Policlinico San Matteo (Pavia, Italy) and the Giovanni Lorenzini Medical Foundation (Milan-Houston). It drew papers from six continents of the world, was attended by over 500 investigators, and demonstrated the extraordinary vitality, depth and breadth of research which characterizes modem neuro-oncology. Over the course of the last decade, there has been a remarkable shift in research carried out in the heterogeneous field of neuro-oncology, which appears to be away from clinical descriptive studies, and toward more basic and fundamental investigation of the pathology, immunohis tochemistry, biochemical and cellular subsets of brain tumors. Besides the traditional fields of neurology, neurosurgery, neuropathology, and radiation therapy, there has been an increased interest and involvement by investigators in the fields of medical oncology, neuroradiology, immunology, and many areas of fundamental neurobiology. It has become evident that interest has also been exhibited in a broader spectrum of tumors than just the malignant glial series, and studies in meningioma, craniopharyngioma, neurinomas, and the pituitary tumors were reported. Several sessions were devoted to the special problems of pediatric brain tumors.

Book excerpt: Twenty years ago when Children's Cancer Association of Japan was born, the diagnosis of childhood leukemia amounted to a death sentence. Only 20% or so of children with leukemia survived more than 5 years. Since then, enormous improvements have been achieved regarding our understanding on the etiology, diagnosis, and the treatment of childhood leukemia. Now, 70% of children with leukemia survive and enter adult life. Even though the improved survival rate of children with leukemia represents a medical success story, we now face new problems. The first problem is the fact that we still lose 20-30% of patients with childhood leukemia. To address this problem, we need to understand the etiology, epidemiology, and biology of leukemia; to identify the patients at greater risk; and to develop adequate treatments. The second problem is the treatment itself. Even though efficacious, the modem treatment for leukemia is a grueling experience for children and their families. We should develop a total care system for families and children based on a deep understanding of their needs. The third problem is the aftereffects of the treatment and of cured leukemia. Extensive radiation and chemotherapy have an entirely different spectrum of long-term effects on children than on adults. These treatments in the early stage of life, when the mind and body are developing, create many physical and psychological problems. These are the present problems of childhood leukemia.

The main purpose of the Conference on Brain Tumor Research and Therapy is the candid and informed discussion of the most up-to-date developments in basic re search and clinical treatment of brain tumors. The 3rd Japanese Conference on Brain Tumor Research and Therapy was held at Nasu (Tochigi Prefecture), Japan, in No vember 1994. It was a great honor to welcome many distinguished guests from over seas who kindly attended each session and made valuable contributions.

Drug development in both Japan and the United States has evolved over the years to an increasing emphasis on second generation compounds. This has come about as a result of the initial successes of chemotherapy development. Many active structures have been uncovered and the armamentarium of the medical and pediatric oncologist has grown dramatically. The uncovering of an active structure provides an opportunity for analog synthesis and attempts at elucidating structure-activity relationships. It is hoped that the therapeutic index of active structures can be improved so as to achieve superior clinical results.
