Book excerpt: Introduction of new diagnostic methods including various imaging techniques in the field of orthopaedic surgery has enabled us to make early and precise diagnoses of musculoskeletal tumors. In addition, recent progress in the chemotherapy and radiotherapy of malignant tumors has raised the five-year survival rate remarkably, although complete recovery from these treatments alone cannot be expected as yet. These medical advances naturally stimulate the search for ways of limb salvage even in malignant musculoskeletal oncology. To this pur pose, not only oncologists, but orthopaedic surgeons, radiologists, engineers, and biomaterials scientists should work together to design suitable surgical procedures and prostheses using autograft, allograft and biomaterials. The First International Workshop on the Design and Application of Tumor Prostheses for Bone and Joint Reconstruction was held in 1981 in Rochester, Minnesota. Participants were of an interdisciplinary nature in order to establish a basis for cooperative efforts, which have subsequently spread throughout the world. Since then, biennial inter national symposia have been held successively in Vienna, Florida, and Kyoto. Previous gatherings of this prestigious series were open only to a small number of experts.

In 1989 the Fifth International Symposium On Limb Salvage (ISOLS) was held in Saint Malo and was attended by more than 300 participants from 34 different countries. The 105 papers presented, grouped under eleven main headings, have been included in this book. Limb salvage has indeed progressed in the years since the first symposium. Initially it essentially concerned tumours and the main aim was to salvage the diseased limb through non-mutilating surgery without jeopardizing the patient's prospects of survival. Now, with a confirmed high rate of 5-year disease-free survival, new goals can be set: improvement of the functional results and of the survival of our reconstructions. How can this be achieved: - By favouring muscle reattachment on the prosthesis, for instance through the use of a bone allograft-sleeved composite prosthesis? - By improving the survival of the arthroplasty through the develop ment of a more biological fixation of its stems? - By limiting the side effects of the necessary adjuvant treatments on the function of the allograft as well as on the fixation of the prosthesis? In addition, limb salvage procedures now concern not only primary tumours (and metastases, which must be treated using the same osteosynthetic and prosthetic techniques), but also reconstructive orthopaedic surgery as a whole.

During this period significant progress has been made in diagnosis, and particularly in staging and treatment of bone tumors. In most centers, limb-sparing surgery has replaced the more radical ablative surgery, with or without preoperative chemotherapy. This became possible due to the introduction and increased application of newer high-quality imaging techniques (CT and MRI) and in particular the use of new cytomorphologic methods of diagnosis, especially immunohisto chemistry. These advances in diagnosis and treatment have been reflected by the publication of new editions of several important text books, since their authors - pathologists, orthopedic surgeons, and radiologists - felt the necessity of updating their works in recent years. However, in these publications different criteria of classification were adopted. The number of cases compiled at the Latin American Registry of Bone Pathology increased from 4913 primary bone tumors and more than 1100 tumorlike lesions at the time of publication of the first edition of this book, to 5274 bone tumors and 1665 tumorlike lesions by the end of December 1985, when I finished my work as head of the Registry and occupied the positions of Professor of Orthopedic Pathology at St. Louis University Medical Center and Distinguished Visiting Professor at Rush-Presbyterian-St. Luke's Medical Center in Chicago.

Book excerpt: Among the standard oncology modalities, surgical oncology is singular in that it lacks a separate board certification or even an added qualification mecha nism. 'Card-carrying' surgical oncologists are certified by the American Board of Surgery, as are all other general surgeons. What distinguishes the surgical oncologist is a set of cognitive skills rather than a specific armamentarium of surgical techniques. This different conceptual framework is derived from ex tensive additional training that leads to an in-depth understanding of the natural history and biologic behavior of the various solid tumor systems. Equipped with this perspective, the surgical oncologist is particularly well positioned to integrate the various available therapeutic modalities into a coherent care program for the solid tumor patient. As a central theme, the chapters of this book demonstrate that increasingly sophisticated diagnostic and staging approaches are helping to move chemo therapy and radiotherapy into the preoperative neoadjuvant setting. This fundamental alteration is based on the awareness that even early-stage solid tumor disease is frequently systemic at the time of presentation, at least on a subclinical level. And although the primary tumor may be controllable by surgery with radiotherapy, the uncontrolled (and initially clinically unappar ent) distant disease ultimately determines patient survival. The other perspec tive driving the neoadjuvant approach is an emerging awareness that for most solid tumor systems, neoadjuvant treatment responses can facilitate less muti lating surgery with comparable levels of local disease control.
