The embankment is 490 feet long and 23 feet high at the maximum section. The spillway is an irregular excavated channel at the right abutment of the dam. An outlet works with a 24-inch diameter cast-iron pipe and a valve at the downstream toe is located near the middle of the embankment. A dike is located about 500 feet right of the embankment. It is 284 feet long and 3 feet high at maximum section. The dam is located on Rattlesnake Creek approximately 3.4 miles west of Moscow, Pennsylvania.

Book excerpt: Maple Lake Dam is an earthfill embankment 14.4 feet high and 283 feet long, excluding spillway. The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Book excerpt: Based on visual inspection, past performance, field surveys and calculations, Glenwood Lake Dam is judged to be in poor condition. The recommended Spillway Design Flood (SDF) for the size (small) and hazard classification (high) of the dam is between the one-half Probable Maximum Flood (1/2 PMF) and the full PMF. Based on the small size and storage capacity, it is judged that the SDF of 1/2 PMF is appropriate for the Glenwood Lake Dam. Under the present conditions, the spillway will pass approximately 10 percent of the PMF without overtopping the dam. Overtopping depths of 1.1 feet and 2.12 feet were calculated for flood magnitudes of 20% and 50% of the PMF, respectively. It was judged that the dam will begin to fail under an overtopping depth of one- foot. Because the present spillway capacity would not pass the SDF without overtopping the dam and failure of the dam would increase the downstream hazard to life and property, the spillway is rated as seriously inadequate and the dam is rated as unsafe, non-emergency. The facility is not properly maintained, as evidenced by the extensive growth of trees on the embankment and the condition of the outlet works valve pit. There is no warning system and evacuation plan in effect at the present time.

The embankment has a puddled impervious core and riprap on both the upstream and downstream slopes. The spillway is located at the right abutment of the dam. It is an excavated, riprapped channel with irregular geometry. The invert of the channel is 2.6 feet below the design level of the top of the dam. A 12-inch diameter cast-iron pipe with a valve at the downstream end extends through the center of the embankment and regulates flows. Olyphant No. 2 Dam is 0.5 mile downstream. The dam is located on Grassy Island Creek approximately 1.7 miles east of the Village of Winton, Pennsylvania.

The purpose of the Phase I investigation is to identify expeditiously those dams which may pose hazards to human life or property. The assessment of the general condition of the dam is based upon available data and visual inspections. Detailed investigation, and analyses involving topographic mapping, subsurface investigations, testing, and detailed computational evaluations are beyond the scope of a Phase I investigation; however, the investigation is intended to identify any need for such studies. In reviewing this report, it should be realized that the reported condition of the dam is based on observations of field conditions at the time of inspection along with data available to the inspection team. In cases where the reservoir was lowered or drained prior to inspection, such action, while improving the stability and safety of the dam, removes the normal load on the structure and may obscure certain conditions which might otherwise be detectable if inspected under the normal operating environment of the structure.

It is 35 feet high at its maximum section and 685 feet long. The main spillway is located at the maximum section of the dam. It consists of a reinforced concrete riser structure near the upstream toe of the dam, a 24-inch diameter conduit, and a concrete outlet structure at the downstream toe of the dam. The auxiliary spillway is an excavated, trapezoidal channel at the left abutment of the dam. Based on visual inspection, available records, calculations, past operational performance, and according to criteria established for these studies, Maple Lake Dam is judged to be in good condition. Based on existing conditions, the main and auxiliary spillways will pass an approximate minimum of 48 percent of the Probable Maximum Flood (PMF before overtopping of the dam occurs. No stability problems were evident for the embankment at the time of the visual inspection, but a potential hazard to stability exists due to erosion that might occur when there is flow in the auxiliary spillway.

The profile and typical sections of the dam are presented and are based on field survey made the day of the inspection. On the inspection date (12/08/80), the lake level was at elevation 1146.8, about 0.2 foot below spillway crest. Pertinent features observed are shown in photographs.

A low area between the two embankments acts as an auxiliary spillway. There is no stability analysis for the embankments. There is no evidence of significant problems threatening the embankments. The spillway weir is judged to be stable.

The existing spillway can pass only 7 percent of the Probable Maximum Flood (PMF) without overtopping of the dam. Because a portion of the masonry gravity dam is founded on earth and there are no erosion protection measures at the toe, it is assumed that the dam could not withstand the depth and duration of overtopping that would occur for the PMF or the 1/2 PMF. Failure of the dam would cause an increased hazard to loss of life downstream. As a whole, the dam is judged to be in fair condition. Stability studies performed as part of this report indicate that the dam does not have any significant deviations from the Office of the Chief of Engineers (OCE) guidelines for stability. Maintenance at the dam and operating facilities is inadequate. There is no program for regular inspection of the dam.
