Book excerpt: This study investigated the relationship between motor unit (MU) properties and the strength and power of two lower limb muscles in healthy young and old adults. Twelve older adults (mean age, 77 ± 5 yrs) and twelve young adults (mean age, 24 ± 3 yrs) were studied. MU properties of the tibialis anterior (TA) and vastus medialis (VM) muscles were determined using decomposition-enhanced spike-triggered averaging (DE-STA). Motor unit number estimates (MUNE) of the TA were significantly reduced (p>0.05) in older adults (102 ± 76) compared to young adults (234 ± 109), primarily as a result of significantly larger surface-detected motor unit potentials (S-MUP) in older adults. Although VM S-MUP values were larger in older adults (60 ± 31 V) compared to young (48 ±42), the difference was not significant. Maximal strength and power were significantly larger in both the TA and knee extensors of young adults compared to old. Maximal power output displayed greater deficits than isometric strength in both lower limb muscles of older adults. Results from this study indicate that there are changes in MU properties with age, and that this effect may be greater in the TA muscle. Further, power, especially in the knee extensors, may be a more sensitive measure of neuromuscular health than isometric strength, and should be the focus of exercise programs in elderly subjects.

Book excerpt: Age-associated biomechanical plasticity (BP) has been established as the distal to proximal shift of joint mechanical output in old adults while walking. The cause of BP is still unknown, but changes in muscle strength of the lower extremities due to age are thought to be one of the underlying causes of BP. Old adults who had overall weaker lower extremities have been shown to have increased BP while walking on level and incline surfaces, however individual muscle groups have not yet been evaluated. We hypothesize that one causal factor of BP with age is that hip extensor muscles are more similar in strength in young and old adults than are ankle plantarflexor muscles, thus enabling old adults to walk with larger mechanical contributions from hip muscles as compensation for reduced contributions from ankle muscles. The purposes of the study were 1) compare muscle strengths of hip extensors and ankle plantarflexors between young and old adults, 2) verify BP in old adults by comparing hip and ankle joint torques and powers between age groups in level and incline walking & 3) examine the relationship between the relative strength in hip vs ankle muscles and the magnitude of BP in old adults during these gaits. 14 young (20yrs) and 22 old (76yrs) adults performed maximal isometric and isokinetic standing hip extension (20° of hip flexion) and seated ankle plantarflexion (15° of dorsiflexion). Resistance training guidelines for older adults are typically based on free-weight and machine exercises, which may be inaccessible and lack carryover to activities of daily living. The present study tested the hypothesis that resistance training adaptations are task specific in older adults. Thirty older adults (8 males, 22 females; mean age = 71 years) were randomly assigned to participate in 6 weeks of supervised, high-intensity resistance training (twice per week) utilizing free-weight and machine exercises (traditional) versus functional activities that were overloaded with a weighted vest (functional). Participants were thoroughly familiarized with the exercises and testing prior to beginning the study. Major outcome measures included assessments of functional performance, five-repetition maximum strength, isometric knee extensor force, and quadriceps muscle mass and muscle quality. Physical activity and nutrition were monitored. The study results demonstrate that the magnitude of improvement within a given outcome was largely dependent on group assignment, with greater improvements in gait speed and the timed-up-and-go in the functional group, but 2-3x greater five-repetition maximum strength improvements for the trap bar deadlift, leg press, and leg extension exercises following traditional resistance training. Both groups showed improvements in isometric knee extensor force and muscle size, suggesting that some aspects of the observed adaptations were generic, rather than specific. Importantly, accelerometer data revealed an increase of time spent in moderate-to-vigorous activities outside of the laboratory. Overall, these novel findings suggest that, among older adults, 1) resistance training adaptations exhibit a high degree of task specificity and 2) significant improvements in functional outcomes can be achieved with the simple use of a weighted vest.
