Book excerpt: This qualitative descriptive study explored the data from twenty-nine nursing homes in Missouri in which opportunity for resident care improvement was identified; to determine presence, and absence, of principles of the Everyday Excellence for Professional Nursing Practice in Long-Term Care Framework. Secondary data from a larger NINR-funded study were analyzed. That larger study was designed to test an intervention focused on building organizational capacity to create and sustain improvement in quality of care in twenty-nine nursing homes with identified opportunity for improvement in resident care. Findings of this secondary analysis identified presence of all principles of this Framework in twenty-nine homes receiving the intervention, although frequency was limited and variable. Select Quality Measure scores, as defined by the Centers for Medicare and Medicaid Services, for each facility are also described. Findings of this study suggest a positive association between presence of select principles of the Everyday Excellence for Professional Nursing Practice in Long-Term Care Framework and improvement in select Quality Measure scores. Additional research is needed to support implementation and evaluate effectiveness of Professional Practice Models in long-term care and their impact on quality outcomes for older adults residing in nursing homes.

The book compares key features of long-term care insurance (LTCI) schemes in 15 pilot cities and evaluates the sustainability of various financing models adopted by the cities in the LTCI schemes. The book uses an interpretive case study approach to give an in-depth look into the LTC models in three pilot cities – Qingdao, Nantong, and Shanghai. The three cities represent three different models of financing and delivering LTC. To assess how effective the LTC models in these three cities are, the book uses five criteria, including utilization of medical resources, cost, equity, quality of care and sustainability. Also, the authors discuss how the financing and delivery of LTC can be improved in China, the impact of the 2019 coronavirus (COVID-19) pandemic on older adults in need of LTC in the country and the implications of China’s LTCI reform for other countries. The book will be a useful reference to scholars and policy-makers who look at urban ageing and healthcare costs and delivery.

Book excerpt: Individuals face significant late-in-life risks, prominently including the need for long-term care (LTC). Yet, they hold little long-term care insurance (LTCI). In this paper we use a structural model and a purpose-designed dataset to understand the determinants of insurance demand. We distinguish between a fundamental lack of desire to insure, crowd out from existing insurance, and unmet demand due to poor products available in the market. The model features individual-specific non-homothetic health-state-dependent preferences over normal consumption, consumption when in need of long-term care, and bequests, which are estimated using strategic survey questions. To account for differences between the modeled and measured insurance products, we study not only individuals' holdings of LTCI, but also their stated demand for an idealized product that mirrors that in the model. We find that many individuals would purchase LTCI and receive a large consumer surplus if it were a better product, while many others do not want to purchase even high-quality actuarially fair LTCI due to the values of their heterogeneous state-dependent preferences, their demographics, and their financial situation.

However, there often is not a strong link between medical care provided in the home and the necessary social services and supports for independent living. Home health agencies and others are rising to the challenges of meeting the needs and demands of these populations to stay at home by exploring alternative models of care and payment approaches, the best use of their workforces, and technologies that can enhance independent living. All of these challenges and opportunities lead to the consideration of how home health care fits into the future health care system overall. On September 30 and October 1, 2014, the Institute of Medicine and the National Research Council convened a public workshop on the future of home health care. The workshop brought together a spectrum of public and private stakeholders and thought leaders to improve understanding of the current role of Medicare home health care in supporting aging in place and in helping high-risk, chronically ill, and disabled Americans receive health care in their communities. Through presentations and discussion, participants explored the evolving role of Medicare home health care in caring for Americans in the future, including how to integrate Medicare home health care into new models for the delivery of care and the future health care marketplace. The workshop also considered the key policy reforms and investments in workforces, technologies, and research needed to leverage the value of home health care to support older Americans, and research priorities that can help clarify the value of home health care. This summary captures important points raised by the individual speakers and workshop participants.
