Book excerpt: This thesis examines how advocates and funders in the U.S. global health and international development community conceptualize vaginal microbicides, or products such as gels, films, rings and sponges that women could apply vaginally for HIV prevention. An interdisciplinary approach was employed, combining anthropological and feminist theoretical frameworks and research methods, such as semistructured interviews, participant observation and a focus group discussion. This thesis finds that U.S. microbicide advocates and supporters see these products as a tool to maintain prevailing gender identities that assign men sexual decision-making authority and prestige. At the same time, U.S. microbicide advocates resist gender identities that define men as powerful and women as passive. In these situations, they view vaginal microbicides as a tool that women can use to wrestle sexual decision-making powers from men. From science and technology studies, this thesis employs the concept of non-human agency to argue that microbicide advocates assign to these products a noteworthy amount of power and capacity. Also from science and technology studies, the theory of situated knowledge helps analyze claims made by microbicide advocates, particularly the political implications of claiming to speak from the position of African women. Politics and power are also central themes when examining how vaginal microbicides are entangled with broader U.S. health and development objectives in Africa, and this thesis argues that the rhetoric circulating in the U.S. microbicide advocacy community echoes historical paradigms about health and sexuality in Africa. This thesis also demonstrates that vaginal microbicides do a significant amount of political work, as U.S. microbicide advocates and supporters endow these vaginal products with a distinct level of power to achieve broader U.S. international development goals, such as improving gender relations and empowering women in Africa.

Clinical trials on HIV and AIDS prevention face challenges associated with infection of participants with HIV and sexually transmitted infections. Participants also experience adverse events associated with voluntary medical male circumcision and the use of microbicides and vaccines in clinical trials. The results obtained from clinical trials investigating the efficacy of microbicides and vaccines in preventing HIV infection are disappointing, but there is hope that one day a breakthrough could be made. Clinical trials on voluntary medical male circumcision and preexposure prophylaxis have shown positive results that warranted approval by the US Food and Drug Administration (FDA) and the World Health Organization (WHO). The major challenge experienced in new HIV prevention clinical trials that are underway and in approved interventions that are being scaled up apparently relates to HIV incidence that threatens the social value and scientific validity of the interventions. Paradoxically, the clinical trials and scale-up interventions have become the new sources of HIV infection although the hypothesized benefit to be realized in the end is conceptualized as outweighing the current risks involved.

Book excerpt: With women comprising nearly 50% of HIV/AIDS infections globally, the development of new woman-initiated HIV prevention methods has become a public health imperative. To date, the female condom and the diaphragm are the only woman-initiated prevention methods available on the consumer market. Recent research has focused on two HIV/AIDS prevention technologies undergoing clinical trials: microbicides and tenofovir (PrEP). Both are being touted as female-initiated HIV/AIDS prevention technologies that women can use covertly, without a partner's knowledge, to improve their odds in protecting themselves against HIV. Use of these new technologies will not only depend on their effectiveness, but on women's willingness to use these products. This cross-sectional study used an integrative conceptual model that included both intrapersonal and interpersonal constructs to explore factors associated with women's intentions to use. Three hundred and forty-eight high-risk women in Toronto, Canada completed self-administered questionnaires (SAQs). Results were remarkably consistent across relationship types and prevention methods examined in the study, with 60% of the women intending to use microbicides and tenofovir. Women who perceived themselves to be at greater risk for STIs, had greater microbicide self-efficacy, and more positive perceptions of microbicides were more likely to intend to use microbicides.
