Book excerpt: Numerous studies have shown that elevated levels of circulating tumor cells (CTCs) in blood of cancer patients are associated with poor response to treatment and inferior survival probabilities. Despite this clinical significance, the molecular biology of CTCs remains poorly understood. The paucity in molecular information can be attributed to the tremendous technical challenges involved in isolating these extremely rare cells. Recent technological advancements in rare-cell technology, however, have allowed for the reliable enrichment and isolation of CTCs. Consequently, the use of recently developed molecular approaches —e.g., multiplexed QPCR, microarray, and next generation sequencing analyses— to profile CTCs have provided novel insights into the molecular makeup of these tumor cells. This book discusses approaches for enrichment and isolation of CTCs as well as recent advances in comprehensive molecular profiling of CTCs using cutting-edge omics technology.

Intriguingly, the state of art on cancer biomarkers research shows interesting and promising results together to clamorous failures. Also from a clinical point of view, there are contradictory results on routine clinical use of the present cancer biomarkers. Some patients may be simply monitored in their course by a periodic blood sample, but sometimes this monitoring shows dramatic limits. A lot of patients show serious and extensive relapses without significant change in serum concentrations of biomarkers tested. Often the physician who should utilize these biomarker does not entirely know their limits and the total potential applications as well and sometimes this knowledge is influenced by economical and marketing strategies. This limited and "polluted" knowledge may have dramatic consequences for patient. The aim of this book is to diffuse all aspects of cancer biomarkers, from their biochemical peculiarities to all clinical implications by passing through their physiology and pathophysiology. This critical approach towards old and new cancer biomarkers should foster a deepened and useful understanding of the diagnostic and prognostic index of these fundamental parameters of laboratory medicine and in the same time facilitating the research of new and more sensitive-specific signals of the cancer cell proliferation.

Once activated, they may have either pro- or anti-inflammatory effects that include pathogen killing, safe disposal of apoptotic cells or tissue renewal. The activation state of macrophages is conceptualized by the so-called M1/M2 model of polarization. M2 macrophages are not simply antagonists of M1 macrophages; rather, they represent a network of tissue resident macrophages with roles in tissue development and organ homeostasis. M2 macrophages govern functions at the interfaces of immunity, tissue development and turnover, metabolism, and endocrine signaling. Dysfunction in M2 macrophages can ruin the healthy interplay between the immune system and metabolic processes, and lead to diseases such as insulin resistance, metabolic syndrome, and type 1 and 2 diabetes mellitus. Furthermore, M2 macrophages are essential for healthy tissue development and immunological self-tolerance. Worryingly, these functions of M2 macrophages can also be disrupted, resulting in tumor growth and autoimmunity. This book comprehensively discusses the biology of M2 macrophages, summarizes the current state of knowledge, and highlights key questions that remain unanswered.

The tumor’s increased cell signalling due to excessive transcription factor activation and increased stellate cell activity leads to collagen deposition formation of a dense stroma around the tumor, which prevents drugs from reaching the malignant cells. This leads to tumor chemoresistance. To circumvent these difficulties, drug therapy targeting the tumor’s specific microenvironment and the additive anticancer effect of phytochemicals can allow for more effective treatment. This volume will be the first on the market on the topic of phytochemicals and their effect on the tumor microenvironment (TME). TME is an emerging area of research and the book will be a welcome introductory addition to the field.
