Book excerpt: Purpose: Borderline personality disorder (BPD) is a highly prevalent, debilitating, and deadly disorder that is difficult to diagnose. As many as 1 in 10 individuals with BPD successfully commit suicide, thus identifying those with BPD is critically important. In the present study, we assess the predictive validity of the McLean Screening Instrument for Borderline Personality Disorder (MSI-BPD, Zanarini et al., 2003) for detecting BPD. Four indices were examined: positive predictive power (PPP), negative predictive power (NPP), specificity (SPE), and sensitivity (SEN). Methods: Two-hundred and thirty-nine individuals ages 18 to 60 (M= 27.5; S.D. = 10.07)) completed the MSI-BPD and the International Personality Disorder Examination (IPDE; Loranger, 1994). Results: Sixty-three participants (26.4%) met criteria for BPD on the IPDE. The kappa value for an MSI-BPD threshold of 5, 6, or 7 indicated moderate levels of agreement (Fleiss, 1981). The MSI-BPD showed excellent sensitivity with cut-off scores of 5 or 6 and weaker sensitivity with cut-off scores of 7 or 8. In contrast, its specificity increased as the cut-off increased. The positive predictive value ranged from 0.48 (cutoff of 5) to 0.56 (cut-off of 7). However, the negative predictive power was high for all cut-offs. Conclusion: The MSI-BPD can be an effective screening tool for detecting BPD. Our findings are generally consistent with the initial validation study for which a cutoff of 7 was recommended. However, we recommend a cut-off score of 6 as useful in a two-stage process to identify those likely to have BPD. When caseness is particularly important, a cut-off of seven may be preferred.

Book excerpt: Borderline personality disorder (BPD) has been widely viewed as a chronic disorder, which has led many clinicians to avoid treating patients with this diagnosis. BPD is also one of the most stigmatized of psychiatric disorders, due to the awkward manner in which these individuals attempt to get their needs met. As such those with BPD are increasingly marginalized by society and prevented from accessing quality care. In the Fullness of Time debunks the common myth that BPD is incurable, drawing on the findings of the NIMH-funded study, the McLean Study of Adult Development, which has found that BPD has the best symptomatic outcome of all major psychiatric illnesses. Citing and analyzing the results of this landmark, decades-long study, Mary Zanarini explains why there is reason for optimism when it comes to BPD: remissions lasting two to eight years are common and stable; furthermore, remission of all 24 symptoms of the disorder are also quite typical. Equally promisingly, the acute and most life-threatening symptoms of BPD, such as self-harm and suicide attempts, remit rapidly, and recur less frequently than do temperamental symptoms. Zanarini also reports on more sobering findings concerning high levels of poor outcomes relating to vocational impairment and physical health, reported by the 40% of patients who have not recovered, which have significant impact on wellbeing and use of medical and other services. Considered together, the findings generated by this important research provide much-needed hope for those diagnosed with BPD, particularly in guiding future research on and treatment for borderline personality disorder.

Book excerpt: Objective: Borderline personality disorder (BPD) is serious and prevalent and is quite heterogeneous. Identifying subtypes of BPD promises useful diagnostic and treatment implications. Although a series of subtyping studies exist, only two have examined BPD subtypes while taking into account BPD severity. We utilize factor mixture modeling (FMM) to identify discrete BPD subtypes, simultaneously considering symptom severity, in a large nonclinical young adult sample. We also consider how identified subtypes may be reflected in individuals reliably diagnosed with BPD. Method: Undergraduate students (N = 20,010; 63.86% female; mean age=18.75, SD = 1.73) and BPD-diagnosed participants (N = 66; 97% female; mean age = 29.74, SD = 10.94) completed a dimensional version of the McLean Screening Instrument for BPD (MSI-BPD). This was condensed to measure the nine DSM BPD criteria on a True/False scale. We conducted FMM to determine classes of individuals characterized by different responses to the MSI-BPD as well as the composition of underlying BPD severity dimensions. Results: The nonclinical sample was comprised of three subtypesAsymptomatic (70%), Impulsive/Externalizing (19%), and Identity Disturbed/Internalizing (11%)falling along a single continuum of increasing BPD severity. In the BPD sample, a single severity dimension best captured BPD symptomatology and no subtypes were identifiable. Conclusions: Our results suggest the importance of both dimensional and categorical conceptualizations of BPD, depending on the sample and level of severity in focus. Impulsive/Externalizing and Identity Disturbed/Internalizing classes suggest different treatment targets for subthreshold BPD and potential etiologically relevant profiles for BPD development. The findings are discussed in terms of their clinical implications regarding diagnosis, treatment, and theoretical conceptualization of BPD.
