Sir William Osler Over a period of five years, the impact of computed tomography (CT) on pediatric neuroradiology at The Hospital for Sick Children, Toronto, has been, as expected, in the assessment of the brain and its abnormalities. Concurrent with this application was the introduction of Metrizamide (Amipaque, Nyegaard & Co. AS, Oslo, Norway), a water-soluble CSF contrast medium, used primarily as a myelog raphic agent. The subsequent application of the wide-aperture CT scanner to imaging of the spine in children provided remarkable advances in the clinical management of spinal disease since CT is far more accurate than standard neuroradiologic procedures. The com bination of CT and Metrizamide added a further dimension to the imaging of the spine and of the spinal cord and nerve roots. Such spinal CT and CT Metrizamide myelography in children now occupies a significant part of day-to-day pediatric neuroradiologic practice. They have dramatically enhanced our understanding of the normal anatomy and pathologic entities of the spine and its contents in children; have altered and improved the surgical management of such diseases; and have significantly improved the clinical manage ment of such diseases in the specialties of neurosurgery, orthopedic surgery, and genito-urinary surgery . .

It is a harmless and almost painless technique that does not require the introduction of contrast medium into the subarachnoid spaces. Its interpretation is based on the modifications of the epidural veins which do not themselves have any proven pathology. The longitudinal epidural veins are situated in the anterolateral angles of the spinal canal between the nerve root and the intervertebral disc. They conse quently represent a major anatomic landmark in the investigation of disc pathol ogy because they are compressed by a disc herniation before the corresponding nerve root is reached. The improvement of the techniques of lumbar phlebography by catheterization of the lateral sacral veins and of cervical phlebography by catheterization of the vertebral veins now permits opacification of the epidural venous system along the total length of the lumbar and cervical spine. Spinal phlebography can consequently be used routinely, primarily in disc pathology but now also in other fields of vertebromedullary pathology such as myelopathy, tumors, or stenosis of the spinal canal, where it provides new information as compared to the other techniques of investigation. The techniq ue and results of spinal phlebography in these various conditions are presented in this monograph which is an attempt, with the collaboration of several authors, to summarize current knowledge in this field. Only lumbar and cervical phlebography will be discussed in this monograph.

Book excerpt: The idea for this treatise on the radiological anatomy of superficial and deep spinal cord vasculature evolved from daily routine neuroradiological work. This was also the reason for subdividing the monograph into a postmortem anatomical and a clinical part. The actual importance of a clear conception of radio anatomic fundamentals was made clear by many clinical conferences with neurologists, neurosurgeons and orthopedists, where a lack of knowledge about medullary syndromes of suspected vascular origin became evident. Also among neuroradiologists there is still widespread uncertainty in the interpretation of myelograms and angiographies in such cases. A study of the spinal cord's angioarchitecture is all the more justified and necessary considering the vast number of descriptions of cerebro vascular anatomy and pathology. The clinical challenge posed by patients suffering from partial or complete transverse spinal lesions has grown due to new diagnostic and therapeutic approaches. Myelography using water-soluble contrast media, X-ray computed tomography, magnetic resonance imaging and spinal angiography today allow and require both earlier and topographically and pathogenetically more exact classification of diseases of the spinal cord and its surrounding structures. Due to progress in microneurosurgery and interventional neuroradiology, even intramedullary lesions have become more and more accessible and treatable. Therefore this monograph mainly addresses those concerned with invasive therapeutic techniques and who are familiar with the interpretation of radio anatomic findings. A comprehensive description of medullary vascular syndromes would be beyond the scope of this treatise.
