In an article on the specificity of acute articular rheumatism, Grenet reports having observed outbreaks of five distinct epidemics among soldiers within a limited area. The disease occurred within a zone of trenches occupied by certain regiments and also appeared in their rest camps, where they came in contact with second line troops who became infected in their turn. The immunity of a regiment after occupying the former cantonment of another regiment in which cases had occurred tended to show that contamination was not caused by the soil or by the quarters, but by direct contact. Further evidence of the contagious character of acute rheumatism is afforded by Grenet in the history of the following case. A man who had hitherto escaped rheumatism, but belonged to a regiment in which an outbreak had occurred, was suddenly separated from it and admitted to a hospital for a wound. Twenty-six days after admission he had a definite attack of acute rheumatism, and six days later two wounded soldiers occupying adjacent beds had similar attacks. While admitting that the contagiousness of acute rheumatism is of slight degree, so that it usually escapes notice, Grenet points out that it is an argument of the highest value in favour of the specificity of the disease. Frauenthal declares that teeth and gums as sources of infection in rheumatism are overestimated, and that much harm is done by the promiscuous extraction of teeth owing to the incompetent interpretation of X-rays. Any acute or chronic infection of the gastro-intestinal canal, genito-urinary tract, or respiratory system may give rise to an acute or chronic inflammation of the joints. According to Lambert, the percentage of acute rheumatism among the total admissions to the Bellevue Hospital, New York, has steadily diminished from 1907, when it was 2.45, to 1919, when it was 0.521. He regards dental hygiene more than any other one factor as responsible for this diminution, and thinks it probable that tonsillectomy is also in part a factor. Symptomatology. In a series of 73 cases of rheumatic fever in soldiers, studied by White at U.S. Base Hospital No. 6, 51 per cent, gave a previous history of rheumatic fever, and 40 per cent, showed large or ragged tonsils. The joints involved in order of frequency were knee, ankle, shoulder, wrist, foot, elbow, hand, and hip. Acute pericarditis, which was usually very transient, was found in 10 per cent., and acute pleurisy in 19 per cent. There was evidence of mitral endocarditis in 40 per cent., and of combined mitral and aortic endocarditis in 8 per cent. Most of these cases were apparently of long standing. Combined with the emigration of white blood-corpuscles, it was found that the borders of the cells forming the vessels are not so distinct as in a normal state. Between them are a greater number of dark spots (stigmata) than in a normal state, generally not so large that red blood-corpuscles could pass them. Dr. Arnold observed that granules of Cinnabar, as well as colloid substances, may leave the vessels through the stigmata. The cause of the easier penetrability of the vessels may be found in an alteration of the condition of the cement connecting their cells. A great number of the emigrated white corpuscles are carried off by the lymph-vessels. The author thinks that with the disturbance in circulation during emigration there are connected currents directed towards the walls of the vessels, and that they are of different strength - London Med. In subacute cases the characteristic color is rarely present; but here we find depositions of broken-down red blood corpuscles in the bone marrow, spleen, and liver; the latter two organs are enlarged. The kidneys, too, are usually swollen, the tubules are filled up with the debris of the red blood cells, the epithelia are in a state of cloudy swelling, which is, however, according to Marchand, of a second ary nature and due only to the pressure of the plugs. Thrombi were not found in any part of the body. The mucous membrane of the gastro-intestinal canal is often found to be swollen and ecchymotic. Unmarked by startling discoveries, undisturbed by the fulmination of new doctrines seeking to overthrow the very basis of the Science, it has been remarkable for steady progress and advance, proceeding step by step, ever excelsior, guided by the light of reason and certified by the test-stone of experience, "proving all things, and holding fast to that which is good." Purposing to take a retrospective glance over the past year's contribution to our knowledge of Medical Science in its various branches, wo do not forget that the subject is a long one (ars longa, vita brevis) and the space at our disposal necessarily restricted; we shall therefore enter on our account without further prelude or observation. Anatomy And Physiology. In the domain of Anatomy and Physiology advances have been made, amongst which we note: 1st. In connexion with the nervous system - the series of very laborious investigations and interesting deductions of Messrs. Charcot, Pitres, and Bourdon, of Paris, and Dr. Broadbent of London, on the subject of cerebral localizations, too long to quote. Prof. Langen-dorf has communicated to the new series of Dubois Keymond's Archiv de Physiol, an interesting account of his investigations on the subject of Inhibitory Nerves, the existence of which has been known since the observations of the brothers Weber on the Par-vagum. He establishes that the reflex inhibitory influence of each half of the brain extends over the opposite side of the body, and that its seat is not only in the lobi optici, but also in the cerebral hemispheres. Dr. Tyson of Philadelphia records (Amer. Jour, of Med. Sciences, July, 1877) a curious case occurring in his practice, the observation of which fully confirmed the view that the cerebellum is a centre of co-ordination of muscular movement; and another case recorded in Virchow's Archives, by Dr. Kohts of Strasbourg, goes to show that the corpora quadrige-mina must be added. Flourens' assertion that "the vestibulary nerve is a completely distinct pair from the auditory nerve or the nervus cochle " has been confirmed by the results of Horbaczewski's investigations on the nerves of the internal ear. M. Luys records (Soc. de Biol.) a case of compensatory hypertrophy of the right hemisphere found after death in a case of right hemiplegia. During the year M. Duret presented to the Societe de Biologie drawings and injections exhibiting the very great correlation which exists between the distribution of the arteries and the physiological regions of the brain, and shewing, amongst other things, that a special artery is given off to the third convolution. The last point in connexion with the cerebral nervous system which we have to mention is the investigation undertaken by Duke Charles, of Bavaria, M.D., to refute the assertion of Popoff that white blood corpuscles were found in the brain as the result of typhus and typhoid fevers. He examined fifty brains, and the result was "that in the cortical portion of every brain, healthy or otherwise, are found numerous white blood corpuscles." These may accumulate on account of retarded circulation combined with an increased quantity of water in the brain.
