However, pain can become chronic due to a multitude of pathophysiological states, such as: trauma, inflammation, neural injury, viral infection, cancer, autoimmune diseases and vascular and metabolic disorders. These pathological states can trigger alterations of the pain pathways that can lead to hypersensitivity, and in such circumstances, pain loses its protective role and instead, becomes persistent and debilitating affecting seriously to the quality of life of patients. Chronic pain is one of the most important health problems worldwide. It has been estimated that 10% of adults are diagnosed with chronic pain each year. However, despite the high prevalence of chronic pain, its management is still no fully satisfactory probably due to the variety of chronic pain conditions with different etiologies (neuropathic, visceral and musculoskeletal pain), and because their pathophysiological mechanisms are only partially known. Therefore, there is a huge need for new effective therapies for the control and/or prevention of the different types of chronic pain. Chronic pain is associated with plastic changes in pain circuits of the peripheral and central nervous system, including changes in the expression of ion channels, neurotransmitters and receptors. In addition, there is growing consensus on non-neuronal mechanisms that can amplify or resolve chronic pain, and cells traditionally thought to act just as coordinators of the inflammatory response (such as immune and glial cells) are now accepted as real modulators of pain signaling. In this regard, animal models of persistent pain, such as models of neuropathic and visceral pain and models of arthritis, are helping to elucidate our understanding of the pathogenesis of chronic pain and make it possible to test experimental treatments.

Acute pain, such as occurs with trauma, often has a reversible cause and may require only transient measures and correction of the underlying problem. In contrast, chronic pain often results from conditions that are difficult to diagnose and treat, and that may take a long time to reverse. Some examples include cancer, neuropathy, and referred pain. Often, pain pathways (nociceptors) are set up that continue to transmit the sensation of pain even though the underlying condition or injury that originally caused pain has been healed. In such situations, the pain itself is frequently managed separately from the underlying condition of which it is a symptom, or the goal of treatment is to manage the pain with no treatment of any underlying condition (e.g. if the underlying condition has resolved or if no identifiable source of the pain can be found.Pain management generally benefits from a multidisciplinary approach that includes pharmacologic measures (analgesics such as narcotics or NSAIDs and pain modifiers such as tricyclic antidepressants or anticonvulsants), non-pharmacologic measures (such as interventional procedures, physical therapy and physical exercise, application of ice and/or heat), and psychological measures (such as biofeedback and cognitive therapy). Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

The 2010 Patient Protection and Affordable Care Act required the Department of Health and Human Services (HHS) to enlist the Institute of Medicine (IOM) in examining pain as a public health problem. In this report, the IOM offers a blueprint for action in transforming prevention, care, education, and research, with the goal of providing relief for people with pain in America. To reach the vast multitude of people with various types of pain, the nation must adopt a population-level prevention and management strategy. The IOM recommends that HHS develop a comprehensive plan with specific goals, actions, and timeframes. Better data are needed to help shape efforts, especially on the groups of people currently underdiagnosed and undertreated, and the IOM encourages federal and state agencies and private organizations to accelerate the collection of data on pain incidence, prevalence, and treatments. Because pain varies from patient to patient, healthcare providers should increasingly aim at tailoring pain care to each person's experience, and self-management of pain should be promoted. In addition, because there are major gaps in knowledge about pain across health care and society alike, the IOM recommends that federal agencies and other stakeholders redesign education programs to bridge these gaps. Pain is a major driver for visits to physicians, a major reason for taking medications, a major cause of disability, and a key factor in quality of life and productivity. Given the burden of pain in human lives, dollars, and social consequences, relieving pain should be a national priority.
