Book excerpt: Windover dam consists of 2 earth embankments abutting a 140 feet long concrete capped masonry spillway, the maximum height of which is 15 feet. The left embankment is 75 feet long and the right embankment is 125 feet long. The crest width is 5 feet. The upstream slope is 1 vertical on 3 horizontal and the downstream slope is 1 and 2. A reservoir drain is located near the left end of the spillway. The examination of documents and the visual inspection of Windover Dam did not reveal conditions which constitute an immediate hazard to human life or property. However, the dam has some deficiencies which require further investigation and remedial action. Using the Corps of Engineers 'screening criteria' for the initial review of spillway adequacy, it has been determined that the embankment would be overtopped for all storms in excess of 37% of the PMF (Probable Maximum Flood). The spillway is, therefore, adjudged as 'seriously inadequate' and the dam is assessed as unsafe, non-emergency.

Book excerpt: The Feeder Dam at Glens Falls is a concrete gravity dam having a 615 foot long ungated river spillway section with flashboards including a 20 foot wide inclined logway. Examination of available documents and a visual inspection of the dam did not reveal conditions which constitute an immediate hazard to human life or property. However, significant deficiencies and deterioration was observed at one portion of the dam, the North Bulkhead, so as to warrant additional study and analysis as well as corrective action by maintenance forces. The spillway capacity of the dam, although not having sufficient discharge capacity for passing one-half the Probable Maximum Flood (PMF), is considered to be inadequate. Other deficiencies found during the visual inspection concerned concrete surface deterioration and cracking, non-operable or non-existent gate machinery, and an overall need for increased maintenance of the dam.

Book excerpt: Examination of available documents and a visual inspection of the dam revealed several deficiencies which may affect the safety of the dam. There was serious leakage in the vicinity of one of the gates in the gatehouse section. The auxiliary spillway section had failed allowing a portion of the river to flow around the left end of the dam. In addition to these observed deficiencies, structural stability analyses performed for the main spillway section indicate that the structure is unstable when subjected to severe loading conditions such as ice loading or flood flows. Further analysis is required. These studies should include subsurface and structural investigations to obtain information about the condition of the structure and its foundation. This information should then be incorporated into a more detailed stability analysis. The spillway, not having sufficient discharge capacity for passing one-half the Probable Maximum Flood (PMF), is considered to be inadequate. For such a large storm event, a high tailwater condition would occur, resulting in the flooding of downstream hazard areas. Hence, dam failure from overtopping would not significantly increase the hazard to loss of life from that which would exist just before failure. Several other deficiencies were also noted on this structure. Among these were deteriorated concrete on the main spillway section, erosion at the east abutment, and minor sloughing at the west abutment. In addition, no emergency action plan exists for this structure.

However, the dam has some deficiencies which require further investigations and remedial actions. It is recommended that within 3 months of notification to the owner, a detailed hydrological and hydraulic investigations be undertaken to more accurately determine the site specific characteristics of the watershed and their affect upon the overtopping potential of the dam. At the same time structural stability analysis of the dam should be performed. Within twelve (12) months of the date of notification to the owner, any modification to the structure deemed necessary as a result of investigations, to achieve a spillway capacity adequate to discharge the outflow from at least one-half PMF should have been completed. In the interim, a detailed emergency operation plan and warning system should be promptly developed. Also, during periods of unusually heavy precipitation, around-the-clock surveillance should be provided. In addition, the dam has a number of problem areas which, if left uncorrected, have the potential for the development of hazardous conditions and must be corrected within twelve (12) months.

A structural stability investigation should be commenced within six months to determine the characteristics of the uplift forces acting on the dam, the properties of the existing dam, and the effect of these conditions on the stability of the dam. Remedial work should be undertaken, depending on the results of this investigation and completed within two years.
