Book excerpt: The dam consists of a zoned, earthfill embankment that is 590 feet long and 52 feet high at its maximum section. The main spillway is a drop spillway located near the right abutment of the dam. It consists of a concrete riser connected to a 30-inch diameter reinforced concrete pipe under the embankment. The auxiliary spillway is an excavated channel along the left abutment of the dam. It is a grass-linked excavation in earth. Based on visual inspection, available records, calculations and past operational performance, PA-467 Dam is judged to be in good condition. The existing spillway can pass the Probable Maximum Flood(PMF) without overtopping of the dam. The spillway capacity is rated as adequate. There is a slope stability analysis for the embankment, and it indicates that the embankment has adequate factors of safety. There is no evidence of significant problems threatening the embankment.

Book excerpt: The purpose of the inspection is to determine if the dam constitutes a hazard to human life or property. The dam consists of a homogeneous earthfill embankment that is 680 feet long and 64 feet high at its maximum section. The main spillway is a drop spillway located near the left abutment of the dam. It consists of a concrete riser connected to a 30-inch diameter reinforced concrete pipe under the embankment. One 12-inch by 21-inch uncontrolled orifice is located in the upstream face of the concrete riser with the orifice crest at El. 1227.0, which is 27 feet below design elevation of the top of the dam. The auxiliary spillway is an excavated channel at the right abutment of the dam. It is grass-lined except for portions that were excavated into rock. The hazard classification is high hazard. Downstream conditions indicate that a high hazard classification is warranted for PA-468 Dam. It is owned by the Susquehanna County Commissioners. Its purpose is for flood control.

Book excerpt: The purpose of a Phase I investigation is to identify expeditiously those dams which may pose hazards to human life or property. The assessment of the general condition of the dam is based upon visual observations and review of available data. Detailed investigations and analyses involving topographic mapping, subsurface investigations, material testing, and detailed computational evaluations are beyond the scope of a Phase I investigation; however, the inspection is intended to identify any need for such studies which should be performed by the owner. In reviewing this report, it should be realized that the reported condition of the dam is based on observations of field conditions at the time of inspection along with data available to the inspection team. In cases where the reservoir was lowered or drained prior to inspection, such action, while improving the stability of the dam, removes the normal load on the structure and may obscure certain conditions which might otherwise be detectable if inspected under the normal operating environment of the structure.

Book excerpt: Lake Sophia Dam consists of an earth embankment approximately 340 feet long with a maximum height of 29 feet from the downstream toe and a crest width of 9 feet. The upstream side of the dam is protected by a small amount of riprap and the downstream face is covered with grass. The flood discharge facilities for the dam consist of a concrete open-channel spillway located near the left abutment. The outlet facilities consist of a 12-inch-diamenter corrugated metal pipe extending through the embankment along the original streambed. Based on the evaluation of existing conditions, the condition of Lake Sophia Dam is considered to be unsafe/nonemergency due to seriously inadequate spillway capacity. The flood discharge capacity was evaluated according to to the recommended procedure and was found to pass approximately 15 percent of the PMF without overtopping the dam. Results of the breach analysis indicate that downstream damage would be significantly increased due to a dam failure.

The condition of Lake Walter Dam is considered to be good. The only significant condition noted which requires further attention is the lack of erosion protection in the emergency spillway discharge channel. The swampy condition at the toe of the dam is not considered to be significant at this time; however, the condition should be monitored to document if seepage is developing.

Although at this time no signs of significant distress were observed, it appears that the center of the dam has significantly settled. The design drawings show the design freeboard to be five feet, but the available freeboard is only approximately two feet. It also appears that fill has been placed on the downstream slope, increasing the crest width from the design value of 10 feet to about 30 feet near the center line of the dam and greater than 50 feet near the left abutment. The dam has no functional low level outlet facility.

Newlin, Jr., and is classified as a 'Low' hazard - 'Small' size dam. The dam was found to be in fair overall condition at the time of inspection. Hydraulic/hydrologic evaluations revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 50- year flood to the 100-year flood is required for Warner Dam. The 100-year flood was chosen as the SDF. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately develop remedial measures to provide adequate spillway capacity. The inspection revealed certain items of remedial work which should be performed by the owner without delay. These include: (1) Provide adequate spillway capacity; (2) Repair the spalling on the downstream face of the concrete corewall; (3) Ensure that the outlet works are operable, or develop an emergency drawdown plan; and (4) Cut all trees on the embankment at ground level. All trees with a trunk diameter greater than 3 inches should have their root systems removed. All resultant areas of erosion and cavities should be filled, graded, compacted and seeded. In addition, the following operational measures are recommended to be undertaken by the owner: (1) Develop a detailed emergency operation and warning system; (2) During periods of unusually heavy rainfall, provide around-the-clock surveillance of the dam; and (3) When warning of a storm of major proportions is given by the National Weather Service, activate the emergency operation and warning system.

In the absence of an adequate spillway, the dam is subject to frequent overtopping. Based on the location of the downstream dwellings and the fact that a few lives could be lost should the dam fail, the dam is classified as a significant hazard dam. Since the spillway cannot pass the 100- year flood without overtopping the dam, the spillway is rated as inadequate. Although there is some seepage thru the dam, there is no evidence of structural instability. There is no formal inspection and maintenance program or warning system and evacuation plan in effect at Acre Pond Dam. The dam is not properly maintained.

The existing spillway can pass only 17% of the Probable Maximum Flood (PMF) without overtopping of the dam. Based on the type of construction and the condition of the dam, it is judged that the dam could not withstand the depth and duration of overtopping that would occur for the PMF or the 1/2 PMF. Failure of the dam would cause an increased hazard to loss of life downstream. As a whole, the dam is judged to be in poor condition. The spillway has undergone a gradual structural failure that has resulted in a reduction of spillway capacity. Continuation of the gradual failure, or perhaps a sudden failure, appears to be possible. There are no functional outlet works for the dam. Maintenance of the project is inadequate. There is no program for regular inspection of the dam.

In accordance with the Corps of Engineers' evaluation guidelines, the size classification of this dam is small and the hazard classification is high. These classifications indicate that the Spillway Design Flood (SDF) should be in the range of one-half the Probable Maximum Flood (PMF) to the full PMF. The recommended SDF for this structure is one-half the PMF. The spillway capacity is adequate for passing only 10 percent of the PMF peak inflow without overtopping the dam. The spillway is considered to be seriously inadequate. The dam is therefore unsafe, non-emergency.
