Book excerpt: Modern energy access is an important determinant of human health, as it plays a critical role in the capabilities of healthcare facilities and aids in the development of clean and safe household environments. But in the developing world, thousands of healthcare facilities and hundreds of millions of households lack access to modern energy services. And the situation will only get worse as the energy needs of communities and the health sectors in these countries are expected to increase dramatically in the years ahead. This special feature begins with a look at energy access and health implications, along with energy access and reliability gaps, at two levels: electrification of healthcare facilities and household energy. It then tackles the barriers to better energy access and reliability, and concludes with opportunities and options-including decentralized renewable energy, hybrid solar PV/diesel, grid extension, energy-efficient medical equipment, a greater availability of clean energy sources and technologies (such as cleaner cook stoves and fuels). The paper finds that addressing this unmet need in an efficient and timely matter will require decision-makers tobetter incorporate health considerations into energy policy, and vice-versa. The UN's 17 Sustainable Development Goals (SDGs) provide an opportunity to apply a nexus approach to energy and health-building on various linkages that energy and health have with sustainable development(including SDG 5 on gender equality, SDG 11 on sustainable urban environments, and SDG 13 on climate action).

Book excerpt: Energy is linked to many of the sustainable development priorities, including public health, gender equality, food security, clean water, education, economic growth, youth and women’s empowerment and climate action. The absence of clean and sustainable energy has a significant adverse impact on the health and livelihoods of the poorest populations. Around 2.3 billion people, mostly in low- and middle- income countries (LMICs), still mainly rely on inefficient and polluting fuels and stoves for daily cooking, exposing them to health-damaging emissions that result in millions of premature deaths each year. Health and livelihood risks are greatest for women and children as they are often tasked with cooking and gathering fuel. The traditional use of biomass for cooking also puts the environment and climate at risk by contributing to deforestation and greenhouse gas emissions. Around 1 billion people worldwide, mostly in poor and rural communities, are estimated to be served by health care facilities without any electricity or reliable power supply, and only one in two hospitals in sub-Saharan Africa has a reliable electricity supply. Without adequate and reliable power, essential medical devices cannot function, basic amenities remain out of reach and delivery of health care services is of substandard quality. Unless rapid action is taken, the world will not meet its targets under the 2030 Agenda for Sustainable Development and the Paris Agreement. In November 2018, during the first World Health Organization (WHO) Global Conference on Air Pollution and Health, Dr Tedros Adhanom Ghebreyesus, Director-General of WHO, announced his vision to build a Health and Energy Platform of Action (HEPA) to enhance multisectoral collaboration between the health and energy sectors and mobilize the political and financial commitments needed to achieve Sustainable Development Goals (SDGs) 3 and 7. In response, WHO, the United Nations Department of Economic and Social Affairs (UN DESA), the United Nations Development Programme (UNDP) and the World Bank, in cooperation with the International Renewable Energy Agency (IRENA) and other key stakeholders, launched HEPA in May 2019, with an initial focus on improving health through promoting clean fuels and technologies for cooking and accelerating access to electricity for health care facilities. Following this announcement, in July 2020, HEPA members worked together to develop a strategic workplan with five overarching objectives to strengthen the cooperation between the health and energy sectors and elevate energy access as a key development priority area for public health, gender equity, climate and sustainable development more broadly (see Box 1). HEPA has gained significant momentum since its establishment in 2019. By October 2022 the platform had over 20 member organizations. All members are committed to working together and synergizing efforts on the ground to accelerate universal electrification of health care facilities and clean cooking. This HEPA action report summarizes the key activities and results achieved and highlights activities under each of HEPA’s five objectives.

The briefing has been structured to permit public health practitioners with limited time to obtain a well-rounded perspective of the topic by reading only sections 1 and 3. The three sections are: 1. Energy sector overview. This covers mutual interests that both sectors share, global trends in energy, and policy challenges from the perspective of the energy sector characterized as goals. These goals are proposed as a result of a review of policy documents and from interviews with energy sector practitioners in order to consider different country contexts and realities. 2. Goals 1 to 4. The second part of the briefing analyses each goal, with a detailed description of policy approaches, health impacts, and areas for potential collaboration between sectors. 3. Book excerpt: This flagship report of the East Asia and the Pacific Region of the World Bank outlines an ambitious course of action to eradicate energy poverty in the Region by 2030. Despite its impressive economic growth, over 1 billion people in the EAP Region still lack the most basic access to electricity and modern cooking solutions. With every second household in the Region depending on solid fuels for cooking, indoor air pollution is a major health risk factor, particularly for women and children. This flagship report urges the governments of EAP countries to work simultaneously on two paths. The first path is to achieve universal electricity access by accelerating both grid and off-grid programs while employing appropriate policies and innovative technical solutions to reduce costs, improve reliability, and provide timely service to all EAP households. The second path calls for a major push to increase access to clean cooking fuels (natural gas, liquefied petroleum gas, and biogas) and to advanced cooking stoves, particularly for biomass in poor rural areas. Both of these paths are affordable. The combined investments required for universal access to modern energy in the EAP Region are estimated at US$78 billion over the next 2 decades. This amount is US$32 billion more than the amount required to maintain the "business-as-usual" scenario over the same period. Although the annual incremental investment will be a small fraction of the Regional GDP, most EAP countries will require significant support from donors and multilateral institutions to reach the goal of universal access to modern energy by 2030.

The influence of humans on climate change has been established through extensive published evidence and reports. However, the connections between climate change, the health of the planet and the impact on human health have not received the same level of attention. Therefore, the global focus on the public health impacts of climate change is a relatively recent area of interest. This focus is timely since scientists have concluded that changes in climate have led to new weather extremes such as floods, storms, heat waves, droughts and fires, in turn leading to more than 600,000 deaths and the displacement of nearly 4 billion people in the last 20 years. Previous work on the health impacts of climate change was limited mostly to epidemiologic approaches and outcomes and focused less on multidisciplinary, multi-faceted collaborations between physical scientists, public health researchers and policy makers. Further, there was little attention paid to faith-based and ethical approaches to the problem. The solutions and actions we explore in this book engage diverse sectors of civil society, faith leadership, and political leadership, all oriented by ethics, advocacy, and policy with a special focus on poor and vulnerable populations. The book highlights areas we think will resonate broadly with the public, faith leaders, researchers and students across disciplines including the humanities, and policy makers.

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.
