Book excerpt: There are two essential fatty acids for humans, a-linolenic acid (ALA) and linoleic acid (LA), which should be taken from foods to maintain health. Once incorporated into cells, ALA and LA, which are omega-3 poly unsaturated fatty acid (n-3 PUFA) and n-6 PUFA respectively, undergo elongation and desaturation to generate longer and more unsaturated fatty acids influencing inflammation and immunological responses. Numerous studies showed a dietary reduction of n-6/n-3 PUFA ratio improves cardiovascular health, inflammation, and insulin resistance. Fish oil, the main resource for n-3 PUFA, is shown to increase these health benefits. In our lab, we investigated the efficacy of ALA-enriched butter (n3Bu) as an alternative to fish oil for n-3 PUFA. n3Bufed mice showed increased bioconversion of ALA to long-chain n-3 PUFA (LC n-3 PUFA) and attenuated high fat (HF) diet-induced insulin resistance and inflammation. Besides, these health benefits, n-3 PUFA is shown to improve obesity and its related diseases by regulating lipid metabolism in both white adipose tissue (WAT) and brown adipose tissue (BAT). Fish oil abundant with n-3 PUFA promotes BAT formation and increases its thermogenic activity in cold acclimation. However, the impact of ALAenriched agricultural products on the BAT function is unknown. In this study, we investigated the effect of ALA-biofortified butter (n3Bu) on lipid metabolism and thermogenic functions in BAT. Intake of n3Bu significantly reduced the whitening of BAT and increased the thermogenesis in response to acute-cold treatment. Consumption of n3Bu promoted bioconversion of LC n-3 PUFA, fatty acid elongation and desaturation, and mitochondrial biogenesis. Taken together, our results support that ALA-biofortified butter is a novel source of n-3 PUFA that potentiates the BAT thermogenic function.

Book excerpt: N-3 polyunsaturated fatty acids (n-3 PUFA) have been postulated to improve the insulin resistance associated with type 2 diabetes since the 1960s when observational studies in the Alaskan Inuit noted a reduced prevalence of type 2 diabetes when this population consumed a traditional diet. These findings were supported by animal studies but results of human intervention studies have been variable with most showing no change in glucose metabolism. More recent studies in growing farm animals suggested that muscle membrane phospholipids required to be enriched to a minimum of 14% n-3 PUFA in order for a change in insulin sensitivity to occur. This study sought to establish the effect of long-term (9 month) high-dose (3g/day) supplement of the n-3 PUFA eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) on insulin sensitivity of glucose and protein metabolism. Thirty-three subjects with impaired glucose regulation underwent hyperinsulinaemic-euglycaemic-euaminoacidaemic clamps pre- and postintervention of n-3 PUFA or a control (maize) oil. A second cohort who all received n-3 PUFA supplementation underwent pre- and post-intervention muscle biopsies. Secondary outcomes included an assessment of inflammatory status and determining whether erythrocyte membrane phospholipid could act as a surrogate for muscle membrane phospholipid. In the clamp cohort, there were no changes in glucose metabolism postintervention; however, there was an increase in insulin-stimulated protein metabolism following the fish oil intervention. In the biopsy cohort, no subject achieved 14% PUFA enrichment in muscle membrane phospholipids; however, all subjects who received n-3 PUFA supplementation did achieve a minimum of 14% enrichment of n-3 PUFA in erythrocyte membrane phospholipid. In agreement with the majority of the literature, n-3 PUFA did not affect glucose metabolism. Insulin-stimulated protein metabolism was improved supporting the findings of another recent human study. These changes in protein metabolism may reduce the sarcopenia associated with aging, potentially delaying the progression of frailty.

Wild game has the terrestrial source of n-3 incorpo rated in its fat since browsing animals derive 18:3n-3 (alpha-linolenic acid) natural ly from leafy plants. There is, however, little opportunity for modern Western man to get n-3 fatty acids from the diet if one does not consume fish. Modern agribusiness provides ani mal feeds high in n-6 fatty acids, mostly derived from linoleic acid (18:2n-6) in corn feed. Therefore, grazing animals have no access to alternative fatty acids in either feed or grasses, the latter containing little or none of these potentially beneficial highly polyunsaturated fatty acids.

However, non-surgical weight-loss treatments are ineffective. Treatments that can reduce cardiometabolic risk in the overweight and obese are urgently needed. Further, as the offspring of obese and/or diabetic women are programmed to become obese and insulin resistant with age, interventions in pregnancy could have substantial long-term impacts on public health. Supplemental omega-3 fatty acids appear promising as they modulate important metabolic pathways and are insulin-sensitising in rats. The central hypothesis in this thesis is that long chain omega-3 polyunsaturated fatty acids, can modify the effects of gene-environment interactions to improve aspects of glucose and lipid metabolism. Four predictions were tested 1) that an n-3 PUFA supplement would be insulin sensitising in overweight men and 2) that an n-3 PUFA supplement used in a model of insulin resistant pregnancy would have metabolic benefits to the offspring 3) that fish oils sold at retail would have excessive oxidation, and 4) that an oxidised fish oil supplement would not have benefits to the offspring when used during pregnancy. I used a randomised controlled crossover trial to investigate the metabolic effects of krill/salmon blended oil on insulin sensitivity in overweight men. At baseline, higher n-3 PUFA levels were associated with greater insulin sensitivity. However, supplementation with the marine oil led to reduced insulin sensitivity, suggesting increased cardiometabolic risk. In an animal model of insulin-resistant pregnancy (maternal high-fat diet supplementation with unoxidized fish oil prevented the development of insulin resistance in the adult offspring. I found that fish oil products purchased at retail were substantially oxidised and did not meet labelled n-3 PUFA content. I then assessed the effects of oxidised fish oil supplementation during rat pregnancy, and found a substantial increase in neonatal mortality and persistent maternal insulin resistance. I have shown that oxidation changes the metabolic effects of n-3 PUFA rich supplements. It is possible that there are negative consequences to consuming oxidised fish oil during human pregnancy, and unrecognised oxidation may have confounded the n-3 PUFA clinical trial literature.
