Globally, it is estimated that 289,000 women died from being pregnant in 2013. Ninety-nine percent of all maternal deaths occur in resource-poor nations, averaging 800 deaths each day or 33 per hour. Improvement in maternal mortality was addressed by the United Nations in 1990 by the Millennium Development Goals (MDG's) in which the 5th goal was global reduction of this statistic by three-quarters by the year 2015. However, this goal will not be achieved. For every mother that dies from pregnancy in resource-poor countries, 15 to 30 additional women develop serious damage. This textbook addresses the continuing problem of maternal deaths in developing nations from three perspectives: medical, anthropological, and epidemiological. The twenty-eight internationally-respected authors in this textbook have had direct field experience with maternal health and pregnancy complications in resource-poor regions. They provide up-to-date analysis of maternal deaths in the regions of the world most affected by this public health problem. Book excerpt: Over 529,000 women die annually from complications during pregnancy, childbirth, or the postpartum period. Nearly all of these deaths occur in developing countries, where fertility rates are higher and a woman's life time risk of dying during pregnancy and childbirth is over 400 times higher than in developed countries. Additionally, an estimated 20 million women endure lifelong disabilities such as pelvic pain, incontinence, obstetric fistula, anemia and infertility. The main direct causes of maternal death are severe bleeding, unsafe abortion, infection, eclampsia, and obstructed labor; the indirect causes include anemia, malaria, heart disease, and HIV. Pregnancy complications are the main cause of death for women aged 15-19. High maternal mortality rates in many countries result from poor reproductive health care, including not having access to skilled care during pregnancy and childbirth and access to safe abortion even where it is legal, especially for the poorest women. Risks of poor outcomes during pregnancy and childbirth are exacerbated by poverty, low status of women, lack of education, poor nutrition, heavy workloads and violence. While many factors contribute to maternal death, one of the most effective means of preventing maternal health is to improve health systems and primary health care to ensure availability of skilled attendance at all levels and access to 24-hour emergency obstetric care. Family planning services could also reduce maternal deaths and morbidities by 30 percent. Prevention of unwanted pregnancies and access to safe abortion as allowed by law and post abortion care services could reduce maternal deaths and injuries caused by unsafe abortions - over 68,000 women die from unsafe abortions annually.

Furthermore, the burden of maternal mortality is not evenly distributed. Ninety-nine percent of pregnancy-related deaths occur in developing countries. The discourse of human rights enables us to place and keep an issue such as maternal mortality on the agenda of public concern -- not because of the loss of productivity implied by that magnitude of death but because it is a disgraceful social injustice. As valuable as the rights perspective is, however, it is not sufficient. Without a sound understanding of the epidemiology of maternal mortality (and therefore, the interventions that can prevent it) the concept of a human right to be free from avoidable death during pregnancy and childbirth will remain empty. Without a clear understanding of the causal chain (and how to break it), programs intended to reduce maternal deaths are likely to be ineffective. In the case of maternal mortality, staff of the Center for Population and Family Health (CPFH) at the Columbia University School of Public Health and of the United Nations Children?s Fund (UNICEF) developed a set of indicators for use in monitoring the availability and use of medical services by women with life-threatening obstetric complications, which were formally issued in 1997 by the World Health Organization (WHO), UNICEF, and the United Nations Population Fund (UNFPA) (the UN Guidelines). These indicators go far toward providing the human rights community with performance standards regarding a key aspect of women's right to health. This article describes the scope of maternal mortality and the state of the public health approaches to addressing it, and then discusses the reasoning behind and implications of using the UN Guidelines. The article then adresses the legal context for creating a state obligation to reduce maternal mortality as part of the right to health, which is in turn part of its economic and social rights obligations under specific international treaties. Finally, the article proposes three ways to use the UN Guidelines in setting enforceable standards with respect to states' obligations to take steps to reduce maternal mortality on a non-discriminatory basis: first, adopting objectively measurable and verifiable indicators to judge whether states are taking steps; second, determining whether states are taking the appropriate (i.e., effective) steps; and third, determining whether such steps are being taken on a non-discriminatory basis.
