Book excerpt: The dam is located on Brixius Creek, a tributary of the Susquehanna River, approximately 1/2 mile north of Union, NY. The examination of documents and visual inspection of the Site 2 Dam and appurtenant structures did not reveal conditions which constitute a hazard to human life or property. The discharge capacity of the spillways is adequate for the PMF (Probable Maximum Flood). However the following remedial actions must be completed within 1 year from notification to the owner: (1) Eliminate the pedestrian traffic on the embankment and auxiliary spillway, backfill all depressions and reseed these areas; (2) repair the depressions on the upstream slope in the vicinity of the riser and beyond the downstream toe of the dam; (3) Remove the parking area from the auxiliary spillway channel and reseed the affected area; (4) Remove the vegetation from the inlet and outlet of the auxiliary spillway, and the downstream channel; (5) Investigate the conditions of the reservoir drain and restore to operational status, if required; (6) Repair the erosion or the Taft Road embankment and clean out the sediment within the 6 feet diameter highway culvert; and (7) provide a program of periodic inspection and maintenance of the dam and appurtenances, including yearly operation and lubrication of the reservoir drain system.

Book excerpt: The Site 1 Dam consists of a 1300 feet long homogeneous earth embankment with an auxiliary spillway located at the right end of the embankment in a cut section beyond the abutment of the embankment. The maximum height of the dam is 65 feet. The embankment has a crest width of 20 feet, an upstream slope of 1 vertical on 3 horizontal, and a downstream slope of 1 on 2.5. The auxiliary spillway is a vegetated earth channel, with a bottom width of 340 feet and side slopes of 1 on 3 (left slope) and 1 on 2 (right slope). The principal spillway is a rectangular concrete riser which extends above the upstream slope near the toe of slope. The riser is topped by a triangular trash rack, the sides of which form a drop inlet, which is utilized during high reservoir levels. Under low flow conditions, a rectangular low stage inlet, in the upstream face of the riser, controls the reservoir level. A 30 inch diameter reinforced concrete pipe controls the flow between the riser and the impact basin located at the toe of the dam. An 18 inch diameter pipe, with a manually operated slide gate, the controls of which are located atop the riser, serve as the reservoir drain system. The examination of documents and visual inspection of the Site 1 Dam and appurtenant structures did not reveal conditions which constitute a hazard to human life or property. The discharge capacity of the spillway is adequate for the PMF (Probable Maximum Flood).

The total discharge capacity of the spillways is adequate to impound and safely discharge the floodwaters resulting from the Probable Maximum Flood (PMF). Several minor deficiencies were noted on this structure. Among these deficiencies were a small depression on the upstream slope near the riser, ponding of water in the bottom of the auxiliary spillway channel, and a substantial accumulation of debris around the intake of the principal spillway riser. These deficiencies should be corrected within 6 months of the date of notification of the owner. In addition, an emergency action plan for notification of downstream residents should be developed within 6 months. (Author).

The dam consists of a compacted earth embankment which is 46 feet high, has a crest length of 1250 feet and a crest width of 16 feet. The upstream slope is 1 vertical on 3 horizontal with a 10 foot wide berm near the base of the slope. The downstream slope is 1 vertical on 2.5 horizontal. The crest and exposed slopes are covered with grass and crownvetch. An earth cutoff trench of varying depth and width keys the embankment into the foundation soils. The examination of documents and visual inspection of the Clark's Creek Site 1 Dam did not reveal conditions which constitute a hazard to human life or property. The total discharge capacity of the spillways is adequate to impound and safely discharge the floodwaters resulting from the Probable Maximum Flood (PMF).

The dam embankment is approximately 530 feet long, 33 feet high and has an upstream slope of 3 horizontal to 1 vertical and a downstream slope of 2.5 to 1. The principal spillway is a drop inlet structure consisting of a single stage reinforced concrete riser, a 24 inch diameter prestressed concrete cylinder pipe and a channel excavated into bedrock at the outlet of the conduit. The emergency spillway is a curved 80 foot wide channel with 3 to 1 side slopes cut into earth at the left abutment. Hydrologic/hydraulic analyses performed in accordance with the Corps of Engineers' Recommended Guidelines for Safety Inspection of Dams indicate that the principal spillway and the emergency spillway would pass 100 percent of the outflow from the Probable Maximum Flood (PMF) without overtopping the dam. Therefore, the combined spillway capacity is adjudged to be adequate.

Book excerpt: Visual inspection of this dam and engineering analyses revealed that the poor condition of the dam creates a hazard to human life and property. As a result of these determinations, the dam has been assessed as unsafe, non-emergency. One of the spillway sections has failed completely, creating a large void in the dam. The other spillway channel is in poor condition, having cracks in the concrete and several small voids. The earth fill upstream of the dam completely blocks the failed spillway section and partially blocks the other section. Due to the serious nature of the deficiencies, it is recommended that within 30 days of the notification to the owner that the dam has been assessed as unsafe, repair work on the structural deficiencies should be commenced. These repairs should be completed within 3 months. Within 3 months of the notification of the owner, a detailed hydrologic/hydraulic investigation should be commenced. This investigation should accurately define the site specific characteristics of the watershed and determine appropriate mitigating measures to be taken in response to the seriously inadequate spillway capacity. These measures should be completed within 1 year of the date of notification.

However, the dam has some deficiencies that need to be evaluated and remedied. The spillway is adjudged to be seriously inadequate and the dam is assessed as unsafe, nonemergency. It should be noted that Eaton Brook Reservoir Dam is located approximately 2.2 miles upstream of Clayton's Dam on Eaton Brook. Its spillway has also been adjudged to be seriously inadequate and the dam assessed as unsafe, nonemergency. Due to the appreciable size of Eaton Reservoir, the breaching of that dam would undoubtedly have a serious effect on Clayton's Dam. Similarly, the failure of either of these dams would have a serious effect on two small dams also on Eaton Brook, located 0.5 miles and 1.7 miles downstream of Clayton's Dam.
