Book excerpt: Intramuscular EMG of the lateral pterygoid muscles, surface EMG of the temporalis and masseter muscles, electrogoniometry and force measurements of the TMJ were synchronously used to investigate the biomechanical role of the two portions of the lateral pterygoid muscle in relation to internal derangement (ID) of the temporomandibular joint (TMJ). This study dealt with the EMG analysis of five static conditions: resting, resisted protraction, maximum voluntary contraction (MVC) in opening, in molar and incisor clenching of TMJ ID and control subjects. The analysis of variance results of the integrated linear envelop (LE) EMG showed no significant differences between the two groups. The integrated LE EMG of the SLP was significantly lower in the TMJ group during molar clenching (104$\mu$V $\pm$ 60.0 over 159$\mu$V $\pm$ 68.8 for a p =.020). The SLP seemed to have lost its discal stabilizing function during clenching. The integrated LE EMG signals of the ILP were significantly higher in the TMJ ID group during rest, resisted protraction and incisor clenching (p =.029, p =.046, p =.031 respectively). The ILP muscle has probably adapted to control the inner joint instability while continuing its own actions. The results of the isometric forces showed that TMJ ID subjects exhibited significantly lower molar bite forces (297.1N over 419N, p =.042) confirming that they have less muscle strength and tissue tolerance than subjects with healthy masticatory muscle system. Incisor bite forces, however, showed a tendency to be higher in the TMJ ID group (233N over 180.5N, p =.168), possibly resulting from the training of a protracted bite and/or hyperactivity of the ILP associated with ID. Therefore a neuromuscular adaptation could be occurring in TMJ ID masticatory system affecting muscular actions and forces. (Abstract shortened by UMI.).

Book excerpt: Temporomandibular disorders (TMDs), are a set of more than 30 health disorders associated with both the temporomandibular joints and the muscles and tissues of the jaw. TMDs have a range of causes and often co-occur with a number of overlapping medical conditions, including headaches, fibromyalgia, back pain and irritable bowel syndrome. TMDs can be transient or long-lasting and may be associated with problems that range from an occasional click of the jaw to severe chronic pain involving the entire orofacial region. Everyday activities, including eating and talking, are often difficult for people with TMDs, and many of them suffer with severe chronic pain due to this condition. Common social activities that most people take for granted, such as smiling, laughing, and kissing, can become unbearable. This dysfunction and pain, and its associated suffering, take a terrible toll on affected individuals, their families, and their friends. Individuals with TMDs often feel stigmatized and invalidated in their experiences by their family, friends, and, often, the health care community. Misjudgments and a failure to understand the nature and depths of TMDs can have severe consequences - more pain and more suffering - for individuals, their families and our society. Temporomandibular Disorders: Priorities for Research and Care calls on a number of stakeholders - across medicine, dentistry, and other fields - to improve the health and well-being of individuals with a TMD. This report addresses the current state of knowledge regarding TMD research, education and training, safety and efficacy of clinical treatments of TMDs, and burden and costs associated with TMDs. Book excerpt: The underlying nociceptive mechanisms in temporomandibular joint (TMJ) and masticatory muscles in many pain conditions are still unclear, largely due to the limited study of peripheral and central neural mechanisms affecting craniofacial musculoskeletal tissues. This study provided evidence in support of Hypothesis 1: Peripheral glutamatergic and capsaicin-sensitive mechanisms modulate the properties of primary afferents and brainstem neurons processing deep craniofacial nociceptive information. Effects of glutamate and capsaicin injected into the receptive field of deep craniofacial nociceptive afferents or TMJ of TMJ-responsive nociceptive neurons in trigeminal subnucleus caudalis/upper cervical cord (Vc/UCC) were studied in halothane-anesthetized rats. When injected alone, glutamate and capsaicin activated and induced peripheral sensitization in many afferents. Following glutamate injection, capsaicin-evoked activity was greater than that evoked by capsaicin alone, whereas following capsaicin injection, glutamate-evoked responses were similar to those of glutamate alone. When injected alone, glutamate and capsaicin also activated and induced central sensitization in most Vc/UCC neurons. Following glutamate injection, capsaicin evoked greater activity and less sensitization compared with capsaicin alone, whereas following capsaicin, glutamate was less effective in activating and sensitizing most Vc/UCC neurons. This apparent desensitizing effect of capsaicin on glutamate-evoked excitability of Vc/UCC neurons contrasts with the lack of capsaicin-induced modulation of glutamate-evoked afferent excitability, suggesting that peripheral and central sensitization may be differentially involved in the nociceptive effects of glutamate and capsaicin applied to deep craniofacial tissues. Further evidence of glutamate-capsaicin interactions was documented in the attenuation by TMJ pre-injection of glutamate receptor antagonists of jaw muscle activity reflexly evoked by TMJ injection of capsaicin. Moreover, additional findings support Hypothesis 2: Surgical cutaneous incision modulates the properties of brainstem neurons processing deep craniofacial nociceptive information. TMJ-responsive nociceptive Vc/UCC neurons could be activated by surgical incision of the skin overlying the TMJ and this incision-induced afferent barrage caused nociceptive neurons to be temporarily refractory to further capsaicin-induced central sensitization.These novel findings suggest that peripheral glutamate and capsaicin receptor mechanisms as well as surgical cutaneous incision may be involved in the nociceptive processing of deep craniofacial afferent inputs and may interact to modulate both activation as well as sensitization evoked from these tissues.
