Thyroid Systems Engineering targets an optimal treatment of people suffering from thyroid hormone disorders. The content is motivated by in-depth observations of such patients whose rich data supported the theoretical framework arising from formal mathematical reasoning, guided by the nature of thyroid physiology. Leveraging on the insights emerging from the unique combination of an electrical engineer working with a clinical thyroidologist, and both being scientists skilled in mathematics, the authors introduce this new discipline and field of scientific investigation aptly designated as Thyroid Systems Engineering. Readers will discover that mathematics can indeed model the behavior of the hypothalamus-pituitary-thyroid (HPT) axis. Focused on modeling, each of the eighteen chapters gives the reader a notion of the application of relevant mathematics to pertinent issues encountered in mainstream thyroidology. Many cellular processes resemble the flux of variables and states in a complex multi-parameter space through time analogous to current flow in electrical networks. It is then logical to apply the principles and physical laws of electrodynamics, electrical network theory, control systems theory and signal theory to many of the biological phenomena encountered in endocrinology. Such an approach is used liberally throughout the book and successfully yields elegant solutions to a number of models presented within. This book can serve as a reference to mathematical modeling in other aspects of endocrine physiology, and as the starting point for a fundamental course in medical modeling. It will appeal to postgraduates in electrical engineering, academic physicians and biomedical researchers. Further, readers equipped with advanced calculus, electrical network theory, control theory and signal theory should be able to follow the mathematical expositions that describe thyrotropic control. They represent a new discipline based on mathematical modeling in physiology applicable to medical diagnostics, measurement and treatment to cooperate in the clinical team and realize an optimized treatment for patients.

Book excerpt: [Truncated abstract] The activity of the reproductive endocrine axis is the result of interactions among many organs and tissues, particularly the hypothalamus, pituitary gland and gonad. However, it depends on more than the communication between anatomical structures because it is also affected by genotype, internal factors (e.g., metabolic inputs) and external factors (e.g., photoperiod, socio-sexual cues, stress, nutrition). This multifactorial complexity makes it difficult to use animal experimentation to investigate the pathways and mechanisms involved. Therefore, in this study, I have turned to mathematical modelling. The general hypothesis was that, by modelling the hormonal feedback loop that links the hypothalamus, pituitary gland and gonad, I would be able to discover the critical control points in this homeostatic system. This would allow me to inform and direct research into the processes that control reproduction, including inputs from environmental factors. My studies began with the development of a model of the negative feedback loop through which testosterone controls the secretion of pulses of gonadotrophin-releasing hormone (GnRH) by the hypothalamus. The model incorporated two critical factors: testosterone concentration and a time delay in the inhibition of the activity of the GnRH 'pulse generator' by testosterone. The general assumptions were: i) there are two positive feedforward processes (GnRH pulses stimulate LH pulses, and, in turn, LH pulses stimulate testosterone secretion); ii) testosterone exerts negative feedback that reduces the frequency of GnRH pulses. The model incorporated a group of equations that represent the GnRH pulse generator, through which the inhibitory effect of testosterone acted to reduce GnRH pulse frequency. Simulations were run with various values for the time delay in feedback and, as model development progressed, the simulations were extended to include combinations of time delays and levels of sensitivity of the GnRH pulse generator to inhibition by testosterone. The output of the simulations showed clearly that a time delay in negative feedback, as well as the concentration of testosterone, can greatly affect the frequency of GnRH pulses and the shape of the GnRH secretory profile. Importantly, the effect of the time delay depends on the sensitivity of the pulse generator to testosterone. In addition, the simulations suggested two additional components that might be involved in the control of the GnRH pulse generator: i) a delay in the rate of adaptation to a change in steroid feedback; and ii) a minimum pulse interval (maximum frequency). These studies iii therefore suggest that the regulation of the activity of the GnRH pulse generator, and thus the frequency and profile of GnRH and LH pulses, requires interactions among these four components. These interactions should be tested in animal experimentation. In the next stage, I extended the model so I could test whether the feedback delay might involve the process of aromatization in which testosterone is converted to oestradiol at brain level. ... This information can be used to direct future experimental studies that will help us to understand the factors that underlie the dynamic behaviour of the hypothalamic and pituitary systems that control reproduction.

Around 1990, however, a major shift began in the way that researchers viewed some nutrients particularly the vitamins. This shift was motivated by the discovery that modest declines in vitamin nutritional status are associated with an increased risk of ill-health and disease (such as neural tube defects, heart disease, and cancer), especially in those populations or individuals who are genetically predisposed. In an effort to expand upon this new understanding of nutrient action, nutritionists are increasingly turning their focus to the mathematical modeling of nutrient kinetic data. The availability of suitably-tagged (isotope) nutrients (such as B-carotene, vitamin A, folate, among others), sensitive analytical methods to trace them in humans (mass spectrometry and accelerator mass spectrometry), and powerful software (capable of solving and manipulating differential equations efficiently and accurately), has allowed researchers to construct mathematical models aimed at characterizing the dynamic and kinetic behavior of key nutrients in vivo in humans at an unparalleled level of detail.

Scientists often perform the processes of modelling subconsciously, unaware of the scope and significance of this activity, and the techniques available to assist in the description and testing of their ideas. Mathematics has three important contributions to make in biological modelling: (1) it provides unambiguous languages for expressing relationships at both qualitative and quantitative levels of observation; (2) it allows effective analysis and prediction of model behaviour, and can thereby organize experimental effort productively; (3) it offers rigorous methods of testing hypotheses by comparing models with experimental data; by providing a means of objectively excluding unsuitable concepts, the development of ideas is given a sound experimental basis. Many modern mathematical techniques can be exploited only with the aid of computers. These machines not only provide increased speed and accuracy in determining the consequences of model assumptions, but also greatly extend the range of problems which can be explored. The impact of computers in the biological sciences has been widespread and revolutionary, and will continue to be so.

A primary focus of neonatology has been to under stand the causes of respiratory distress in the neonate. Success has resulted, not only in the lengthening of gestation, but an improved understanding of the causes and treatment of neonatal respiratory disease. With increasing success has come the necessity to under stand the metabolic principles of the parturient, the fetal/placenta unit, and the neonate. These principles are clearly very important from multiple aspects. Increased understand ing of metabolism of the pregnant woman would explain the aberrations occurring in normal and abnormal pregnancy and improve nutritional support for the parturient. A prime example of altered metabolism is the parturient with diabetes. Understanding metabolism ofthe fetal/placenta unit is necessary to increase the probability that the fetus will be born appropriate for size irrespective of the gestational age. The various compo nents of neonatal metabolism are important, not only for understanding the changes in physiology and biochemistry occurring in the developing neonate, but the principles by which nutritional support should be provided.
