The female athlete has a complex system of steroid hormones that are continually changing. These sex hormones that fluctuate throughout each month may influence knee injuries, specifically the anterior cruciate ligament. The increased incidence in women is thought to be multifactorial, a combination of structural, anatomical, or biomechanical factors. The NCAA has reported that 75 percent of anterior cruciate ligament injuries are non-contact in competitive jumping or pivoting sports. In this study, the effects of the menstrual cycle on neuromuscular performance were investigated. Fifteen healthy females with regular menstrual cycles completed the various tests of this study for three phases of the menstrual cycle. All females were categorized as moderate or vigorous exercisers from an activity questionnaire. This study used a repeated measures experimental design; therefore, each participant served as her own control. The participants completed a series of two tests, including functional balance and fatigability.

Book excerpt: Background: The menstrual cycle is a complex physiological process that is characterized by continually fluctuating levels of hormones. The effects of the menstrual cycle on athletic performance are not well understood by coaches, parents, athletes, and sports medicine practitioners, partly because the existing literature in this area is widely varied and does not provide concrete answers. Purpose: The purpose of this paper is to provide an updated review of the existing literature that explores the relationships between the menstrual cycle and various aspects of athletic performance. Data Sources: The MEDLINE, EBSCO, PubMed, and Web of Science databases from January1, 1980 through December, 31 2017 were searched for English language articles. Study Selection: Research studies regarding the menstrual cycle's effects on strength, power, sprint, and endurance performance were selected for analysis. Data Extraction: A standardized protocol with predefined inclusion criteria was developed and used to extract details about participants, methods, interventions, comparisons, outcomes, results, and study designs from each study. Studies were also evaluated for overall quality and applicability. Data Synthesis: 51 studies were selected for analysis and subdivided into topical categories. Due to the wide variety in PICOS among the selection, a qualitative approach was used to synthesize the data. The majority of studies were experimental, repeated measures in design and used exercise tests to evaluate the MC's effect on athletic performance. Many studies had poor methodologic quality, threatening their validity. The majority of available evidence did not control for all potentially influential variables to a satisfactory degree. Overall, many studies contradicted one another in results, and clear relationships between the MC and athletic performance could not be established. Conclusions: The evidence provided by the existing body of literature from direct comparisons of interventions, outcomes, and results cannot be used to draw robust conclusions. Available evidence does not clearly support a relationship between the MC and strength or endurance performance. In addition, it appears that the MC has no significant impact on anaerobic or sprint performance. Future research and applications would benefit from the standardization of methodological protocols, including verification of MC phase and ovulation.

Book excerpt: Since the observation in the 19th century that an extract of the suprarenal bodies injected into the circulation caused a rise in blood pressure, the endocrine system has become a major component in our understanding of human physiology. The introduction of radioimmunoassay techniques and the ability to measure minimal amounts of hor mones (a term derived from the Greek "to excite") have shown that acute exercise causes a release of a large number of hormones and that chronic exercise may further lead to long-term alterations in endocrine homeostasis. Actually, almost every organ and system in the body is affected by physical activity and exercise, much of it through the endocrine and neuroendocrine system. Investigation ofthe effect of acute or chronic physical activity on the endocrine system is a complex matter since the stimulus called "exercise" has many components, such as mode, intensity, duration, and others. In addition, several other factors, such as age, gender, training status, body temperature, circadian rhythm, metabolic state, menstrual cycle, and various external conditions as well as psychological factors, can modify the effect of physical activity on hormonal secretion. Moreover, the physiol9gical stimulus of exercise often provokes several and parallel cascades of biochemical and endocrine changes. It is therefore often extremely difficult to distinguish between primary and secondary events and between cause and effect. These limitations will be discussed in Chapter 1.
