The embankment is 490 feet long and 19 feet high at maximum section. There are two outlet works at the dam. The first outlet works, which is near the right end of the embankment, consists of a concrete intake structure, two 24-inch diameter cast-iron pipes, and a valve house. This outlet works is connected to the water supply system. The second outlet works, which is near the left end of the dam, is similar to the first except it consists of a single 24-inch diameter pipe. The second outlet works provides the emergency drawdown capability. The concrete chute spillway is at the left abutment of the dam. Its crest is 3.0 feet below the design elevation of the top of the dam and is 75 feet long. The approach channel is short and concrete paved. The exit channel is a continuation of the chute. A bridge extends across the spillway crest. It is supported by two piers.

Book excerpt: Based on visual inspection, available records, calculations, past operational performance, and according to criteria established for these studies, Dam F is judged to be unsafe, nonemergency, because the spillway capacity is rated as seriously inadequate. The spillway can pass 29 percent of the Probable Maximum Flood (PMF) without overtopping of the dam. The Owner has placed sandbags along the spillway crest, which reduces the spillway capacity further. The resulting outflows from the failure of Dam F would overtop and cause the failure of Dam G. This would result in the loss of life. As a whole, the dam is judged to be in fair condition. There are bulges on the downstream slope that apparently have not stabilized. The dam has essentially no operational emergency drawdown capability. Maintenance at the dam is marginal.

The overall appearance and general condition of the dam and appurtenances are fair. The spillway crest has been raised and the dam crest is two feet below design elevation. These and other noteworthy deficiencies noted. The reservoir pool was 3.5 feet below spillway crest (corewall) on the day of the initial inspection, 22 October 1980. The lake was purposely drawn down so that maintenance of the dam could be performed. On the date of the review inspection, the reservoir level was one foot above the spillway crest (corewall).

Book excerpt: Bear Creek Lake Dam is a rock filled timber crib structure approximately 325 feet long and 17 feet high at its maximum section. An ungated drop spillway with an 158-foot long sharp crested weir is located approximately 60 feet from the right abutment (looking downstream). The 80 acre lake is used for recreation by the residents of Bear Creek Village. It is unknown when the dam was constructed. In the state inspection report of April, 1915 it is stated, 'This dam was constructed a great many years ago, but it has been repaired from time to time, so that more than likely none of the original structure is now in existence.' Examination of the results of the hydrologic and hydraulic analysis indicate that the drop spillway is capable of passing 11 percent of the Probable Maximum Flood (PMF) without overtopping of the embankment. Failure of the dam would significantly increase the hazard to loss of life downstream of the dam. Therefore, the capacity of the drop spillway is classified as 'seriously inadequate.' The dam is considered to be 'unsafe (non-emergency).'

Based on visual inspection, available records, calculations, past operational performance, and according to criteria established for these studies, Sunrise Lake Dam is judged to be in good condition. The spillway capacity is rated as adequate. The spillway can pass the Probable Maximum Flood (PMF) without overtopping of the dam. The headwall at the conduit outlet is tilted and is considered to be unsatisfactory. In the absence of stability computations showing that an adequate factor of safety exists, the downstream slope is considered to have a marginal factor of safety because of the steepness of the slope.

Book excerpt: Penn Lake Dam is an earth embankment approximately 350 feet long and 44 feet high at its maximum section. The spillway consists of twelve 42-inch diameter, 20-foot long pipes located approximately 1,000 feet left (looking downstream) of the dam. Earth swales on both sides of the battery of pipes function as auxiliary spillways. The 56 acre reservoir is used for recreation by members of the Penn Lake Association. Examination of the results of the hydrologic and hydraulic analyses indicates that the spillway (pipes and swales) is capable of passing 48 per cent of the Probable Maximum Flood (PMF) without overtopping of the embankment. Failure of the dam would increase the hazard of loss of life downstream of the dam. The capacity of the spillway system is therefore classified as 'Seriously Inadequate.' The dam is considered to be unsafe (non-emergency). Based on visual observations made during the date of the inspection, the dam is considered to be in poor condition.

Book excerpt: In reviewing this report, it should be realized that the reported condition of the dam is based on observations of field conditions at the time of inspection along with data available to the inspection team. In cases where the reservoir was lowered or drained prior to inspection, such action, while improving the stability and safety of the dam, removes the normal load on the structure and may obscure certain conditions which might otherwise be detectable if inspected under the normal operating environment of the structure. Phase I inspections are not intended to provide detailed hydrologic and hydraulic analyses. In accordance with the established Guidelines, the spillway design flood is based on the estimated 'Probable Maximum Flood' for the region (greatest reasonably possible storm runoff), or fractions thereof. The spillway design flood provides a measure of relative spillway capacity and serves as an aid in determining the need for more detailed hydrologic and hydraulic studies, considering the size of the dam, its general condition and the downstream damage potential.

If the dam were raised to its design elevation, the spillway would pass about 28 percent of the PMF. The spillway capacity would still be rated as seriously inadequate. As a whole, the dam is judged to be in poor condition. Lack of maintenance has caused the spillway to deteriorate to such an extent that it is on the verge of collapse. It is unlikely that the spillway could withstand any flood of significant duration. (Author).

Location of observed deficiencies are shown on the General Plan presented. The profile and typical sections of the dam are presented and are based on field survey made on the day of inspection. The survey datum for this inspection is elevation 1495 feet above mean sea level for the normal water surface of the lake. On the inspection date (12/02/80), the lake level was approximately at elevation 1495, which is 1.5 feet below the low point on the dam crest. Deficiencies observed during the field inspection are described below, and are further illustrated. Visible features are depicted in photographs presented.

Because of the nature of its construction, the steep downstream slope, and cracks that have developed on the top of the embankment, the stability of the embankment is considered marginal.
