroscience and Nervous System Disorders convened a workshop, inviting a range of stakeholders - including developers of devices and new technologies, researchers, clinicians, ethicists, regulators, and payers - to explore the opportunities, challenges, and ethical questions surrounding the development, regulation, and reimbursement of these devices for the treatment of nervous system disorders as well as for non-therapeutic uses, including cognitive and functional enhancement. This report highlights the presentation and discussion of the workshop.
