Book excerpt: Dentro de la problemática multifactorial de los recursos hídricos, el acceso al agua de calidad es uno de los principales desafíos para las naciones, sin importar su nivel de desarrollo económico. La Organización Mundial de la Salud reconoce que el acceso al agua en cantidad y calidad adecuadas es esencial para la salud, es un derecho humano y componente de una política efectiva de protección a la salud (OMS, 2006). El presente estudio tiene como objetivo determinar aquellas directrices que guíen hacia el aseguramiento de la calidad del agua destinada al consumo humano y con ellas analizar los sistemas encargados de satisfacer una de las necesidades básicas de la población. Bajo este enfoque, se presenta una metodología que permite evaluar el aseguramiento de la calidad del agua en sistemas de abastecimiento de agua potable de una manera integral. Esta metodología se basa en el análisis individual de los componentes del sistema de abastecimiento de agua potable: cuenca de captación, obra de toma, planta potabilizadora, tanque de regulación y red de distribución. A excepción de la planta potabilizadora, se evalúa cada uno de los elementos con el fin de identificar las áreas de acción críticas. La planta potabilizadora fue evaluada mediante una metodología específica (Cázares et. al., 2009) y no forma parte de los objetivos de esta tesis. La cuenca de captación se estudia evaluando la vulnerabilidad del agua en el punto de aprovechamiento a ser contaminada por las actividades que se realizan dentro de la cuenca; este enfoque identifica las principales fuentes de contaminación y fundamenta un programa de monitoreo de calidad del agua. Tanto la obra de toma como los tanques de regulación y la red de distribución, se evalúan mediante un diagnóstico del estado actual, bajo los lineamientos internacionales de aseguramiento de la calidad del agua, y mediante el monitoreo de indicadores de desempeño, con el fin de evaluar el cumplimiento de las funciones específicas de los elementos.

Book excerpt: Se exponen los resultados de un estudio sobre la calidad del agua del río Burgay en la provincia del Cañar empleando un índice formulado por el Instituto Mexicano de Tecnología del Agua, con 18 parámetros físico, químicos y microbiológicos convencionales; por constituir este río una fuente de abastecimiento para algunas poblaciones, el estudio incluyó la búsqueda de plaguicidas de uso agrícola que modifican considerablemente la calidad del agua y limitan sus usos posteriores, sobre todo el destinado a consumo humano. El estudio evidencia que los índices convencionales miden un riesgo sanitario a corto plazo, puesto que se investiga la presencia de organismos patógenos; en cambio la presencia de plaguicidas, cuando no se aplican sistemas de tratamiento específicos para su remoción, representan un riesgo químico a largo plazo, debido al consumo continuo de pequeñas dosis de estas sustancias con propiedades acumulativas y con efecto tóxico en la salud de la población. El riesgo sanitario a corto plazo normalmente es medido por la entidad encargada de la dotación del servicio, a través de ensayos sistemáticos, periódicos y documentados; pero la toxicidad de la fuente por requerir análisis más complejos y un mayor nivel del capacitación, debe ser parte de los Planes de Vigilancia de la Calidad, y que normalmente los ejecuta una entidad diferente a la prestadora del servicio. Se enfatiza en la responsabilidad de las instituciones competentes para coordinar acciones y llevar a cabo los dos procesos: Control de Calidad Interno y Sistema de Vigilancia Externo, como un mecanismo que permita una evaluación oportuna e integral de la fuente, minimizando el riesgo y garantizando la salud de los consumidores.

Chronic Kidney Disease is also a very common disease, affecting more than 10% of the world population, ranging from stage 1 to stage 5 before dialysis. Approximately 1% of the population in industrialized countries is affected by end-stage renal disease (ESRD), needing a renal replacement therapy either hemodialysis or peritoneal dialysis, and ultimately by renal transplantation. Those CKD patients are more susceptible to exhibit reduced vitamin D stocks. Consequently, more than eighty percent of CKD patients have either insufficient or deficient vitamin D levels for multiple reasons.
