Book excerpt: Based on a visual inspection and available engineering data, Gravel Lick Dam and its appurtenances are considered to be in fair condition. The spillway and dam are capable of passing and/or storing the flow resulting from a storm of PMF magnitude without overtopping the embankment. As a result, the spillway is considered adequate. Seepage through the downstream slope, steadily increasing pore pressures near the downstream toe and erosion below the downstream bench level are apparent. It is recommended that: (1) the owner engage the services of a qualified registered professional engineer to investigate the seepage through the downstream face and evaluate the increasing pore pressure conditions at the toe; (remedial measures should then be implemented as required); (2) the erosion gullies along the downstream face be backfilled and seeded; (3) a formal warning system be implemented to protect those persons downstream who may ultimately be affected by an embankment breach; and (4) the dam should be inspected on a periodic basis to check for hazardous conditions which might develop.

Book excerpt: Galion Bay Dam consists of an earth embankment approximately 400 feet long with a maximum height of 37 feet from the downstream invert of the outlet conduit and a crest width of 20 feet. Galion Bay Dam is one of the three dams which impound Lake Rene. The other two dams are Wolf Creek Dam, located at the west end of Lake Rene, and Lake Rene Dam, located on the east side of the reservoir. Lake Rene Dam, which was constructed in 1955, was the original dam at the site. Galion Bay Dam and Wolf Creek Dam were constructed in 1971 in conjunction with the enlargement of Lake Rene. The flood discharge facilities for Lake Rene consist of primary and emergency spillways associated with Galion Bay and Wolf Creek dams. The Galion Bay Dam flood discharge facilities are comprised of a drop inlet primary spillway located at the center of the embankment and an emergency spillway on the right abutment (looking downstream). The primary spillway structure consists of a single-stage reinforced concrete riser, a 36-inch-diameter reinforced concrete conduit, and a plunge pool at the downstream end of the outlet pipe. The emergency spillway is a 30-foot-wide trapezoidal earth channel excavated into the right abutment. The reservoir outlet pipe for the dam consists of a 15-inch steel pipe extending from the upstream toe of the dam to the drop inlet structure.

Lake Rene Dam is one of three dams which impound Lake Rene. The crest width is approximately 10 feet. A 10-foot-wide berm is located on the downstream slope at about midheight of the embankment. The downstream slope of the embankment is approximately 1.5H to 1V above the berm and 2.5H to 1V below that level. The flood discharge facilities for the reservoir consist of a concrete overflow structure located on the left abutment (looking downstream). The overflow section is approximately 15 feet wide and 3 feet deep and discharges onto an approximately 30-foot-long concrete chute. According to the previous inspection reports, the outlet facilities consist of two cast-iron pipes 4 and 6 inches in diameter. the location of the pipes and the manner in which flow through these pipes is controlled is unknown. In the design drawings, flow through these pipes is shown to be controlled by valves located in a valve chamber on the downstream side of the dam. However, this valve chamber could not be located at the site during this inspection. As it presently exists, the dam has no functional emergency drawdown facilities. (Author).

Book excerpt: Polk Dam is owned by the Polk State School and Hospital and is classified as a 'Significant' hazard - 'Small' size dam. The dam was found to be in fair overall condition at the time of inspection. Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway will not pass the 100-year flood without overtopping the dam. A spillway design flood (SDF) in the range of the 100-year flood to the 1/2 Probable Maximum Flood (1/2 PMF) is required for Polk Dam. The 100-year flood was chosen as the SDF. During the 100-year flood, the dam is overtopped by a maximum of 0.61 foot for a duration of 1.50 hours. The spillway is therefore considered 'inadequate.' It is recommended that the owner immediately initiate an engineering study to further evaluate the spillway capacity and develop recommendations for remedial measures to reduce the overtopping potential of the dam. Several items of remedial work should be performed by the owner without delay.

The size classification of the facility is intermediate and the hazard classification is considered to be high. The emergency spillway is capable of discharging the peak inflow resulting from a storm of PMF intensity and is, therefore, considered adequate. It is recommended that the owner: (a) Repair the damaged pond drain gate control and replace the missing manhole cover atop the service spillway riser. Consideration should be given to redesigning the gate control mechanism to protect it from future vandalism. (b) Consult with the U.S.D.A., Soil Conservation Service, to assess the erosion on the upstream slope in the area of the service spillway riser and implement any remedial measures deemed necessary. (c) Curtail the unauthorized vehicular travel across the embankment and repair and reseed any damaged (bare) areas. If control of unauthorized travel is not practical, some provision should be made to protect the embankment from rutting and erosion by placing a layer of gravel or stone across the crest. (d) Develop a formal operation and maintenance manual to ensure the continued proper care of the facility. In addition, a formal warning system should be implemented which provides detailed procedures to protect the lives and property of downstream residents. Included in the plan should be provisions for around-the-clock surveillance of the facility during periods of unusually heavy percipitation.

This assessment is based on visual observations that indicate a possible inadequate margin of safety against slope failure of a high ground water level exists in Embankment A. The condition is not considered to represent an immediate threat to the structure because of the relatively low impoundment pool elevation. It should, however, be evaluated. If a high ground water does not exist in Embankment A, the facility can be considered to be in good condition. The structure is classified as an 'intermediate' size, 'high' hazard dam. Corps of Engineers guidelines recommend the Probable Maximum Flood (PMF) as the Spillway Design Flood for an 'intermediate' size, 'high' hazard dam. The visual inspection indicated several minor deficiencies in addition to the possible high ground water condition. The deficiencies can be corrected or improved through implementation of the following recommended evaluation, remedial, monitoring and/or maintenance efforts.

Book excerpt: The New Dam consists of an earth embankment approximately 330 feet long with a maximum height of 37 feet from the downstream toe and a crest width of 32 to 35 feet. The flood discharge facilities for the dam consist of a combined primary and emergency spillway located on the right abutment (looking downstream). The spillway is an earth channel excavated into the right abutment and includes neither an overflow control structure, nor an energy dissipation structure at the downstream end. The width of the channel varies from 30 feet at the upstream end to 20 feet at the downstream end. The channel follows the right abutment for approximately 200 feet, then turns left, and discharges freely over the hillside into the stream valley. The outlet works consist of a 24-inch cast-iron blowoff pipe and a concrete intake tower. The concrete intake tower is located on the upstream side of the dam approximately mid-height of the embankment. All the operating equipment on the intake tower has seriously deteriorated and is no longer functional. The blow-off pipe appears to extend 50 to 100 feet further upstream from the intake tower. No low-level intake structure was visible. Flow through the outlet conduit is controlled by a valve located at the downstream end. The valve was reportedly left open to draw down the lake.
