Book excerpt: The purpose of this text is to help develop a better understanding of the nature of cancer, to expand prevention strategies, and to improve diagnostic and prognostic skills centred around multiple primary cancers. Clinical studies focusing on the aetiology of multiple primary cancers, statistical data presented in tables, and discussions on various organ sites and the possible occurrence of multiple primary cancers are addressed throughout the text. The first part reviews various methodological and statistical considerations in the study, management and prevention of primary, secondary and multiple primary cancers. An entire chapter contains tables of incidence rates of various multiple primary cancers that prove helpful in subsequent chapters. Part Two discusses predisposing genetic conditions and various forms of environmental factors that are associated with multiple primary cancers. Finally, the last section review selected types of cancers and focuses on possible clinical implications for occurrences of specific multiple primary cancers.

Book excerpt: A second primary cancer (SPC) is a new primary cancer developing in a person with a history of cancer at a new site or tissue and subsequent to the initial cancer. Advances in cancer treatment have led to marked improvements in cancer cure rates over the past 30 years, and the control of risk factors has further increased longevity. Still, cancer survivors are at increased risk not only for recurrence of the primary cancer but also for the development of second primary malignancies, the latter being especially true for survivors of childhood cancer. SPCs are now more common, and research into them has intensified.This monograph provides a synthesis of the current research on SPC, culminating in pertinent summary charts, covering the whole spectrum of first and second primary cancers and the association between them. Readers will gain a general understanding of the epidemiology and of the excess risk of SPCs following an implicated first malignancy. The known or suspected etiologic factors for SPCs are identified, and the field is further narrowed to those factors that are modifiable in practice. Among endocrine adverse effects, severe hypothalamic damage may be considered the most harmful in survivors, leading to morbid obesity, propensity to metabolic syndrome and cardiovascular disease. This book aims to disseminate the knowledge about endocrine and metabolic adverse effects of cancer therapies and about survivorship care. Since the number of cancer survivors is steadily growing in the general population, this publication is intended not only for endocrinologists but also for oncologists, onco-hematologists, internists, pediatric specialists in those areas and general practitioners, with the aim to better counsel and monitor cancer survivors.
