Book excerpt: Cancer patients have multiple factors that contribute to anxiety; for instance, chemotherapy, radiation, and surgery. Analysis from multiple studies indicates music therapy is a beneficial intervention in reducing anxiety induced from cancer treatments. Music has no side effects; it is economical, easy, and proven effectiveness in relieving anxiety to cancer patients receiving treatments. Music was offered to cancer patients undergoing chemotherapy, radiation, port placement, mastectomy, and wound dressing changes. Patients were allowed to listen to music of their choice, on a MP3 that was provided, for 20-30 minutes prior to treatment and some patients were offered to listen to music during treatment as well. Results showed that music therapy had significantly reduced anxiety in cancer patients.

Book excerpt: The purpose of this study was to investigate the effects of familiar live music on the anxiety levels of patients undergoing chemotherapy treatment. Fifty patients randomly selected were assigned to 2 groups: experimental (n=25) and control (n=25). Pre and posttests consisted of questionnaires and the recording of the patient's heart rate and blood pressures. Subjects in the experimental group received 20 minutes of familiar live music during their chemotherapy treatment. Subjects in the control group received standard chemotherapy. It was assumed that those patients receiving music therapy intervention would: a) lower their anxiety levels; b) experience a decrease in heart rate and blood pressure; c) improve their levels of negative reactions including fatigue, worry, and fear; and d) improve their levels of positive reactions including comfort and relaxation. Results of the study showed statistically significant improvement for the experimental group on the measures of anxiety, fear, fatigue, relaxation, and diastolic blood pressure. No significant differences between groups were found for heart rate and systolic blood pressure. Descriptive values indicated that, on average, the experimental group was influenced positively by the music intervention, and participants improved their quality of life while undergoing chemotherapy treatment. The overall results confirmed that music intervention behaved as a powerful independent variable and significantly decreased levels of anxiety, fear, and fatigue, and increased the levels of relaxation for patients undergoing chemotherapy treatment.

Book excerpt: Patients coming into a facility for invasive procedures often times have a high level of anxiety about the procedure and the outcome, (Ghetti, 2012). Based on research studies, the theory of music therapy intervention can reduce the patient's anxiety before and during an invasive procedure, (Bradt, Dileo and Shim, 2013). Music is a welcome distraction proven to help the patient relax, lower blood pressure, heart rate, respiratory rate and cover up environmental noise, (Comeaux and Steele-Moses, 2013). Data reveals some patients need less sedation and pain medication to lessen base-line vital signs and anxiety levels if music therapy is used, (Matsota, Christodoulopoulou, Smyrnioti, Pandazi, Kanellopoulos, Koursoumi, E., and others, 2013). By implementing music therapy in phases through out the facility, education can be audited through the PDCA model, Plan Do Check Act for process improvement. Starting in the preoperative preparation area for patients having heart catheterizations will allow ample control over the study with less staff to train in the start of the project. This plan allows for the implementer to oversee the study with the assistance of the research committee. Patients will be educated on the trial when checking in for the procedure and given an option to participate. A VAS-A, Visual Analog Scale for Anxiety will be used before and after the procedure, (Abend, Dan, Maoz, Raz, and Bar-Haim, 2014). Patient satisfaction scores will be analyzed before and after the trial. The ultimate goal is to improve patient satisfaction scores by reducing level of anxiety, with the added benefit of possibly lowering cost per case. As evidence of less sedation involved allowing the patient to be dismissed sooner, (Bernatzky, Presch, Anderson and Panksepp, 2011).

Abstract: In Hong Kong, there has about 70% patients who attend Accident and Emergency Department (AED) is categorized as semi- or non-urgent cases. The medical conditions with the relative longer waiting time provoke the anxiety level. Furthermore the anxiety is worsen by the noisy waiting environment. These phenomena not only affect the psychological but also the physiological changes of patients. Therefore, there is a need to develop an evidence-based intervention that can effectively reduce the anxiety level during the waiting time. The music intervention is proposed to be implemented to reduce the anxiety in the AED setting as this intervention has been proven with a significant anxiolytic effect in variety of healthcare setting. This dissertation is a translational research that aims to identify the advantages or music intervention, develop an evidence-based practice (EBP) guideline for health care providers, and design an evaluation plan for evaluating the protocol effectiveness. The transferability and feasibility of the literature were high. The target setting is one of the AEDs in Hong Kong and the music intervention can be given in cubicle or waiting room by using Mp3 or speakers respectively. The music album is preferably chosen by patients and the intervention last for about 30-60 minutes. An implementation plan for pilot testing was developed to trying out the guideline and to reduce the potential problems during implementation. Comprehensive evaluation plans on stakeholders were developed. This dissertation contributes to develop an evidence-based protocol for healthcare-providers to follow and demonstrate in real situation.
