It has been encountered in all races, in every region of the world and shows no preference for either sex. In general, its initial manifestations begin in the fifth decade of life. With the world population showing an increase in numbers of people in the older age groups, Parkinson's disease will undoubtedly be encountered with increased frequency in the years to come. Though its cause is unknown, significant strides in understanding its nature and controlling its symptoms have been made during the past two decades. Contained in this volume is a comprehensive review of the present knowledge of Parkinson's disease. Though James Parkinson is credited with the uncovering the illness which now bears his name, and his monograph on the Shaking Palsy written in 1817, is truly a medical classic, descriptions of this disease can be found in medical writings going back to the time of Galen. Indeed, he himself was the first to admit that he was not describing a new disease but bringing an old one to the attention of the medical researchers of his time so that it would become a subject of interest and investigation. Specifically, his objective was the morbid anatomist whose efforts he hoped would bring to light the cause and nature of the disorder. This, then, would become a basis for· treatment or even cure.

Book excerpt: Many extrapyramidal motor diseases result from underlying neurodegenerative processes, which however are very advanced at the time the clinical diagnosis is made. From animal experiments it seems likely that we may soon have several substances that provide neuroprotective effects, but that their therapeutic use must be as early as possible. This fact was the starting point for the meeting, held at Chiemsee, that formed the basis of this book. The idea was to develop in strumental methods that, first, permit an earlier diagnosis by drawing on the experience of experts and, second, provide a better resolution of successful therapy than the currently used methods. As early as 1886, Charcot tried to differentiate the tremor of polysclerosis and Parkinson patients by using a drum developed by Marey, a physiologist. Subse quently, many apparatuses were developed to better identify movement disorders, initially for diagnostic purposes. Most of these methods proved to be inferior to the skill of experienced observers. In the last two decades, however, both the technical advances made in measuring methods and the improvements achieved in methods of evaluation have led to marked advances in the quantification of motor dis turbances. An objective of the symposium held at Chiemsee and of this book is to provide an overview of the procedures that are currently commonly used for extrapyramidal movement disorders, and to identify their potential and limitations.

The Symposium was sponsored by the Department of Pharmacology and Therapeutics and the Tarbox Parkinson's Disease In stitute of the Texas Tech University School of Medicine at Lubbock. The Tarbox Parkinson's Disease Institute was established in 1973 with funds appropriated by the State of Texas and is dedicated to re search, patient care, and educational activities related to Parkinson's disease. The Institute is named after Mr. Elmer L. Tarbox, who recently served the Lubbock area as a Representative to the Texas Legislature, and is himself a parkinsonian patient. Mr. and Mrs. Tarbox attended the Symposium as honored guests. The First Tarbox Parkinson's Disease Symposium was devoted to both basic and clinical aspects of Parkinson's disease, with an emphasis on discussion of drug therapy. This discussion focused not only on readily available drugs such as levodopa, but also on new investigational drugs such as bromocriptine. Thanks are especially due to Thelma Saunders and the office staff of the Department of Pharmacology and Therapeutics, Texas Tech Univer sity School of Medicine, for their excellent service in producing camera-ready copy for the Publisher.
