Drawing on a rich array of oral histories, archival records, newspapers, films, and photographs from post–World War II New York City, Carroll shows how poor people transformed the antipoverty organization Mobilization for Youth and shaped the subsequent War on Poverty. Highlighting the little-known National Congress of Neighborhood Women, she reveals the significant participation of working-class white ethnic women and women of color in New York City's feminist activism. Finally, Carroll traces the partnership between the AIDS Coalition to Unleash Power (ACT UP) and Women's Health Action Mobilization (WHAM!), showing how gay men and feminists collaborated to create a supportive community for those affected by the AIDS epidemic, to improve health care, and to oppose homophobia and misogyny during the culture wars of the 1980s and 1990s. As of 2003, there were nearly 27 million men, women, and children suffering from AIDS in sub-Saharan Africa. Today that number has been reduced by more than half. The number of people with access to antiretroviral drugs--a treatment which renders AIDS survivable rather than fatal--has gone from around 50,000 to more than 11 million. All of this is thanks to a Bush administration program known as PEPFAR. Even on the day of its launch during the 2003 State of the Union, no one much noticed it. It cost a fraction of a percentage of the overall budget and was far less expensive than the Iraq war, effectively announced on the same day. Yet PEPFAR is, according to journalist Emily Bass, "the best thing America has done beyond our borders in this century." To End a Plague is not merely a history of this extraordinary program; it describes the cost of success in our broken political system. PEPFAR was likely a cynical political ploy--a "legislative trophy" as the New York Times described it--and its overseers, including the now-famous Coronavirus Task Force leader Deborah Birx--had to make moral and political compromises to keep it from being shut down. Yet, preventative measures are not implemented universally and high costs of medicines limits treatment efforts. The global response to HIV/AIDS faced similar issues, but overcame them through a global movement that brought attention to the crisis and ultimately resulted in the creation and implementation of and access to better tools for HIV prevention and treatment. This also included effective policies and programs behind and supporting the movement. Such could be done for hepatitis, specifically using lessons from the HIV response. Here, we will discuss the current and potentially severe future burden of hepatitis globally, the challenges in addressing this epidemic, and how principles applied from the global HIV response can facilitate a successful and similar hepatitis movement.

Book excerpt: Civil Society Organizations (CSOs) can make a vital contribution to public health and health systems but harnessing their potential is complex in a Europe where government-CSO relations vary so profoundly. This study is intended to outline some of the challenges and assist policy-makers in furthering their understanding of the part CSOs can play in tandem and alongside government. To this end it analyses existing evidence and draws on a set of seven thematic chapters and six mini case studies. They examine experiences from Austria Bosnia-Herzegovina Belgium Cyprus Finland Germany Malta the Netherlands Poland the Russian Federation Slovenia Turkey and the European Union and make use of a single assessment framework to understand the diverse contexts in which CSOs operate. The evidence shows that CSOs are ubiquitous varied and beneficial and the topics covered in this study reflect such diversity of aims and means: anti-tobacco advocacy food banks refugee health HIV/AIDS prevention and cure and social partnership. CSOs make a substantial contribution to public health and health systems with regards to policy development service delivery and governance. This includes evidence provision advocacy mobilization consensus building provision of medical services and of services related to the social determinants of health standard setting self-regulation and fostering social partnership. However in order to engage successfully with CSOs governments do need to make use of adequate tools and create contexts conducive to collaboration. To guide policy-makers working with CSOs through such complications and help avoid some potential pitfalls the book outlines a practical framework for such collaboration. This suggests identifying key CSOs in a given area; clarifying why there should be engagement with civil society; being realistic as to what CSOs can or will achieve; and an understanding of how CSOs can be helped to deliver.
