Available in PDF, EPUB and Kindle. Book excerpt: Rupture of the anterior cruciate ligament (ACL) is a devastating injury affecting approximately 250,000 individuals each year in the United States. Upwards of 130,000 anterior cruciate ligament reconstructions (ACLR) are performed each year. While ACLR restores mechanical stability of the knee, many patients continue to experience functional performance deficits, reduced capacity for physical activity and participation, and second injury. Current rehabilitation strategies may not be sufficiently addressing neurophysiologic alterations and psychological factors related to injury. An improved understanding of these factors may result in more effective post-operative rehabilitation protocols, fewer second injuries and safer return to sports. ☐ The overall goals of this work were: 1) to compare corticospinal, intracortical, and spinal-reflexive excitability in athletes after ACLR and controls, and explore the relationship of these neurophysiologic measures to quadriceps strength, 2) to explore the relationship between psychological readiness to return to sport and gait biomechanics, and 3) to determine if a secondary ACL injury prevention program affects psychological readiness to return to sport and if an improvement in psychological readiness is associated with better outcomes. ☐ This work includes data from two distinct studies. For goal #1, athletes after ACLR underwent neurophysiologic testing via transcranial magnetic stimulation and peripheral electrical stimulation when they achieved 3 important rehabilitation milestones: 1) 2 weeks after surgery, 2) achievement of a “quiet knee” defined as full range of motion and minimal effusion, 3) return to sport (RTS) activities time point defined as achievement of a quadriceps index ≥ 80% and at least 12 weeks post-ACLR. For goals #2 and #3, athletes after ACLR were enrolled after completing impairment resolution defined as: full range of motion, minimal effusion, adequate quadriceps strength and greater than 12 weeks from surgery. Following enrollment, all athletes completed a secondary ACL injury prevention program. Psychological readiness to return to sport and self-reported functional measures were evaluated at enrollment, after the prevention program, and at 1 year after ACLR. ☐ Athletes after ACLR demonstrated alterations in corticospinal, and intracortical excitability early after surgery compared to controls, and these neurophysiologic measures were associated with isometric quadriceps strength during the course of rehabilitation. Changes in these measures did not occur throughout rehabilitation indicating that current rehabilitation may not be addressing cortical alterations associated with chronic quadriceps dysfunction. ☐ For goals #2 and #3 we found that there is a relationship between knee kinematic and kinetic symmetry and psychological readiness to return to sport, prior to return to sport after ACLR. Athletes who scored lower on a psychological readiness to return to sport measure displayed greater asymmetries during gait. Additionally, athletes who demonstrated an improvement in psychological readiness to return to sport had better self-reported function following a secondary injury prevention program and at one year after ACLR. ☐ In conclusion, neurophysiologic pathways from the motor cortex to the quadriceps muscle and psychological factors are related to common impairments and outcomes after ACLR. While research is emerging in these two areas, more research is needed to address the less than optimal outcomes currently related to recovery after ACLR. Addressing cortical excitability and psychological readiness to return to sport during rehabilitation have the potential to improve outcomes after ACLR.

Book excerpt: Background: Anterior cruciate ligament injury (ACL) is one of the most common knee injuries in physically active populations. ACL injuries are typically treated by surgical reconstruction and post-surgical rehabilitation in the United States. However, even after completing rehabilitation and being cleared by physicians, there is a high percentage of re-injury risk in ACL injury patients. The high re-injury rate could be explained by neuromuscular deficits: which are one of the key factors which contribute to re-injury. In addition to the neuromuscular deficits, movement pattern and physical activity levels could be affected by psychological status after ACL reconstruction (ACLR). Kinesiophobia, fear of movement, is one of the psychological factors we can measure after ACLR. Elevated levels of kinesiophobia is reported in 61% of ACLR patients within 1-2 months after surgery. Also, kinesiophobia correlates with post-surgical activity level and return to play status. However, there is limited knowledge of association between kinesiophobia and knee motor control neural activity after ACLR. Purpose: The purpose of the current study was to determine the correlation between the Tampa Scale for kinesiophobia (TSK) and brain activation during hip-knee joint movement in ACLR patients. Method: Cross-sectional study, ten participants were included in current study (4 males and 6 females, 20.3±2.00 years, 1.72±0.11 m, 68.72 ±14.79 kg, 29.1±25.7 months from injury). All participants completed the TSK prior to the functional neuroimaging season. Functional magnetic resonance imaging (fMRI) technique was used to measure brain activation during hip-knee joint extension-flexion movements in a supine position. The task consisted of a total of four cycles for 30 seconds of activation and 30 seconds of rest. Result: Mean TSK score was 31.3±3.65 (range: 26-37). Book excerpt: Abstract: Anterior cruciate ligament (ACL) ruptures are common activity-related knee injuries usually requiring surgical reconstruction and extensive rehabilitative therapy to restore knee stability and function. Despite surgical reconstruction and physical rehabilitation, injury of the ACL dramatically increases the risk for a second injury (re-tear or contralateral leg), costly and long-term disabling osteoarthritis as well as decreased lifelong physical activity. The mechanism of this injury is typically non-contact, meaning the individual experiences a loss of neuromuscular control during jump landing, running or a change of direction maneuver without contact from another person or any other external force. Current interventions focus on biomechanical adaptations, mitigating their effectiveness to address the full neuromuscular control system. In spite of the plethora of biomechanical and patient data collected over the last three decades, the re-injury rate has remained high, if returning to activity, and patient dysfunction remains prolonged. Previous investigators have observed central nervous system and somatosensory deficits despite ACL reconstruction and rehabilitation, and we suggest that this is likely due to a neuroplastic adaptation not currently understood. This project connects highly dynamic measures of knee neuromuscular function with whole brain activation patterns to generate a brain-behavior model. The integration of neuroimaging and biomechanics in this investigation identifies the central nervous system components of ACL injury not accounted for in current assessment and intervention techniques. Quantification of the underlying neurological changes that may be driving the biomechanical outcomes after ACL injury will allow neural correlates of function to be targeted in rehabilitation.

Book excerpt: Traumatic knee injuries, such as anterior cruciate ligament (ACL) sprains detrimentally impact long-term health by initiating a cycle of chronic pain, inactivity, and disability. Neural activity alterations have been suggested as a large contributor to numerous aspects of reduced physical function including but not limited to persistent quadriceps strength deficits, aberrant biomechanical movement patterns and reduced self-reported function after ACL reconstruction (ACLR). Emerging evidence strongly points to the notion that psychological deficits also hinder recovery. Notably, neural impairments observed following ACLR may initiate psychological dysfunction, a process defined as learned helplessness (LH). This project will innovatively investigate the constructs of commonly affected after ACLR, including constructs of quadriceps strength, biomechanics, neural activity and psychological function, specifically focusing on establishing LH in this cohort. The central tenet of this work will establish the framework that after ACL injury and ACLR, patients experience alterations in afferent neural drive due to loss of mechanoreception, pain, and swelling, leading to subsequent alterations in central nervous system neural activity. This change in neural activity can either acutely foster an environment of uncontrollability promoting LH or persist, leading to a cyclical pattern of muscle inhibition and weakness, altered joint biomechanics, depressed self-reported and psychological function that generates the construct of LH in these patients; conversely, the change in neural activity can be restored, placing patients on the path to more positive outcomes. The negative psychological responses that ensue likely can further exacerbate the negative neural responses, creating a cyclical pattern that results in decreased overall quality of life in these patients. A natural extension of our need to comprehensively care for the patient after ACLR is to firstly continue to further understand the relationship between common post-operative outcomes and secondly to more thoroughly consider psychological health during ACL rehabilitation and to recognize LH as a potential barrier to successful recovery. By elucidating these relationships, we can begin to develop future evidence-based interventions capable of promoting both physical and psychological health and improved long-term quality of life.

Book excerpt: A patient with a left anterior cruciate ligament (ACL) reconstruction was seen for physical therapy treatment for 9 sessions over 8 weeks at an outpatient physical therapy clinic. Treatment was provided by a student physical therapist under the supervision of a licensed physical therapist. The patient was evaluated at the initial encounter with range of motion measurements, knee circumference measurements, the Lachman test, manual muscle tests, the Timed Up and Go test, the Five Times Sit to Stand test, the Lower Extremity Functional Scale, and the Lysholm score. A plan of care was then established. Main goals for the patient were to improve range of motion, increase strength, decrease knee swelling, increase functional mobility, improve functional transitional movements, and return to participation in hiking for recreation. Main interventions used were manual therapy, therapeutic exercises, neuromuscular electrical stimulation, modalities, patient education and gait training. The patient improved strength, gait mechanics, lower extremity function, mobility, transitional movements, and demonstrated decreased knee swelling. The patient continued to be seen at the outpatient physical therapy clinic for the duration of her care before she was discharged to continue living at home with a home exercise program.
