Book excerpt: Minimally invasive joint replacement has become one of the hottest topics in the orthopedic world. In the case of the knee, however, this concept has been widely misunderstood. All too frequently it is taken to mean keyhole surgery in which traditional components are implanted with a shorter surgical time. Such an approach in fact has few benefits and various possible dangers. In this book, international experts discuss truly minimally invasive knee reconstruction techniques that involve tissue-sparing surgery and small implants and permit preservation of both the anterior and the posterior cruciate ligament. These techniques have the key advantage of respecting the physiological joint biomechanics. They also entail minimal removal of bone tissue from the tibia and only a small skin incision that avoids damage to the extensor apparatus. All of the most innovative techniques used in uni- and bicompartmental knee reconstruction are covered in detail, including computer-assisted procedures. Long-term results are reported, and pitfalls, highlighted. In no other way can one learn to recognize chronic problems. Once this step has been mastered it can be seen that the reconstructive procedures that Muller advocates do, for the most part, follow the laws of Nature rather than create the kind of abnormalities which often disrupt natural anatomical func tion.

Prosthetic design and operative technique have been greatly improved since then, and there is now an accepted standard concept of total knee arthroplasty. Thirteen years after the London symposium, another international symposium on total knee replacement was held, this time in Nagoya, Japan, in 1987. Its ambitious objective was to push forward the frontiers of continuous investigation and improve ment of total knee replacement. The fruits of the individual efforts presented at the Nagoya symposium were published in a volume of proceedings entitled Total Knee Replacement. In the years since 1987, further investigations have been conducted in various parts of the world regarding prosthetic design, fixation, long-term radiological follow-up, biomechanical evaluation, and biomaterials research. In knee ligament reconstruc tion, rapid progress has been made in the past five years in clinical practice and fundamental research by means of arthroscopic surgery and tissue transplantation, and we have come close to establishing a standard treatment. Under these circumstances, an international symposium on knee joint reconstruc tion was planned for 1994, again to be held in Nagoya, to provide ample opportunity for exchanging information and sharing clinical experience from around the world.
