Book excerpt: In June 1987, neurobiologists, immunologists, molecular biologists, virologists and neurologists from several countries met in Vancouver to discuss recent advances of relevance to multiple sclerosis. The symposium was a part of the 22nd Canadian Congress of Neurological Sciences meeting and was sponsored by funds from the Multiple Sclerosis Society of Canada and the Medical Research Council of Canada. The presentations covered five major topics: basic neurobiology, molecular biology, the role of viruses in demyelination, immune function and dysfunction in multiple sclerosis, and clinical magnetic resonance imaging studies. It was heartening to note that scientists from several different disciplines were working towards a common end-point: the understanding and treatment of multiple sclerosis. In this book, speakers at the symposium have each presented a chapter of their findings and discussions. In addition, some non-participants at the symposium have been invited to submit chapters in order to give this volume a more complete scope. It is hoped that the reader will find this book a useful reference for several subjects of interest to multiple sclerosis. In closing, I would like to thank the following for their help and support: the Multiple Sclerosis Society of Canada and the Medical Research Council of Canada for their financial support; the contributors of this book for their manuscripts; Dr. A. Book excerpt: A satellite symposium entitled "Myelination and Demyelination: Recent Chemical Advances" was held in Helsinki from August 29 to 31, 1977, after the Sixth International Meeting of the International Society for Neurochemistry (ISN) in Copenhagen. Myelin is a nervous tissue structure that is most suitable as a subject of biochemical investigation. It is easy to isolate in a highly purified form, is rather stable even after death, and is af fected by a variety of neurological and other diseases. Its lesions are particularly important in the study of multiple sclerosis, a disease which is relatively prevalent in Finland and has therefore been of interest to a great many Finnish scientists. The first half of this book is concerned with the biochemical composition and molecular organization of myelin, the second half with the experimental and clinical aspects of demyelination. The comments given after each of the presentations at the symposium were not recorded as such, but each author was requested to modify his or her paper accordingly. The editing and publication of this book would not have been possible without the excellent efforts and co-operation of my wife, Mrs. L.-M. Book excerpt: The division of the mature mammalian brain and spinal cord into regions of "white" matter and "gray" matter is observable upon the most cursory inspection. Microscopic examination indicates that the "white" matter regions are characterized by a multilamellar sheath (myelin) which sur rounds the axons. A morphologically similar myelin imparts the white color to tracts of the peripheral nervous system, although, as is empha sized throughout the book, there are very significant morphological and chemical differences between central and peripheral myelin. The rapid postnatal accumulation of myelin is temporally associated with increasing functional capacity and presumably indicates some important role related to neuronal activity. The ability of myelinated axons to conduct electrical impulses much more rapidly and with much lower use of energy, relative to unmyelinated axons, is now generally accepted. As is evident from the above capsule description, a considerable body of information concerning myelin is available. However, as in other areas of research the unknown expands before us more rapidly than we are able to add to our core of knowledge. This is a corollary of the axiom that a good experiment raises more questions than it answers. Among the areas currendy under investigation in many laboratories are the following: interaction of myelin-forming cells with neurons, assembly and metabo lism of myelin, molecular architecture of myelin, and the details of the functional role of myelin.

The move towards oral immunomodulatory therapies has been a major advance, as has the finding of new genes linked to susceptibility that may open the door to new therapeutic approaches. However, a frontier that has been making significant strides in recent years has been that surrounding the neurobiology of myelin regeneration and axon protection: such have been the advances that clinical translation is on the cusp of being achieved. Two broad approaches to therapeutic enhancement of remyelination are envisaged: promoting endogenous remyelination by targeting cells present in the CNS, or, replacing lost myelinating cells from exogenous sources. Current research on oligodendrocyte biology, the pathology of MS, imaging of lesions and the biology of remyelination are paving the way toward opening this new translational frontier.
