Book excerpt: In Reproductive Labor and Innovation, Jennifer Denbow examines how the push toward technoscientific innovation in contemporary American life often comes at the expense of the care work and reproductive labor that is necessary for society to function. Noting that the gutting of social welfare programs has shifted the burden of solving problems to individuals, Denbow argues that the aggrandizement of innovation and the degradation of reproductive labor are intertwined facets of neoliberalism. She shows that the construction of innovation as a panacea to social ills justifies the accumulation of wealth for corporate innovators and the impoverishment of those feminized and racialized people who do the bulk of reproductive labor. Moreover, even innovative technology aimed at reproduction—such as digital care work platforms and noninvasive prenatal testing—obscure structural injustices and further devalue reproductive labor. By drawing connections between innovation discourse, the rise of neoliberalism, financialized capitalism, and the social and political degradation of reproductive labor, Denbow illustrates what needs to be done to destabilize the overvaluation of innovation and to offer collective support for reproduction.

Book excerpt: An argument that more people should have children with Down syndrome, written from a pro-choice, disability-positive perspective. The rate at which parents choose to terminate a pregnancy when prenatal tests indicate that the fetus has Down syndrome is between 60 and 90 percent. In Choosing Down Syndrome, Chris Kaposy offers a carefully reasoned ethical argument in favor of choosing to have such a child. Arguing from a pro-choice, disability-positive perspective, Kaposy makes the case that there is a common social bias against cognitive disability that influences decisions about prenatal testing and terminating pregnancies, and that more people should resist this bias by having children with Down syndrome. Drawing on accounts by parents of children with Down syndrome, and arguing for their objectivity, Kaposy finds that these parents see themselves and their families as having benefitted from having a child with Down syndrome. To counter those who might characterize these accounts as based on self-deception or expressing adaptive preference, Kaposy cites supporting evidence, including divorce rates and observational studies showing that families including children with Down syndrome typically function well. Himself the father of a child with Down syndrome, Kaposy argues that cognitive disability associated with Down syndrome does not lead to diminished well-being. He argues further that parental expectations are influenced by neoliberal ideologies that unduly focus on the supposed diminished economic potential of a person with Down syndrome. Kaposy does not advocate restricting access to abortion or prenatal testing for Down syndrome, and he does not argue that it is ethically mandatory in all cases to give birth to a child with Down syndrome. People should be free to make important decisions based on their values. Kaposy's argument shows that it may be consistent with their values to welcome a child with Down syndrome into the family.

Drawing on an ethnography of Down’s syndrome screening in two UK clinics, Thomas explores how and why we are so invested in this practice and what effects this has on those involved. Informed by theoretical approaches that privilege the mundane and micro practices, discourses, materials, and rituals of everyday life, Down’s Syndrome Screening and Reproductive Politics describes the banal world of the clinic and, in particular, the professionals contained within it who are responsible for delivering this programme. In so doing, it illustrates how Down’s syndrome screening is ‘downgraded’ and subsequently stabilised as a ‘routine’ part of a pregnancy. Further, the book captures how this routinisation is deepened by a systematic, but subtle, framing of Down’s syndrome as a negative pregnancy outcome. The delivery room is designed around the obstetric bed which was designed for the benefit of the obstetrician rather than the woman giving birth. Despite research showing the benefit of upright positions in labour and birth, most women in the UK still give birth in the semi-reclined position, pushing their baby out against the forces of gravity. The author argues that unnatural positions make labour and birth more painful and difficult for modern women than it was for their ancestors. How did we come to put the needs of care givers above those of the labouring woman? Is there anything that can be done? Starting with a short history of birth furniture, Dynamic Positions in Birth goes on to explore the anatomy and physiology of labour from an evolutionary perspective and explores how rethinking positions for labour and birth could benefit mothers and their babies. Equally important is the need to change attitudes to birth so that women are encouraged to play a more active part in the birth of their babies instead of being subjected to clinical interventions designed to mitigate the adverse effects of labouring in a starkly unnatural environment. Margaret Jowitt argues that it is possible to give women labouring in hospital a better chance of giving birth naturally. The book concludes by calling for a fresh look at the environment for birth. Delivery rooms can be made more user friendly by introducing furniture designed around women’s need for physical support during labour as well as for the birth, and by hiding away the more alarming technology unless it is needed. Women need a less forbidding environment and more encouragement to move freely and adopt positions which will enhance their chance of achieving a normal birth.
