Book excerpt: Based on the visual inspection, operational history, and available engineering data, the dam and its appurtenances are considered to be in good condition. The size classification for the facility is intermediate and the hazard classification is high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the Probable Maximum Flood (PMF). Results of the hydrologic and hydraulic analysis indicate that the facility is capable of passing and/or storing only 36 percent of the PMF. The analysis also indicates that spillway discharges are controlled by the downstream valley capacity when the flow over the weir exceeds approximately 3 feet (10 feet below the embankment) after which the pool level and tailwater will rise at roughly the same rate. Thus, failure from overtopping is essentially precluded and the spillway is considered inadequate, but not seriously inadequate. The consequences of embankment inundation by tailwater and subsequent rapid drawdown are uncertain; although, it is noted that portions of the downstream slope are designated as pervious fill and rock toe on the design drawings. This construction is favorable with respect to rapid drawdown response; however, thorough inspection of the embankment should be performed during and after inundation.

Observations made during the visual inspection suggest that the embankment may be structurally unsafe as extensive areas of seepage were noted on the downstream embankment slope and immediately below the toe area to the right of the outlet conduit. Seepage conditions became evident shortly after reservoir filling and have persisted in spite of the installation of a remedial sand drain system near the toe. Failure of the embankment does not appear imminent and the condition is presently considered non-emergency. It is recommended that the owner immediately develop and implement a warning system to notify downstream inhabitants should hazardous conditions develop. Included in the plan should be provisions for around-the-clock surveillance of the facility during periods of unusually heavy precipitation.

The embankment is reportedly comprised of 3 sections which include an impervious core, a semi-pervious upstream section and a pervious downstream section. The facility is served by a chute channel spillway, with discharges controlled by a free overfall, concrete weir structure. The spillway is located near the right abutment, and has a crest length of 39 feet. In addition, the facility is equipped with a 18-inch diameter steel pipe encased in concrete used as a low level outlet as well as a 16-inch diameter steel siphon which was installed in 1973.

Results of the hydrologic and hydraulic analysis indicate that the facility can accommodate approximately 32 percent of the PMF and/or 64 percent of SDF (1/2 PMF). Book excerpt: The assessment of Bigby Run Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The Bigby Run Dam appears to be in fair condition. Maintenance is considered fair. The concrete spillway is deteriorating with marked deterioration on the right spillway wall. Minor seepage was observed at the toe of the dam, adjacent to the left abutment contact. Seepage was estimated at 2 to 5 gallons per minute. The seepage should be monitored. The Bigby Run Dam is a high hazard-small size dam. The recommended Spillway Design Flood (SDF), for a dam of this size and classification, is in the range of 1/2 PMF to PMF. Since the Bigby Run Dam just meets the minimum criteria; and since the dam is located in a rural area suggesting only appreciable economic loss; compliance with the current practice of the Baltimore District Corps of Engineers leads to the selection of the 1/2 PMF as the Spillway Design Flood (SDF).

The emergency spillway is capable of discharging the peak inflow resulting from a storm of PMF intensity and is, therefore, considered adequate. It is recommended that the owner: (a) Repair the damaged pond drain gate control and replace the missing manhole cover atop the service spillway riser. Consideration should be given to redesigning the gate control mechanism to protect it from future vandalism. (b) Consult with the U.S.D.A., Soil Conservation Service, to assess the erosion on the upstream slope in the area of the service spillway riser and implement any remedial measures deemed necessary. (c) Curtail the unauthorized vehicular travel across the embankment and repair and reseed any damaged (bare) areas. If control of unauthorized travel is not practical, some provision should be made to protect the embankment from rutting and erosion by placing a layer of gravel or stone across the crest. (d) Develop a formal operation and maintenance manual to ensure the continued proper care of the facility. In addition, a formal warning system should be implemented which provides detailed procedures to protect the lives and property of downstream residents. Included in the plan should be provisions for around-the-clock surveillance of the facility during periods of unusually heavy percipitation.

Book excerpt: Polk Dam is owned by the Polk State School and Hospital and is classified as a 'Significant' hazard - 'Small' size dam. The dam was found to be in fair overall condition at the time of inspection. Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway will not pass the 100-year flood without overtopping the dam. A spillway design flood (SDF) in the range of the 100-year flood to the 1/2 Probable Maximum Flood (1/2 PMF) is required for Polk Dam. The 100-year flood was chosen as the SDF. During the 100-year flood, the dam is overtopped by a maximum of 0.61 foot for a duration of 1.50 hours. The spillway is therefore considered 'inadequate.' It is recommended that the owner immediately initiate an engineering study to further evaluate the spillway capacity and develop recommendations for remedial measures to reduce the overtopping potential of the dam. Several items of remedial work should be performed by the owner without delay.

Mine No. 60-Pond 5 is located at the headwaters of an unnamed tributary to the Center Branch of Pigeon Creek, in Somerset Township, Washington, County, Pennsylvania. The Mine No. 60-Pond 5 is classified as a 'high' hazard dam. The No. Mine 60-Pond 5 is owned by the Bethlehem Mines Corporation Eight-Four, Pennsylvania. The field inspection of Mine No. 60-Pond 5 was performed on 18 March 1981 and 26 March 1981 and consisted of: Visual observations of the embankment crest and slopes, groins and abutments; Visual observations of the principal (and emergency) spillway including approach channel, overflow crest, and discharge channel; Visual observations of the embankment's downstream toe area including drainage channels and surficial conditions; Visual observations of downstream conditions and evaluation of the downstream hazard potential; Visual observations of the reservoir shoreline and watershed; Visual observations of the outlet works; Transit stadia surveys of relative elevations along the embankment crest centerline, spillway, and across the embankment slopes. The visual observations were made during periods when the reservoir was at normal operating level.

Results of the hydrologic and hydraulic analysis indicate that the facility is capable of passing and/or storing only 20 percent of the PMF without overtopping the embankment. Overtopping and embankment failure is also anticipated under less than 1/2 PMF flooding resulting in an increase in potential for loss of life. Thus, based on criteria contained in the recommended guidelines the spillway is considered seriously inadequate. Seepage was noted at the downstream toe in an area behind the old gate house. Also, seepage was observed to be issuing from the left embankment-abutment junction, about midway between the toe and the crest of the dam. Historical records indicate seepage was frequently noted in both areas. General surficial deterioration is evident on portions of the spillway, particularly the sidewalls.
