Excerpt: ...affected, and three or four drops of a solution of persulphate (1 part in 4 of water) were sufficient for cure; the patient was in the upright position, and pressure was carefully applied above and below the seat of puncture (Ranking, ii., 1860). Sentoux, collecting 126 cases, found 100 cured, 19 relieved, 6 unaffected, and only 1 death. Denuce reports many successful cases (Moniteur des Sciences Medicales, November 15, 1862; British and Foreign Review, April, 1862); the perchloride was used with certain precautions. Desgranges thought the method, with ordinary care, to be free from danger--2 drops sufficed for the largest varicose lobule. He notes the improvement in varicose ulcers after the operation (Abstract: British and Foreign Review, ii., 1858). Morgan, of Dublin, succeeded in obliterating part of the saphena vein by injecting 5 drops of Monsel's solution in two places, carefully isolated: coagulum formed in fifteen minutes, and the case did well; in another, with irregular dilatation, and large varicose clusters about the knee, the same method of injection above and below was equally successful (Medical Press, July 14, 1869). Brainard's experiments on the formation of clot in veins under the use of sulphate.were very satisfactory (loc. cit); and Gross, speaking of naevus, and discouraging the use of perchloride, yet says that by " Monsel's solution any case of arterial or venous tumor, unless very bulky, may generally be promptly and effectually cured" ("System of Surgery"); this would seem sufficient to warrant a further trial. Book excerpt: Excerpt from An Ephemeris of Materia Medica, Pharmacy, Vol. 1: Therapeutics and Collateral Information, 1882 and 1883 When such a collective subject has been the business of one lifetime, and becomes the expectancy of two other commencing lives educated with special reference to the subject, it does not seem irrational to hope that information may be given which may be interesting and useful to the medical and pharmaceutical professions, since the subject is the very foundation upon which the utility of these professions to mankind depends. The younger associates in this undertaking may, perhaps, at first do but little of the writing, but they will do much of the work upon which the writing is to be based. To the professions of Medicine and Pharmacy, then, whatever may be here offered is respectfully dedicated by the writer and his two sons. The Strength of Opium and the New Pharmacop ia. When, in 1848, a law was passed by Congress "To prevent the importation of adulterated and spurious drugs and medicines," a careful examination of authorities seems to have been made in order to fix upon a strength below which Opium should not be admitted into the country, and this minimum strength was fixed at 9 per cent. of pure morphia. From that time to the present, this 9 per cent. has been the rule of downward limit for all the United States Custom House examiners, and Opium below that strength has never been lawfully admitted to entry in any Custom House of this country. As the loss of water in drying commercial Opium varies between 17 and 23 per cent., the average loss falls at about 20 per cent., and this is proved to be true by a large experience with the better grades of the drug. Hence Opiums which in the moist chemical condition yield 9 per cent. of pure morphia, when dried and powdered would give 11.25 per cent. pure morphia. The Pharmacop ia of 1850 is silent upon the strength of its Opium; but that of 1860 prescribes that the officinal Opium shall not contain less than 7 per cent. of morphia. This was a curious mistake to make by so high an authority, for if no Opium could lawfully get into the country which contained less than 9 per cent., it had the inferential force of an invitation to unlawful importations. E. 'a' regular classification, under which all the Operations and effects that pertain to a whole class are to be stated, explained and enforced in the form Of a proem to the class; instead of doing it in con nexion with each individual article. Again, the several articles were to be distributed into groups subordinate to the class, and every thing peen liar-to each group was to be said in connexion with the group, and not in connexion with each individual article; while only'the peculiarities of each article, when it happened to have any, were to be mentioned under the individual article. Upon this plan all the individual articles were to have been fully treated Of in their respective classes and of course were to have been mentioned only in one single place, viz: immediately fol lowing the proem to the class into which they were reduced. Under this method, nothing but the peculiarities of the article in comparison with other articles of the same group, would have remained to be said under the individual article itself; and in many instances this would have been scarcely any thing. What medicinal peculiarities, in comparison with each other, that can be specifically described, have Sulphate of Potassa, Sulphate of Soda, Sulphate of Magnesia, Tartrate Of Potassa and Soda, etc? Again, what medicinal peculiarities have a large number of the simple bitter Tonics in comparison with each other? Excerpt: ...a larger quantity might not cure the disease. As a chronic disease usually yields slowly and gradually, it is much less necessary to continue the use of an Adenagic for some time after an apparent cure of the complaint. In fact, when a chronic disease appears to be gone, it is really so. But to this there are some exceptions, as for example, some cases of very much protracted cutaneous affections. These will some times b%cured to all appearance wholly, but return again in a short time after the suspension of the use of the remedy, at least in a moderate degree, and this several times in succession. Under these circumstances, it is necessary to resume the remedy for a short period. In one case I was obliged to continue the remedy for a twelve-month, with the result of a complete cure. From these facts it will be readily understood how important it must be to employ the medicine, in such doses and quantities as will permit its long continuance. Can a mere and pure Adenagic operation be exerted in such a degree as to constitute noxious, deleterious, poisonous, or in one word, dangerous effects; and can it be made to extinguish life? This question is not as easily answered as might at first view be imagined. The simple and pure Adenagics are in general feeble articles; while the more active ones have other powers conjoined, of such a character and in such a degree, as to prevent our distinguishing the exhaustion of the one, from that of the other. After much attention to the subject and long investigation, I have at last come to the conclusion that there is such a thing as Ultimate-Adenagy; i. e. Adenagic effects beyond the medicinal grade, which if produced with sufficient intensity, and continued long enough, may prove injurious, dangerous, ..
