However, the degree of the demands on the personal ity of the surgeon who has to carry out very serious operations to the exclusion of all others and has little opportuinity to recover psycholog ically by working on simpler cases is concealed from the outer world. The mental strain which the coworkers and staff have to tolerate may not be denied". So wrote Wilhelm Tonnis in 1939. Although fifty years have now elapsed, his appraisal is still relevant today - hence my intention to discuss the everyday demands placed on neurosurgeons. My aim is not self-glorification to engender sympathy; rather it is to highlight the effects and repercussions of such demands for the well-being of the patients entrusted to us. The major onerous demands include: - those in the operating theater - those resulting from staff problems in nursing - those resulting from legal developments - those resulting from the increasing administrative tasks that cost valuable time and energy which are lost to our actual work in look ing after patients: science and research also suffer from this. I shall not go into the latter point within this preface. Let me first turn to the strains occurring in the operating theater.

Ljunggren's opening historical survey set the stage and must be the most comprehensive yet published. Subarachnoid hemorrhage from aneurysm rupture still constitutes, I dare say, the most difficult problem for neurosurgeons, in relief of the brain injury and arterial reaction and the technical perfection of aneurysm obliteration, even for small, as well as large and giant sacs. Very large high flow A VMs can be as demanding too. The bulk of the conference was devoted to subarachnoid hemorrhage, aneurysms and A VMs which were discussed under about 14 headings each. But vein of Galen malformations, dural A VMs, cavernous angiomas and venous angiomas (renamed developmental venous anomalies) came under scrutiny, not always with consensus. Trends are perceptible such as fibrinolysis of subarachnoid clot, non surgery for Galenic and dural malforma tions, the benignity of venous angioma, but there is still much variation in approach, pharmacologically, technically and with such as the evolving endovascular and radiosurgical stories, used alone or in conjunction.

The condition was sufficiently troubling to classical philosophers and jurists to have apparently provoked comments by Solon in approximately 500 B. C. and Plato in the fourth century B. C. (Plutarch 1967 translation; Plato 1921 translation). Medical recognition can be traced at least as far back as the second century A. D. (Adams 1861). However, several factors have converged in this century to extend the absolute dimensions of the problem of senile dementia and to increase societal, medical, and scientific recogni tion of the magnitude of the condition. Perhaps the most important factor relating to the present importance of senile dementia is demographic. Although the human population has been increasing since the mid-eighteenth century, it has only been since the advent of the twentieth century that a decrease in mortality has been noted for those over the age of 45 (McKeown 1976). Consequently, the absolute number of aged persons and the proportion of increasingly aged persons in the populations of the world's industrial nations have been steadily increasing. For example, in the United States, 4% ofthe population was over the age of 65 in 1900. In the 1970 census, this proportion had grown to 10%.
