The female athlete has a complex system of steroid hormones that are continually changing. These sex hormones that fluctuate throughout each month may influence knee injuries, specifically the anterior cruciate ligament. The increased incidence in women is thought to be multifactorial, a combination of structural, anatomical, or biomechanical factors. The NCAA has reported that 75 percent of anterior cruciate ligament injuries are non-contact in competitive jumping or pivoting sports. In this study, the effects of the menstrual cycle on neuromuscular performance were investigated. Fifteen healthy females with regular menstrual cycles completed the various tests of this study for three phases of the menstrual cycle. All females were categorized as moderate or vigorous exercisers from an activity questionnaire. This study used a repeated measures experimental design; therefore, each participant served as her own control. The participants completed a series of two tests, including functional balance and fatigability.

Book excerpt: The purpose of this study was to examine the effects of an 8 week neuromuscular training program on lower extremity knee valgus angle in African American athletes. We hypothesized a neuromuscular training program would signficantly decrease lower extremity knee valgus angles during a drop vertical jump at landing compared to a control group. Fifteen female volleyball, track, softball players underwent an 8 week intervention neuromuscular training program that included three main components. While eleven controls underwent the resistance training protocol for eight weeks. With this directional hypothesis we used a T-Test on the difference between tests. Intervention athletes pre- and posttest 2-dimensional analysis demonstrated decreased knee valgus angles during a drop vertical jump landing maneuver. Intervention mean was a 9.4 degree decrease in valgus from pretest and control mean was 0.6 degree decrease in valgus from pretest. The subjects undergoing the neuromuscular training program demonstrated a significantly greater decrease in knee valgus angle compared to controls. Measurements taken from the suibjects with 2-dimensional motion analysis were used to calculate the knee flexion range of motion during landing from a drop box vertical jump. Maximum knee flexion range of motion increased fro 74.7 plus or minus 1.9 to 78.5 plus or minus 1.6. Control subjects did not demonstrate significant changes in knee valgus or knee flexion anlges during the 8 week protocol. The results of this study support the hypothesis that an 8 week neuromuscular training program that combines injury prevention-training components can decrease an injury risk factor such as knee valgus in African American female athletes.

Book excerpt: Introduction: Following the onset of maturation, female athletes have a significantly higher risk for anterior cruciate ligament injury when compared with male athletes. It is therefore important to evaluate this increased risk in this population and some possible preventative measures. Objective: To examine the effect of a neuromuscular exercise training (NME) program on hip strength and core stability in healthy adolescent females. Study Design: Randomized Clinical Trial which employed a repeated measures design. Participants: Healthy adolescent females aged 12-18 years old. Intervention: A 5-week progressive perturbation NME program was administered according to previously described and validated methodologies. The supervised program was administered 2x/week for 5 weeks. The program consisted of a series of destabilizing perturbations that were applied to the unstable surfaces with the participant in a unilateral stance. Outcome Measurements: Hip external rotation (ER) and hip abduction strength were evaluated using held-hand dynamometry. A dynamic seated stability test was used to evaluate core stability. Vertical jump test using the Vertex was used to evaluate lower extremity functional power and performance. Results: 40 healthy adolescent females (age 13.5 ± 0.2 years, height 165.3 ± 7.5 cm, weight 70.0 ± 25.3 kg, BMI 25.2 ± 6.6 kg/m2) were recruited. Two-way ANOVAs compared hip ER and abduction strength, core stability, and vertical jump pre- and post- NME training between the control and NME groups. Independent samples t-tests demonstrated that the percentage change in hip abduction strength and core stability was different between the control and NME group, with the NME group showing more improvement at follow-up. There was no significant relationship noted between strength scores and core stability or vertical jump performance. Conclusions: Overall, the results suggest the possibility of some impact as a result of the NME program, but more research is still needed to clarify the results of this and other investigations which have examined the efficacy of NME training programs in specific at-risk populations such as physically active adolescent females.

Neuromuscular control is a risk factor for musculoskeletal injury and likely fluctuates. Previous studies investigating the relationship between neuromuscular control and injury have typically utilized a single baseline neuromuscular control assessment with prospective injury monitoring, which does not capture potential variability in neuromuscular control that may influence injury risk. Preventive training programs (PTPs) are designed to improve neuromuscular control and can reduce lower extremity injury rates. Training load, or the volume and intensity of activity, may also modulate neuromuscular control. High training load can lead to fatigue, which has been shown to impair neuromuscular control. The relationship between neuromuscular control and training load has not been established. Purposes:1) Evaluate variability in neuromuscular control in adolescent female basketball athletes; 2) Evaluate the effect of PTPs on variability in neuromuscular control measures; 3) Examine the association of neuromuscular control and training load in adolescent female basketball athletes. Study Design: Cohort, observational (Purposes 1 and 3) and interventional (Purpose 2). Methods:Adolescent female basketball athletes (n=67) completed serial neuromuscular control and power assessments during the competitive basketball season. Participantsfrom one organization (n=47) were exposed to PTPs. An additional sample of adolescent female basketball athletes (n=15) completed neuromuscular control assessments before and after daily participation in multi-day basketball tournament. Training load was recorded for Purposes 1 and 3. Statistical Analyses:Repeated measures ANOVAs assessed neuromuscular control variables over time. Bivariate correlations were used to explore the relationship between neuromuscular control and training load. Results: Neuromuscular control varied over an athletic season and during a multi-day tournament. The role of PTPs in neuromuscular control variability is unclear. Findings regarding the relationship between neuromuscular control and training load during an athletic season were inconsistent. There was no association between training load and neuromuscular control during tournament play. Conclusion:Neuromuscular control is dynamic. Single-point neuromuscular control screenings should not be viewed as an absolute determinant of injury risk. Measures of neuromuscular control should be interpreted within the context of the individual and other injury risk factors.
