At the same time, there is evidence that the incentives for improved health care organization efficiency are minimal due to the revenue or costs reimbursement systems, and there is also evidence that inefficiencies exist among a sizeable percentage of these institutions and particularly hospitals. Other techniques used to evaluate efficiency of heatlh care organizations including ratio analysis and regression analysis ahve been found to have serious limitation [14] so that.dea represents a new and possibly superior alternative which we con sider in this study. Book excerpt: In this paper we show how both the choice of specific constraints on input and output weights (in accordance with health care policy-makers' preferences) and the consideration of exogenous variables outside the control of hospital management (and linked to past policy-makers' decisions) can affect the measurement of hospital technical efficiency using the Data Envelopment Analysis (DEA). Considering these issues, the DEA method is applied to measure the efficiency of 85 (public and private) hospitals in Veneto, a Northern region of Italy. The empirical analysis allows us to verify the role of weight restrictions and of demand in measuring the efficiency of hospitals operating within a National Health Service (NHS). We find that the imposition of a lower bound on the virtual weight of acute care discharges weighted by case-mix (in order to consider policy-maker objectives) reduces average hospital efficiency. Moreover, we show that, in many cases, low efficiency scores are attributable to external factors, which are not fully controlled by the hospital management; especially for public hospitals low total efficiency scores can be mainly explained by past policy-makers' decisions on the size of the hospitals or their role within the regional health care service. Finally, non-profit private hospitals exhibit a higher total inefficiency while both non-profit and for-profit hospitals are characterised by higher levels of scale inefficiency than public ones.

Data Envelopment Analysis (DEA) is utilised to estimate a best practice frontier and to evaluate the performance characteristics of different hospitals in the two countries. The measurement categories of input factors are represented by the hospital size (amount of beds), number of wards, number of employed medical specialists, sum of total inpatient admissions, demographic environment (population density in population/km2), and one output factor (mortality rates). Regarding these factors, the main findings reveal that the allocation of inefficient hospitals in England is broadly spread and, therefore, the mean efficiency value of hospitals in England is ≈44% (whereas 100% describes efficient units). In contrast, the mean efficiency value of German hospitals equates to ≈91%. Consequently, the allocation of inefficient German hospitals is less spread and the overall performance of hospital health supply is more efficient in Germany compared to England. Most remarkably, the amount of inpatient admissions is one of the main drivers for efficiency and especially the English hospitals suffer from high numbers of inpatient admissions, which may be reduced due to improvements in the primary healthcare supply provided by general practitioners.

Book excerpt: Data Envelopment Analysis (DEA) is a methodology particularly well-suited to measuring the efficiency of hospitals because it is able to accommodate multiple heterogeneous inputs and outputs in order to model the complex relationships that exist within them. This thesis uses DEA to develop two hospital efficiency models in collaboration with the Canadian Institute for Health Information (CIHI). The first model measures the clinical efficiency of Canadian acute care hospitals in the treatment of patients with acute myocardial infarction (AMI). The model uses inputs measuring the intensity and duration of clinical resource utilization and includes survival rate as the key output. The second model measures the overall production efficiency of acute care hospitals and uses labour and capital inputs together with outputs measuring inpatient and outpatient activity. Both models include non-discretionary variables adjusting for case mix variations among the hospitals.
