Half of melanomas have an activating mutation in the BRAF gene, prompting development of novel drugs, vemurafenib and dabrafenib, specifically targeting mutated BRAF. Trametinib and cobimetinib, which block MEK, a BRAF effector protein, have been used in combination with BRAF inhibitors. A promising new melanoma treatment is immunotherapy, approach that boosts patient's own immune system to attack cancer. Pembrolizumab and nivolumab inhibit PD-1, whereas Ipilimumab targets CTLA-4, another immunity check point, to boost the immune response. Here we focus on pathways, mechanisms, targets and treatments of human skin cancers, with particular emphasis on the new developments in the research on melanomas.

Book excerpt: This book presents the morphology, possible mechanisms of formation, and clinical relevance of tumor deposits (TDs) in different organs and parts of the body. It also describes the diagnostic features that are currently available for TDs and discusses the prognostic impact of TDs. The poor prognostic and predictive values of TDs were found to be as strong as distant metastasis in different series. TDs, best characterized by colorectal cancer, are irregular discrete tumor masses that are discontinuous from the leading edge of the tumor. However, even in colorectal cancers, the place of TDs in tumor staging (tumor, lymph node and distant metastasis categories) remains controversial. This book presents the possible mechanisms of TD formation, including neurotrophic and angiotrophic extravascular migratory metastasis, in comparison with the well-known mechanisms of metastasis. It also shows that TDs were identified in esophageal, gastric, salivary gland, head and neck, and breast carcinomas. The possible mechanisms leading to the frequent association of TDs and peritoneal carcinomatosis are discussed. This is a valuable resource for pathologists, oncologists, and surgeons who are looking for detailed information on the latest scientific insights into the pathology and prognostic implications of TDs.

Yet, in spite of the pioneering work and outstanding contributions of investigators such as D. Coman, H. Green, B. Fisher, S. Wood and I. Zeidman, studies on metastasis rarely achieved the popularity afforded to more esoteric areas of tumor biology. Tumor dissemination, occurring as it does in a responding host and being composed of a series of dynamic int~ractions, is a highly complex phenomenon. Few investigators were brave enough to attempt to unravel the mechanisms involved. Paradoxically, this very complexity may have contributed, in part, to the recent upsurge of interest in metastasis research. More and more researchers are becoming fascinated by the complexities of the cellular interactions involved in tumor spread. Accompanying this intellectual stimulation have been technological advances in related fields which allow the derivation of new model systems. The mechanisms of metastatic spread are increasingly amenable to both the reductionist and holistic approaches and it is the purpose of this volume to present many of these model systems while emphasizing the intricacy and complexity of the processes they mimic. We have attempted to emphasize two topics not previously covered in depth in previous books on metastases. These are in vitro models of invasion and in teractions of tumor cells with connective tissue.
