Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from The New York Medical Journal, Vol. 40: A Weekly Review of Medicine; July to December, 1884, Inclusive I recently saw a case which presented the features of a hybrid general paresis with bulbar, and finally very marked and persistent renal symptoms, the amount of urine voided daily exceeding twenty ounces at times, while at others it was scanty, but contained albumin or sugar at all times. During the past six weeks the patient's legs and feet have been the seat of a puffy swelling, the skin being red and glossy; and there was very slight pitting on pressure, which disappeared in the course of a few days. The patient's dis ease began with embarrassment in articulation, pupillary alteration, tremor of the tongue, and finally of the whole body, and then came the delusions of grandeur, which were of the most extravagant nature, and he became salacious in the extreme. Nearly all of the cranial nerves were affected. There were facial anaesthesia, impairment of smell and taste, and very curious vaso-motor changes, which consisted mainly in a discoloration of the skin of the left arm and hand. From time to time I found sugar in his urine, and this seemed to be the case after an attack of excitement. With the kind assistance of Dr. Landon, of the Hudson River State Hospital for the Insane, I am enabled to present the records of urinary analyses in twelve cases of general paresis of the insane, which were made every day for fifteen days. In these patients the bulbar symptoms were marked and the urine was increased in quantity. In all there was a varying amount of sugar. It may be stated that these patients were upon the same diet and treated precisely alike, so the results have more value than if the cases were selected at random. Of these twelve cases, it will be seen that in two there was considerable sugar, and in one case there had been some months previously a large quantity found. In Case 6 both albumin and sugar were found. On ten occasions the former was present, and on five absent. It is to be noted that, as a rule, when albumin was present the sugar was not to be found, and nice versa. The quantity of sugar seemed to bear no important relation to the quantity of urine, and on days when no sugar was found the patient often passed from 50 to 60 oz. In some of the cases, however, this was not so. About the Publisher Forgotten Books publishes hundreds of thousands of rare and classic books. He said that about one month previous he first noticed slight soreness on the inner side of the right cheek, close to the upper jaw, and located the point about opposite the second molar tooth. On examination, I found on the roof of his mouth, close to that tooth, and at about the centre of the roof, patches of what appeared to be false membrane, which could be stripped OE readily, and left a raw, bleed ing surface. Further examination revealed the same kind of patches on the epiglottis. About the Publisher Forgotten Books publishes hundreds of thousands of rare and classic books. Download or read book The New York Medical Journal Vol 57 written by Frank P. Foster and published by Forgotten Books. This book was released on 2018-01-31 with total page 646 pages. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from The New York Medical Journal, Vol. 57: A Weekly Review of Medicine; January to June, 1893, Inclusive When codeine is relinquished it is advisable that the patient be relieved of ordinary duties for a few days, and avoid all sources of annoyance as far as practicable. But strict confinement and enforced inactivity should be avoided if possible. On the contrary, there are times when vigor ous exercise will relieve restlessness, especially calisthenics before going to bed. In conclusion, let me say that if any one subject to the morphine habit expects any golden specific to bear him back to health without some effort and some sacrifice he will be disappointed. It is hardly possible in the nature of things that the laws of conservation and compensation of the material world should have no counterpart in conditions governing the nutrition of nerves of sense. The sole advantage of the treatment above given lies in the fact that the penalty may be paid in installments. This is not possible by any gradual reduction of morphine alone, as the supreme difficulty consists in renouncing the last traces. To those who may attempt for the first time a cure by this method, the inconvenience may seem greater than this account would lead one to suppose. Let them try any other method generally practiced. It is chiefly by compari son with other methods that its value becomes apparent, and in this respect it is difficult to speak of it in terms of moderation. Book excerpt: Excerpt from New York Medical Journal, Vol. 112: Incorporating the Philadelphia Medical Journal and the Medical News; A Weekly Review of Medicine; July to December, 1920, Inclusive Cream, good butter (without butyric odor), and a little olive oil are the most suitable fats. Yolks of eggs are rich in fats, but should be used spar ingly as they often cause distress. Fats, while somewhat diminishing acidity, tend to prolong diges tion, and consequently must be given with some caution. Moreover, there is no advantage in giving fats so freely that they disturb the intestinal diges tion, and pass off in large quantities by rectum.
