With this fifth annual National Healthcare Quality Report (NHQR), these reports will have provided more than 50,000 data points about health care quality in the United States. Has it made a difference? Have Federal and State governmental agencies, provider organizations, insurers, and employers made progress in improving health care quality and safety? While every previous release of the NHQR has attempted to summarize the direction in which health care quality is going, this fifth report tries to summarize the progress that has been made and the remaining challenges to improve health care quality in this Nation. The NHQR is built on 218 measures categorized across four dimensions of quality—effectiveness, patient safety, timeliness, and patient centeredness. This year's report focuses on the state of health care quality for a group of 41 core report measures that represent the most important and scientifically credible measures of quality for the Nation, as selected by the HHS Interagency Work Group. The distillation of 41 core measures for the 2007 report provides a more readily understandable summary and explanation of the key results derived from the data. While the measures selected for inclusion in the NHQR are derived from the most current scientific knowledge, this knowledge base is not evenly distributed across health care. The analysis in the following pages centers on measures for which data are available from the baseline year of 2000 or 2001 and the comparison year of 2004 or 2005. Three themes that emerge from the 2007 NHQR emphasize the need to accelerate progress in achieving high quality health care: Health care quality continues to improve, but the rate of improvement has slowed; Variation in quality of health care across the Nation is decreasing, but not for all measures; The safety of health care has improved since 2000, but more needs to be done.

As mandated by the U.S. Congress, the National Healthcare Quality Report (NHQR) focuses on "national trends in the quality of health care provided to the American people" (42 U.S.C. 299b-2(b)(2)). The National Healthcare Disparities Report (NHDR) focuses on "prevailing disparities in health care delivery as it relates to racial factors and socioeconomic factors in priority populations" (42 U.S.C. 299a-1(a)(6)). As in previous years, the 2013 NHQR and NHDR track more than 200 health care process, outcome, and access measures, covering a wide variety of conditions and settings. Data years vary across measures; most trend analyses include data points from 2000-2002 to 2010-2011. It is important to note that the reports provide a snapshot of health care prior to implementation of most of the health insurance expansions and consumer protections included in the Affordable Care Act and serve as a baseline against which to track progress in upcoming years. Each year, the reports emphasize one priority population; this year's reports provide expanded analyses of people with disabilities, including children with special health care needs and adults with multiple chronic conditions.

At the request of Congress, the Agency for Healthcare Research and Quality (AHRQ) annually produces the National Healthcare Quality Report (NHQR) and the National Healthcare Disparities Report (NHDR). The reports have revealed areas in which health care performance has improved over time, but they also have identified major shortcomings. After five years of producing the NHQR and NHDR, AHRQ asked the IOM for guidance on how to improve the next generation of reports. The IOM concludes that the NHQR and NHDR can be improved in ways that would make them more influential in promoting change in the health care system. In addition to being sources of data on past trends, the national healthcare reports can provide more detailed insights into current performance, establish the value of closing gaps in quality and equity, and project the time required to bridge those gaps at the current pace of improvement.

It is produced by the Agency for Healthcare Research and Quality (AHRQ) on behalf of the U.S. Department of Health and Human Services (HHS) and in collaboration with an HHS-wide Interagency Work Group. The NHDR provides a comprehensive national overview of disparities in health care among racial, ethnic, and socioeconomic groups in the general U.S. population and within specific priority populations, and it tracks the progress of activities to reduce disparities. The NHDR tracks disparities related to the quality of and access to health care. This fifth report attempts to answer the following question: Are we getting better at addressing disparities in the quality of and access to health care for priority populations in America? To do this, the report examines a set of 42 measures of quality and 8 measures of access. For each measure, the 2007 NHDR attempts to present a snapshot of the gaps between each racial, ethnic, and socioeconomic priority group and a comparison group. More importantly, where gaps exist, this report attempts to systematically discuss whether these gaps are getting bigger or smaller. Measures of health care quality address the extent to which providers and hospitals deliver evidence- based care for specific services, as well as the outcomes of the care provided. They are organized around four dimensions of quality-effectiveness, patient safety, timeliness, and patient centeredness-and cover four stages of care-staying healthy, getting better, living with illness or disability, and coping with the end of life. Measures of health care access include assessments of how easily patients are able to get needed health care and their actual use of services. They are organized around two dimensions of access-facilitators and barriers to care and health care utilization. The NHDR is complemented by its companion report, the National Healthcare Quality Report (NHQR), which uses the same quality measures as the NHDR to provide a comprehensive overview of the quality of health care in America. Both reports measure health care quality and track changes over time, but with different orientations. The NHQR addresses the current state of health care quality and the opportunities for improvement for all Americans as a whole. In the 2007 NHDR, three key themes emerge: Overall, disparities in health care quality and access are not getting smaller; Progress is being made, but many of the biggest gaps in quality and access have not been reduced; The problem of persistent uninsurance is a major barrier to reducing disparities.

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. Beginning in 2003, the Agency for Healthcare Research and Quality (AHRQ) will produce an annual report on the national trends in the quality of health care delivery in the United States. AHRQ commissioned the Institute of Medicine (IOM) to help develop a vision for this report that will allow national and state policy makers, providers, consumers, and the public at large to track trends in health care quality. Envisioning the National Health Care Quality Report offers a framework for health care quality, specific examples of the types of measures that should be included in the report, suggestions on the criteria for selecting measures, as well as advice on reaching the intended audiences. Its recommendations could help the national health care quality report to become a mainstay of our nation's effort to improve health care.

Comprehensive and accurate disease surveillance systems are needed to implement successful efforts which will reduce the burden of chronic diseases on the U.S. population. A number of sources of surveillance data-including population surveys, cohort studies, disease registries, administrative health data, and vital statistics-contribute critical information about chronic disease. But no central surveillance system provides the information needed to analyze how chronic disease impacts the U.S. population, to identify public health priorities, or to track the progress of preventive efforts. A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases outlines a conceptual framework for building a national chronic disease surveillance system focused primarily on cardiovascular and chronic lung diseases. This system should be capable of providing data on disparities in incidence and prevalence of the diseases by race, ethnicity, socioeconomic status, and geographic region, along with data on disease risk factors, clinical care delivery, and functional health outcomes. This coordinated surveillance system is needed to integrate and expand existing information across the multiple levels of decision making in order to generate actionable, timely knowledge for a range of stakeholders at the local, state or regional, and national levels. The recommendations presented in A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases focus on data collection, resource allocation, monitoring activities, and implementation. The report also recommends that systems evolve along with new knowledge about emerging risk factors, advancing technologies, and new understanding of the basis for disease. This report will inform decision-making among federal health agencies, especially the Department of Health and Human Services; public health and clinical practitioners; non-governmental organizations; and policy makers, among others.

This year's report focuses on the state of health care quality for a group of 46 “core” report measures which represent the most important and scientifically credible measures of quality for the Nation, as selected by the Department of Health and Human Services (HHS) Interagency Work Group. The distillation of 46 core measures for the 2005 report provides a more readily understandable summary and explanation of the key results derived from the data. Also included in the report are four new composite measures, which summarize data from a collection of individual measures. Composite measures were created for heart attack, heart failure, pneumonia, and patient centered care, in addition to an overall measure of the state of health care quality improvement. Four themes that emerge from the 2005 NHQR extend the meaning of those from the 2003 and 2004 reports and add new dimensions on understanding change over time: Health care quality continues to improve at a modest pace across most measures of quality; Health care quality improvement is variable, with notable areas of high performance; Health care quality is improving, but more remains to be done to achieve optimal quality; Sustained rates of quality improvement are possible.

Both reports' perspectives are needed for a complete understanding of quality of healthcare, and both reports support HHS Secretary Mike Leavitt's 500-Day Plan to fulfill the President's vision of a healthier America, specifically in the areas of better transparency of health care quality information and eliminating inequities in health care. The NHQR comprises 211 measures. This large measure set is distilled to 42 core measures which are the major focus of the 2006 report; of these, 40 have data for 2 or more years. The measures are balanced across the four dimensions of quality and provide a more readily understandable summary and explanation of the key results derived from the data. Major additions to the core measures have been made this year. Among them are three new measures on prevention, including advice from health care professionals on eating, exercise, and vision care, and two new composite measures for patient safety, including measures on postoperative complications and adverse events. Also, new measures were added to the overall measure set in the areas of asthma, hospice care, and patient centeredness in hospitals. The Highlights section offers a concise overview of findings from the 2006 NHQR. Four themes emerge from the 2006 NHQR: Most measures of quality are improving, but the pace of change remains modest; Quality improvement varies by setting and phase of care; The rate of improvement accelerated for some measures while a few continued to show Deterioration; Variation in health care quality remains high.
