Book excerpt: The patent ductus arteriosus continues to pose a considerable challenge to clinicians and scientists alike. Why does it close spontaneously in most infants but remain open in others? How best can we select those infants who are most likely to benefit from treatment, i.e. are there echocardiographic criteria that would help in defining a more selective treatment approach? Would it be better to take an aggressive approach and prescribe prophylactic treatment to all extremely immature infants ‐ and if so, what is the best way to define such a subgroup? Or should we be more restrictive in defining treatment indications and adopt a 'wait and see' policy in most, if not all, premature infants? Finally, are there data to suggest that one of the treatment approaches that are available to close the patent ductus arteriosus is superior to the other? This book deals with these questions and tries to give some answers, based on the evidence currently available. It is intended for neonatologists, pediatric cardiologists and researchers.

Book excerpt: Any notion linguistically expressed, even one such as the syllable, is always the result of several different viewpoints. In order to take this into account, this book draws inspiration from the scheme of quaternion, as conceived by Sir William Rowan Hamilton and later introduced in theoretical linguistics by Ferdinand de Saussure. The first term of the quaternion (The Dawn of the Syllable) is provided by historical observations. The second term (Beyond the Sound of Syllables) is composed of different descriptive analyses of the syllable carried out in some particular languages and dialects. The third term (The Body of Syllables) presents the analytical-instrumental analysis of the syllable, while the fourth (De Syllaba Ventura) proposes some theoretical considerations.

Risk assessment of prenatally-induced adverse health effects is still a difficult task from both experimental data as well asfrom observations in humans. In the contributions to this book three major aspects are dealt with: - Quantitative extrapolations of experimental data to the situation possibly relevant for man. - The significance for a risk assessment with respect to man of minor or rare structural abnormalities observed in experimental studies - The future need to assess congenital dysfunctions (e.g. of the hormone or the immune system) beside the present evaulation of structural defects. Limitations as well as gaps of the present knowledge in this area of basic and applied research are pointed out. Since the results of prenatally-induced lesions may manifest themselves not only pre- but often not before late postnatally, numerous aspects of structural and functional abnormaldevelopment must be studied in experimental and clinical investigations.

Mit Ausnahme der 1985 erschienenen Aufsatzsammlung von Rooth und Saugstadt existieren zur Geschichte der Perinatalen Medizin nur Einzeldarstellungen unterschiedlicher Teilgebiete (z.B. fetale Überwachung sub partu, Ultraschall), entweder als Paper in Zeitschriften oder Kapitel in entsprechenden Lehrbüchern erschienen. Bislang gibt es jedoch kein Werk, das in einem integrativen Zusammenhang die Geschichte des noch jungen Fachs mit Schwerpunkt auf dem Beginn in den 1960er und frühen 1970er Jahren aber auch unter Würdigung wesentlicher Vorläufer und kurzer Darstellung der späteren Entwicklungen aus der Sicht einiger Beteiligter darstellt. Die Autoren des Bandes sind ausgewiesene Fachleute mit teilweise jahrzehntelanger Expertise auf den von ihnen beschriebenen Feldern.
