The global financial crisis that began in 2008 compounded the burden of high food prices, exacerbating the problems of hunger and malnutrition in developing countries. The tandem food price and economic crises struck amidst the massive, chronic problem of hunger and undernutrition in developing countries. National governments and international actors have taken a variety of steps to mitigate the negative effects of increased food prices on particular groups. The recent abrupt increase in food prices, in tandem with the current global economic crisis, threatens progress already made in these areas, and could inhibit future efforts. The Institute of Medicine held a workshop, summarized in this volume, to describe the dynamic technological, agricultural, and economic issues contributing to the food price increases of 2007 and 2008 and their impacts on health and nutrition in resource-poor regions. The compounding effects of the current global economic downturn on nutrition motivated additional discussions on these dual crises, their impacts on the nutritional status of vulnerable populations, and opportunities to mitigate their negative nutritional effects.

Available in PDF, EPUB and Kindle. Book excerpt: "Background: Current nutrition trends indicate that malnutrition and food insecurity are not only a matter of undernutrition but also overnutrition. This nutrition transition not only affects individuals of low income levels, but all corners of the world and individuals of all ages, sexes, and income classes. This is especially important in the middle income class where demands for processed foods are at a high.Objective: To explore the effect of the size of national middle class on global food security, as well as to determine the factors impacting this relationship. Methodology: Data were collected from the 2014 Gallup World Poll cross-sectional survey. Data from 124 countries were analyzed, for a total of 157,060 respondents from nationally representative samples, of ages 15 years and older. Considering its validation worldwide by the Food and Agriculture Organization, the Food Insecurity Experience Scale was used to assess respondents' food security status. By using annual household income expressed in international dollars, Lorenz curves were produced to define middle income class in each country as those in the middle 60% of national household income. Spearman's rank correlations, Type II Regressions, and Generalized Linear Models (GLMs) were employed to assess the relationship between middle class size (independent variable) and the prevalence of food insecurity (the dependent variable) at a global and regional scale. Data were disaggregated based on sex and rural-urban distribution at the global scale. Regions explored were Europe, Latin America and the Caribbean, Africa, and Asia. Full models controlled for variables such as coefficient of variation in national incomes, rate of GDP growth, community basics, national institutions, and corruption. All tests were evaluated at 0.05 significance level. Results: At a global level, national size of middle income groups was found to be negatively associated with food security even when data for rural and urban women and men were analyzed separately. A similar association was found in Africa, Europe, and Latin America and the Caribbean, but not in Asia. Global GLMs demonstrated negative predictions of variation in national incomes and community basics index, as well as a positive prediction of GDP growth rate, for prevalence of food security. Conclusion: These findings suggest that global food security may be diminished by the expansion of middle class. This warrants for further investigation in this income group to determine the causes of food insecurity, as well as to provide economic and food stability for those vulnerable to this phenomenon." --

Book excerpt: New evidence this year corroborates the rise in world hunger observed in this report last year, sending a warning that more action is needed if we aspire to end world hunger and malnutrition in all its forms by 2030. Updated estimates show the number of people who suffer from hunger has been growing over the past three years, returning to prevailing levels from almost a decade ago. Although progress continues to be made in reducing child stunting, over 22 percent of children under five years of age are still affected. Other forms of malnutrition are also growing: adult obesity continues to increase in countries irrespective of their income levels, and many countries are coping with multiple forms of malnutrition at the same time – overweight and obesity, as well as anaemia in women, and child stunting and wasting.

Book excerpt: This year’s report presents evidence that the absolute number of people who suffer from hunger continues to slowly increase. The report also highlights that food insecurity is more than just hunger. For the first time, the report provides evidence that many people in the world, even if not hungry, experience moderate food insecurity as they face uncertainties about their ability to obtain food and are forced to compromise on the quality and/or quantity of the food they consume. This phenomenon is observed globally, not only in low- and middle-income countries but also in high income countries. The report also shows that the world is not on track to meet global nutrition targets, including those on low birthweight and on reducing stunting among children under five years. Moreover, overweight and obesity continue to increase in all regions, particularly among school-age children and adults. The report stresses that no region is exempt from the epidemic of overweight and obesity, underscoring the necessity of multifaceted, multisectoral approaches to halt and reverse these worrying trends. In light of the fragile state of the world economy, the report presents new evidence confirming that hunger has been on the rise for many countries where the economy has slowed down or contracted. Unpacking the links between economic slowdowns and downturns and food insecurity and malnutrition, the report contends that the effects of the former on the latter can only be offset by addressing the root causes of hunger and malnutrition: poverty, inequality and marginalization.

Book excerpt: Globalization of the food supply has created conditions favorable for the emergence, reemergence, and spread of food-borne pathogens-compounding the challenge of anticipating, detecting, and effectively responding to food-borne threats to health. In the United States, food-borne agents affect 1 out of 6 individuals and cause approximately 48 million illnesses, 128,000 hospitalizations, and 3,000 deaths each year. This figure likely represents just the tip of the iceberg, because it fails to account for the broad array of food-borne illnesses or for their wide-ranging repercussions for consumers, government, and the food industry-both domestically and internationally. A One Health approach to food safety may hold the promise of harnessing and integrating the expertise and resources from across the spectrum of multiple health domains including the human and veterinary medical and plant pathology communities with those of the wildlife and aquatic health and ecology communities. The IOM's Forum on Microbial Threats hosted a public workshop on December 13 and 14, 2011 that examined issues critical to the protection of the nation's food supply. The workshop explored existing knowledge and unanswered questions on the nature and extent of food-borne threats to health. Participants discussed the globalization of the U.S. food supply and the burden of illness associated with foodborne threats to health; considered the spectrum of food-borne threats as well as illustrative case studies; reviewed existing research, policies, and practices to prevent and mitigate foodborne threats; and, identified opportunities to reduce future threats to the nation's food supply through the use of a "One Health" approach to food safety.
