Book excerpt: As a result of the current legislative changes there will be a projected shortage of about 45,000 primary care physicians by 2020 to treat the newly insured who have chronic medical conditions, especially those living in rural and urban areas who receive their medical services in community health centers. Nurse practitioners (NPs) will necessarily become and indispensible component of the national health care system to meet the needs to provide medical care for the chronically ill. Current literature confirms the growing need for experienced NPs to mentor students and inexperienced NPs to achieve excellence in practice and ensure achievement of career goals. Despite this, NP mentors are a rare commodity. During the process of becoming and advanced practice nurse, every nurse practitioner (NP) has been mentored by an experienced NP; it is not possible to become an independent NP without the support and guidance of experienced mentors. An integral part of a practice-based discipline such as advanced practice nursing requires a commitment from NPs who have previously forged the path to independence. Over the last few years, the available numbers of NP mentors have declined, resulting in limited access to health care for patients and overuse of emergency departments (EDs) for primary care services. This Capstone Project explores the financial and social impact a Community Health Center (CHC) endured in 2007 resulting from a lack of NP mentors for new hire NPs. A mentoring program was developed by the senior NP with input from colleagues at the CHC following Cohen's framework for an effective mentoring program.

At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. Available in PDF, EPUB and Kindle. Book excerpt: In 2009, the American Academy of Nurse Practitioners report identified more than 125, 000 Nurse Practitioners (NP) working in the United States. As the number of NPs increases and their role expands, questions about their work satisfaction and ability to learn and perform the role arise. Since NPs have to combine a nursing philosophy with a medical care model, it is often difficult to mediate the two approaches and fully understand the role. Mentoring has been successfully used to facilitate role transition and role understanding for nurses and physicians, but has been rarely studied in NPs. Although widely used in nursing and medicine, the concept of mentoring is poorly defined and often used interchangeably with coaching and precepting, thus making program design and evaluation difficult. The purpose of this study was to develop a theory of mentoring for NPs in a hospital setting. A total of 18 participants (eight mentors and eight mentees) were interviewed to learn about their experience with mentoring. A three component mentoring process emerged from the data: forming the relationship, developing the relationship, and outcomes. While some of the themes identified in this study have been previously discussed, others like journey, career path, gain a friend, and defining self have never been associated with mentoring. Nurse practitioners participating in this study were continuously defining both personally and professionally in an effort to improve themselves. The journey to a successful mentoring relationship required hard work, commitment, and trust. When mentoring participants took the time to know each other and were able to build trust, the relationship moved beyond the traditional framework of formal mentoring into informal mentoring and friendship.

Mentoring of Novice Nurse Practitioners eases role transition and improves job satisfaction by serving as a bridge from academics to practice. For a mentorship to be successful, mentees must possess the knowledge and self-efficacy skills needed to be active partners in the mentor/mentee relationship. Problem: An Adult Emergency Department (AED) in a large teaching hospital that did not have a mentorship program, experienced an increase in turnover of nurse practitioners who were in the first two-years of practice attributed to role transition difficulties. Purpose: To determine if a Mentee Workshop for novice nurse practitioners using Bandura's Self-Efficacy Theory leads to acquisition of knowledge and self-efficacy skills for the mentee to successful in a mentorship and increase staff retention among this group. Methods: This quality improvement project used Plan-Do-Check-Act to guide the development of a Mentee Workshop for Novice Nurse Practitioners. Literature review was performed regarding mentoring programs and mentor/mentee training. The General Self-Efficacy Scale and Turnover Intention Scale was used pre- and post-implementation and analyzed through mode, mean, and range of scores with statistical consultation. Results/Implications: There is a gap in knowledge regarding nurse practitioner mentee training programs. The available literature considers mentoring to be a gold standard of nursing practice and a means to increase retention and self-efficacy. An evidence-based Mentee Workshop may be effective to increase retention and self-efficacy in Novice Nurse Practitioners.
