The papers in this collection present fine-detailed ethnographic studies of cultures in Africa and Oceania, with a focus primarily on MDG 3, targeted to “promote gender equality and empower women” and MDG 5, targeted to “improve maternal health” to ascertain whether or not these goals have made or missed their mark. Ethnographic case studies located in Solomon Islands, Marshall Islands, Federated States of Micronesia, Papua New Guinea, Vanuatu, Ghana, Malawi, Cameroon, and South Ethiopia show that women in these cultures, regardless of nation state, face the same issues or problems—lack of empowerment, gender inequities, and inadequate access to cultural or state resources—to realize good health in general and good maternal and reproductive health, in particular.

Answering some of the same how and why questions found in Terence E. Hays’ Ethnographic Presents: Pioneering Anthropologists in the Papua New Guinea Highlands (1993), First Fieldwork begins where that collection left off in the 1950s and covers a broader selection of Pacific Islands societies and topics. Chapters range from candid reflections on working with little-known peoples to reflexive analyses of adapting research projects and field sites, in order to better fit local politics and concerns. Included in these accounts are the often harsh emotional and logistical demands placed on fieldworkers and interlocutors as they attempt the work of connecting and achieving mutual understandings. Evident throughout is the conviction that fieldwork and what we learn from and write about it are necessary to a robust anthropology. By demystifying a phase begun in the mid-1980s when critics considered attempts to describe fieldwork and its relation to ethnography as inevitably biased representations of the unknowable truth, First Fieldwork contributes to a renewed interest in experiential and theoretical nuances of fieldwork. Looking back on the richest of fieldwork experiences, the contributors uncover essential structures and challenges of fieldwork: connection, context, and change. What they find is that building relationships and having others include you in their lives (once referred to as “achieving rapport”) is determined as much by our subjects as by ourselves. As they examine connections made or attempted during first fieldwork and bring to bear subsequent understandings and questions—new contexts from which to view and think—about their experiences, the contributors provide readers with multidimensional perspectives on fieldwork and how it continues to inspire anthropological interpretations and commitment. A crucial dimension is change. Each chapter is richly detailed in history: theirs/ours; colonial/postcolonial; and the then and now of theory and practice. While change is ever present, specifics are not. Reflecting back, the authors demonstrate how that specificity defined their experiences and ultimately their ethnographic re/productions.

Though their access to the United States is unusually easy, through a unique migration agreement, it keeps them in a perpetual liminal state as nonimmigrants, who never fully belong as part of the United States Chuukese women move to Guam, sometimes with their families but sometimes alone, in search of a better life: for jobs, for the education system, or to access safe health care. Yet, the imperial system they encounter creates underlying conditions that greatly and disproportionately impact their ability to succeed and thrive, negatively impacting their reproductive health. Through clinical and community ethnography, Sarah A. Smith illuminates the way this system stratifies women’s reproduction at structural, social, and individual levels. Readers can visualize how U.S. imperialist policies of benign neglect control the body politic, change the social body, and render individual bodies vulnerable in the twenty-first century but also how people resist.

The studies are against the background that in September 2000, world leaders from 189 countries, including 147 Heads of State, gathered at the United Nations General Assembly to consider the challenges of the new millennium. They adopted the Millennium Declaration, which set out a vision for inclusive and sustainable globalization: UN 2000 (A/RES/55/2). The leaders pledged to work towards ensuring that conditions of extreme poverty are eradicated wherever they existed. To realise this declaration, the UN established eight MDGs to be achieved by 2015. The goals were broken down into 18 concrete targets and 48 indicators to track progresses in implementation. For the years lost 2000, countries in sub-Saharan Africa have been striving to achieve the goals. So far, some have achieved some of the goals, and the results toward the rest of the goals are also by and large positive, though off-target.

Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.
