He outlines an alternative to the disease model defined by nine dimensions of dysfunction that encompass the range of human dysfunctions typically classified as mental disorders. Gallon explains the history of psychiatry and how it came to develop a medical model that codifies mental disorders in the psychiatric bible, the Diagnostic and Statistical Manual of Mental Disorders (DSM), now in its fifth edition. He demonstrates how, in 1950s and 1960s when the miracle psychiatric drugs came on the market, it was to the great economic advantage of both pharmaceutical companies and psychiatrists to describe people's problems in the language of medicine. His alternative to this disease model suggests descriptive types--Reality Misperception, Mood Dysfunction, Anxiety, Cognitive Competence, Social Competence, Somatoform Dysfunction, Substance Dependence, Motivation and Impulse Control, and Socialization Dysfunction--that we can construct to discuss the kinds and severities of problems people experience. US college enrollments swelled from 2.6 million students in 1955 to 17.5 million by 2005. Ivy League universities, specifically Harvard, Yale, and Princeton, face significant challenges in maintaining their professed goal to educate a reasonable number of students from all ethnic, racial, religious, and socio-economic groups while maintaining the loyalty of their alumni. College admissions officers in these elite universities have the daunting task of selecting a balanced student body. Added to their challenges, the economic recession of 2008-2009 negatively impacted potential applicants from lower-income families. Evidence suggests that high Standard Aptitude Test (SAT) scores are correlated with a family’s socioeconomic status. Thus, the problem of selecting the "best" students from an ever-increasing pool of applicants may render standardized admissions tests a less desirable selection mechanism. The next admissions battle may be whether well-endowed universities should commit themselves to a form of class-based affirmative action in order to balance the socioeconomic advantages of well-to-do families. Such a policy would improve prospects for students who may have ambitions for an education that is beyond their reach without preferential treatment. As in past decades, admissions policies may remain a question of balances and preferences. Nevertheless, the elite universities are handling admission decisions with determination and far less prejudice than in earlier eras.

After looking at the treatment of gays in the decades after World War II and the birth of the modern gay rights movement with the Stonewall Rebellion in 1969, Klarman describes the key legal cases involving gay marriage and the dramatic political backlashes they ignited. He examines the Hawaii Supreme Court's ruling in 1993, which sparked a vast political backlash--with more than 35 states and Congress enacting defense-of-marriage acts--and the Massachusetts decision in Goodridge in 2003, which inspired more than 25 states to adopt constitutional bans on same-sex marriage. In the end, Klarman discusses how gay marriage is likely to evolve in the future, predicts how the U.S. Supreme Court might ultimately resolve the issue, and assesses the costs and benefits of activists pursuing social reforms such as gay marriage through the courts.
