It has been increasingly recognized that maintaining skeletal health is an important factor in achieving longevity in healthy subjects. Diabetes has been established as a disease independently associated with sarcopenia and increased risk of bone fracture resulting from osteoporosis; therefore, it is reasonable to maintain that musculoskeletal health is important in preserving good health. As either bone or muscle is intimately involved in the regulation of metabolic status, keeping the musculoskeletal system healthy is important for improving abnormal glucose metabolism toward normal levels and is vital, as well, for maintaining normal activities of daily living. Muscle is a target of insulin for enhancing the entry of glucose; thus it is conceivable that sarcopenia and muscle containing fat streaks causes insulin resistance in diabetic patients. Furthermore, bone is an organ that regulates Ca and Pi levels in serum by releasing or resorbing Ca and Pi to and from bone tissue. It is now known that bone is a definitive endocrine organ for regulating glucose metabolism and Pi metabolism by secreting osteocalcin and FGF-23 from osteocytes/osteoblasts. Readers will learn of the recent findings in this area, and this book will benefit physicians who deal with diabetes, particularly orthopedists and bone specialists, as well as all physical therapists.

SSDI provides disability benefits to people (under the full retirement age) who are no longer able to work because of a disabling medical condition. SSI provides income assistance for disabled, blind, and aged people who have limited income and resources regardless of their prior participation in the labor force. Both programs share a common disability determination process administered by SSA and state agencies as well as a common definition of disability for adults: "the inability to engage in any substantial gainful activity by reason of any medically determinable physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than 12 months." Disabled workers might receive either SSDI benefits or SSI payments, or both, depending on their recent work history and current income and assets. Disabled workers might also receive benefits from other public programs such as workers' compensation, which insures against work-related illness or injuries occurring on the job, but those other programs have their own definitions and eligibility criteria. These manifestations, which are some of the causes of chronic disability had been poorly reported. The musculoskeletal disorder causes considerable morbidity and the reduction in the quality of life of patients, these can have improved if these disorders are diagnosed early. There is a paucity of data on musculoskeletal complications in diabetic mellitus. Studies have reported a higher prevalence of musculoskeletal disorders in diabetic patients. However, no systematic review of literature that described the prevalence of musculoskeletal disorders among patients with diabetes.Objective To determine the prevalence of musculoskeletal disorders among patients with diabetes mellitus and establish the most common body area affected by musculoskeletal disorders.Methods: We conducted a literature search of an electronic database of CINAH, Pubmed, Web of Science and Google Scholar using the following keywords as the search term Musculoskeletal disorders and Diabetes mellitus. The screening was also performed and a duplicate of records was also removed. Assessment of methodological quality for prevalence and incidences studies for each included study was also performed. Pooled estimates of prevalence with 95% confidence intervals were calculated using a meta-analysis of proportion for overall musculoskeletal disorders prevalence and the most common body area of musculoskeletal disorders reported.Results: Five thousand and eighty-eight records were identified from the database; twenty-one studies fulfilled the inclusion criteria and were incorporated into the review. The methodological quality scores range from 2 to 8 points the studies were classified as high with the point of > 6 and moderate > 4 points and low

Book excerpt: Diabetes mellitus has become a common disease. It causes a complicated physiological cascade of changes to the body, including the skeleton. Diabetes is characterized by hyperglycemia because of insulin deficiency or insulin resistance. Type 2 diabetes is the most common type of diabetes. Although symptoms of diabetes have been documented throughout history, the effects of diabetes on bone are indirect and not well understood. At least thirteen musculoskeletal disorders (MSDs) have been associated with diabetes in modern medical literature. Most of these disorders result from changes to bone quality, primarily through the accumulation of byproducts from hyperglycemia. The MSDs included in this research are hyperostosis frontalis interna, periodontal disease, frozen shoulder, hand-related disorders, carpal tunnel syndrome, diffuse idiopathic skeletal hyperostosis, peripheral neuroarthropathy, gout, lower-extremity amputation, degenerative joint disease, osteoporosis, osteomyelitis, and fractures. This research developed new methods for determining the effects of diabetes on the severity of the MSDs and to identify which MSDs can be used to predict diabetes in human skeletal material from three modern anatomical collections. This research also applied the new methods to four Midwestern archaeological precontact American Indian skeletal collections to detect a change in the prevalence of diabetes-associated MSDs as the consumption of carbohydrates increased with the adoption of maize into the diet. The results from the modern skeletal material suggest there is a suite of MSDs that are consistently more severe in those with diabetes than in those without diabetes. This suite of MSDs provides a predictive model for the potential diagnosis of diabetes in the past. The predictive model was then used to identify three precontact American Indian individuals as potential case studies of the earliest and only case studies to date of diabetes in precontact American Indians. Overall, this research expands on what is known about how diabetes affects skeletal material and what diabetes may look like in the archaeological record. The more MSDs present, the stronger the likelihood diabetes may be diagnosed. However, a definitive diagnosis of diabetes may never be known without supporting medical documentation.
