Book excerpt: Résumé : les maladies parodontales sont des maladies inflammatoires à étiologie infectieuse, dont la thérapeutique initiale non chirurgicale consiste à supprimer mécaniquement les pathogènes par détartrage et curetage radiculaire. En fonction des caractéristiques de la lésion, l'approche chirurgicale trouve également ses indications, afin de réduire la charge bactérienne et l'inflammation. Ces procédures conduisent à la réduction de profondeur de poche et un gain d'attache clinique, mais ne conduisent pas à la régénération des tissus parodontaux en soi car elles ne restituent ni la forme, ni la fonction des tissus détruits. Nous nous intéressons de plus près à la modulation de la réponse infectieuse en tant que thérapie adjuvante, en parodontologie. L'utilisation de médicaments anti-inflammatoires non-stéroïdiens, notamment les inhibiteurs sélectifs de Cox2, ont montré une amélioration des résultats sur les tissus parodontaux après débridement mécanique. Cependant, l'intérêt de l'usage systémique de médicaments anti-inflammatoires en parodontologie reste limité et discutable, en raison des nombreux effets indésirables liés à l'administration de ces molécules. Afin de palier à ces risques, une nouvelle approche thérapeutique consiste à cibler de plus près les acteurs de la réponse inflammatoire dans la cicatrisation parodontale. Différentes stratégies ont été proposées afin de promouvoir la régénération parodontale via la modulation de cytokines, chimiokines et enzymes dérivées de l'hôte, dans le but d'inhiber les voies responsables de la destruction tissulaire et d'activer la prolifération et différenciation tissulaire. L'absence de régénération tissulaire est également due à la compétition tissulaire entre les différents tissus parodontaux et les taux variables de prolifération, maturation et différentiation des cellules parodontales lors de la cicatrisation, d'où l'usage des membranes en tant que barrière physique. Ces dernières années, ces membranes ont évolué jusqu'à devenir de véritables systèmes de délivrance de facteurs de croissance et différenciation et d'efficaces régulateurs de la réponse inflammatoire, rendant la perspective de régénération parodontale vraie envisageable.

Book excerpt: Control of periodontal infection and inflammation is crucial for optimal periodontal wound healing and regeneration. For this purpose, three different and novel strategies were developed and tested for their impact on periodontal wound healing parameters in vitro and in vivo. Firstly, an ibuprofen-functionalized polycaprolactone (IBU-PCL) membrane was developed as an anti-inflammatory barrier membrane that successfully reduced inflammatory markers expression in gingival cells in vitro and decreased soft tissue inflammation, thus, improving periodontal tissue healing in an experimental periodontitis model in vivo. Secondly, chlorhexidine and ibuprofen containing in-situ forming implant (CHX-IBU ISFI) was developed to target both infection and inflammation that successfully reduced Porphyromonas gingivalis growth and inflammatory response of gingival cells in vitro as well as improved soft tissue periodontal wound healing in vivo. Lastly, a thermosensitive chitosan-based hydrogel functionalized with atorvastatin encapsulated in a nano-emulsion (ATV-KELP NE) was characterized and used to treat an induced bone defect in vivo that resulted in improved soft and hard tissue healing by counteracting infection and modulation of immuno-inflammatory response.

Book excerpt: Ce projet porte sur l'impact de l'hyperglycémie sur la sécrétion de cytokines pro-inflammatoires et de métalloprotéinases matricielles par les cellules gingivales à la suite d'une stimulation de la réponse inflammatoire par des biofilms sous-gingivaux dysbiotiques. La maladie parodontale est une maladie inflammatoire induite par l'infiltration de bactéries parodontopathogènes et conduit à la destruction irréversible de l'ensemble des tissus de soutien de la dent. Celle-ci induit une inflammation qui joue un rôle important dans le développement d'une dysbiose du microbiome sous-gingival. Il a été suggéré qu'une dysbiose du microbiome sous-gingival augmentait le risque de parodontite chez des patients diabétiques à cause d'une inhibition de leurs réponses immunitaires et métaboliques. Cependant, l'impact de l'hyperglycémie sur la régulation de la réponse inflammatoire gingivale est peu connu et la modulation de cette réponse est peu étudiée. Nous avançons l'hypothèse qu'un environnement hyperglycémique peut affecter les interactions hôte-microbiome et par conséquent influencer la réponse inflammatoire d'un patient atteint de parodontite. Pour cela nous avons étudié la réponse inflammatoire d'un modèle de co-culture de fibroblastes gingivaux et de macrophages stimulés par des échantillons de plaque dentaire provenant d'individus sains ou atteints de parodontite, et ce, en présence de concentrations normales ou élevées de glucose. Le profil de sécrétion de cytokines pro-inflammatoires et métalloprotéinases matricielles a été par la suite analysé. Nous avons observé une augmentation significative des niveaux de sécrétions de plusieurs cytokines reflétant l'impact de l'environnement hyperglycémique sur la modulation de l'inflammation. Les résultats que nous avons obtenus permettront éventuellement de planifier des traitements parodontaux personnalisés et adaptés, ainsi que de développer des stratégies de prévention individualisées pour mieux contrôler le développement et la progression de la maladie parodontale chez les patients diabétiques.
