Deficiencies visually observed by the inspection team were minor amounts of seepage at random slab, joints and in abutment drains, minor concrete spalling on the spillways downstream surface, exposed reinforcing steel on vertical supports, and the absence of containment walls on the spillway.

Based on visual observations, this dam appears to be in fair condition.

Johnson Lake Dam has a height of twenty-five (25) feet and a storage capacity of fifty-eight (58) acre-feet. In accordance with the guidelines a small size dam has a height greater than or equal to twenty-five (25) feet but less than forty (40) feet and a storage capacity greater than or equal to fifty (50) acre-feet but less than one thousand (1,000) acre-feet. The size classification is determined by either the storage or height, whichever gives the larger size category. Johnson Lake Dam is classified as a small size dam. In accordance with the guidelines and based on visual observation, the dam is classified as having a high potential for damage and loss of life. Failure would threaten life and property. The estimated damage zone extends approximately two (2) miles downstream of the dam. Within the damage zone are thirty (30) or more dwellings, U.S. Highway 65 and a football field all of which are located in the City of Carrollton.

Deficiencies visually observed by the inspection team were erosion and seepage indicated by moist soil near the east abutment on the downstream embankment slope. There were no observed deficiencies or conditions existing at the time of the inspection which indicated an immediate safety hazard.

Deficiencies visually observed by the inspection team were minor seepage through the spillway weir wall, erosion on the upstream embankment, minor deterioration of the concrete weir, separation of riprap, brush growth on both faces of the structure, and a slide area on the downstream face. There were no observed deficiencies or conditions existing at the time of the inspection which indicated an immediate safety hazard. Future corrective action and regular maintenance will be required to correct or control the described deficiencies.
